Barnes Environmental Residuals Land Application Program Laud Application of Residual Solids (503) Carteret County In accordance with your permit modification request received on May 18, 2010, we are forwarding herewith a modified Attachment A, certified July 8, 2010, to Barnes Environmental Waste and Hauling, Inc. for the subject residuals management and distribution program. This certification is being issued to approve additional tonnage for the two approved residuals sources, which are the Mariner's Point Wastewater Treatment Plant and the NC Aquarium — Pine Knoll Shores Wastewater Treatment Plant. The tonnage from the Mariner's Point Wastewater Treatment Plant is 2 dry tons per year and the tonnage from the NC Aquarium - Pine Knoll Shores Wastewater Treatment Plant is also 2 dry tons per year. Please replace the existing Attachment A of Permit No. WQ0018525 that was previously certified on October 15, 2009, with this modified Attachment A. It is the Permittee's responsibility to ensure that a complete permit is created by inserting this modified Attachment A into Permit No. WQ0018525, issued on October 15, 2009. This modified Attachment A shall be effective from the date of certification; shall void Attachment A of Permit No. WQ0018525 that was previously (most recently) certified on October 15, 2009; and shall be subject to the conditions and limitations as specified therein and well as those specified in the above -referenced permit, As always, remember to take the time to review this modified Attachment A thoroughly, as some of the conditions contained therein may have been changed since the last certification. Note that this modified Attachment A is hereby incorporated by reference into Permit No. WQ0018525, issued on October 15, 2009. If any parts, requirements, and/or limitations contained in this certification or modified Attachment A are unacceptable, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of the certification.
