This certification is being issued to modify Attachment A to: ♦ Approve an increase in the maximum volume of residuals that may be Iand applied from the Town of Holly Springs' wastewater treatment plant from 311.90 dry tons per year to 400 dry tons per year. Note that this approval requires that the Town of Holly Springs perform at least quarterly monitoring of the residuals to assure its quality (i.e., pollutants, r►utrients/micronutrients, and pathogen/vector attraction reduction) in accordance with 40 CFR Part 503. This certification is being issued to modify Attachment B to: Approve land application sites owned by various landowners in Chatham County and Harnett County for land application of residuals under this program. These sites, totaling 207.90 net acres, are new and are spread over five sites (i.e., Site Nos. NC-CH-012, NC-CH-100, NC- HA-504, and NC-HA-505). Please replace the existing Attachment A of Permit No. WQ0019098 that was previously certified on June 1, 2001, with this modified Attachment A. In addition, please replace the existing Attachment B of Permit No. WQ0019098 that was previously certified on November 12, 2002, with this modified Attachment B. Note that only the vicinity and buffer reaps for any new and modified residuals source -generating facilities and land application sites are attached to the modified Attachment A and Attachment B. It is the Permittee's responsibility to ensure that a complete permit is created by inserting the modified Attachment A, Attachment B, as well as their respective vicinity and buffer maps, into Permit No. WQ0019098, issued on June 1, 2001, and making sure to retain any vicinity and buffer maps for any existing and unmodified residuals source -generating facilities and land application sites. As always, remember to take the time to review the modified Attachment A and Attachment B thoroughly, as some of the conditions contained therein may have been changed since their last certification. Note that the modified Attachment A and Attachment B are hereby incorporated by reference into Permit No. WQ0019098, issued on June 1, 2001. If any parts, requirements, and/or limitations contained in the certification or modified Attachment A or Attachment B are unacceptable, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of the certification. This request shall be in the from of a written petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699- 6714. Unless such demands are made, this permit shall be final and binding.
