Carteret County Dear Mr. Barnes: In response to a request received on May 7, 2014 from Martin Mabe of S&ME, Inc. to expedite permission to apply residuals onto two new application fields, we are forwarding herewith modified Attachments B, certified May 12, 2014 to Barnes Environmental Waste and Hauling, Inc. for the subject residuals program. This certification is being issued to modify Attachment B by adding Fields WT-05 and WT-06 to the list of approved application sites in Attachment B. This modified Attachment B shall be effective from the date of certification until July 31, 2014 (i.e. expiration date of the current permit); shall void Attachment B of Permit No. WQ0018525 that was previously certified on October 15, 2009, and shall be subject to the conditions and limitations as specified therein and well as those specified in the above -referenced permit. Please replace the Attachment B of Permit No. WQ0018525 that was issued on October 15, 2009 with this modified Attachment B. Please note that only the reaps for any new residuals -generating sources and/or land application sites are attached to this letter. It is the Perznittee's responsibility to ensure that a complete permit is created by inserting these documents into the previously issued permit. If any parts, requirements, and/or limitations contained in this certification or modified Attachment B are unacceptable, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of the certification. This request shall be in the form, of a written petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714.
