This modification includes changing the sampling point for fecal coliform and increasing the instantaneous loading rate on soils in Zone 4. Please take the time to review this permit thoroughly as some of the conditions contained therein may have been added or changed since the last issuance of this permit. Of special interest are the following: Condition 11.8 - Note that within 90 days of permit issuance the permittee shall submit a detailed map identifying the acreage of wetted area for all irrigation zones for both the Farm and the Golf Course This permit shall be effective from the date of issuance until May 31, 2011, shall void Permit No. WQ0017791 issued June 28, 2006, and shall be subject to the conditions and limitations as specified therein. Please pay particular attention to the monitoring requirements in this permit. Failure to establish an adequate system for collecting and maintaining the required operational information will result in future compliance problems. If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right to request an adjudicatory hearing upon written request within thirty (30) days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings, 6714 Mail Service Center, Raleigh, NC 27699-6714. Reclaimed water, following treatment and testing according to the most recent issuance of the Goldsboro Water Reclamation Facility's NPDES permit and compliance with the conditions provided herein, may be diverted to the permitted reclaimed water distribution, storage, and utilization facilities for the purpose of supplying a reclaimed water demand not to exceed 495,500 gallons per peak day to a spray irrigation site known as "The Farm" and not to exceed 30,000,000 gallons per year to approved areas within the Goldsboro Municipal Golf Course. The reclaimed water distribution, storage, and utilization facilities shall consist of the following minimum components: approximately 4,900 linear feet of existing 24-inch reclaimed water distribution main and approximately 4,300 linear feet of 12-inch reclaimed water distribution main to convey reclaimed water from the Goldsboro Water Reclamation Facility to either the constructed wetlands treatment system or the remote reclaimed water storage and distribution facilities; a reuse tank influent valve vault; an 300,000- gallon covered, vented, ground reuse storage tank with audible and visual high water alarms and a 40 horsepower (hp) pump, approximately 15 linear feet of 12-inch fill piping, and approximately 100 linear feet of 12-inch suction piping; a reclaimed wastewater pump station for "the Farm" with audible and visual high-water alarms and two (2) 75 hp constant speed, horizontal split -casing pumps each with a design operating point of 875 gallons per minute (GPM) at 201 feet of total dynamic head (TDH); a golf course reuse flow meter and valve vault; a spray irrigation flow meter and valve vault; approximately 2,300 linear feet of 12-inch reclaimed water distribution main to convey reclaimed water to a spray irrigation site or to the booster pump station at the golf course; Approved irrigation onto "the Farm", a 144-acre spray irrigation site consisting of approximately 91 acres of wetted area, 16 solid -set irrigation zones each containing four fully -adjustable, big gun sprinklers that are capable of delivering a maximum of 225 GPM at 80 pounds per square inch (psi) to effect a maximum 308-foot diameter and are set on an overlapping pattern on five-foot high risers, approximately 9,000 linear feet of I0-inch distribution piping, approximately 3,200 linear feet of 12-inch distribution piping, Connection of reuse wastewater to the booster pump station at the golf course with appropriate back flow prevention to protect the fresh water supply to the pump station and a rain sensor to prevent irrigation during precipitation. Approved irrigation onto the Goldsboro Municipal Golf Course, utilizing an existing irrigation system including approximately 67.55 wetted acres on fairways, approximately 22.10 wetted acres on tees, approximately 17.99 wetted acres on perimeter areas, and approximately 3.52 wetted acres on greens for a total of 111.16 wetted acres, and to include a rain sensor system to discontinue irrigation during precipitation events. Automatic irrigation control valves; flood protection of, all weather access to, and site restriction of the reclaimed water distribution, storage, and utilization facilities; as well as all meters, valves, fittings, piping, instrumentation/control systems, electrical items, structural units, and other related appurtenances required to make a complete and operational reclaimed water distribution, storage, and utilization facilities. All instrumentation, controls, and telemetry associated with the reclaimed water distribution, storage, and utilization facilities shall be connected to and automatically -activated to operate from the existing instrumentation, controls, and telemetry at the Goldsboro Water Reclamation Facility for the Farm. All golf course reclaimed water controls will be located at the booster pump station at the golf course. The Reclaimed wastewater system shall be designed and operated with no discharge of wastes to the surface waters, with the exception of known stormwater drainage grates that divert water to a stormwater drainage ditch on site, pursuant to the application received February 2, 2006 and subsequent additional information received by the Division, and in conformity with the project plan, specifications, and other supporting data subsequently filed and approved by the Department of Environment and Natural Resources and considered a part of this permit. This permit shall be effective from the date of issuance until May 31, 2011, shall void Permit No. WQ0017791 issued June 28, 2006, and shall be subject to the following specified conditions and limitations: I. PERFORMANCE STANDARDS 1. The reclaimed water utilization facilities shall be effectively maintained and operated at all times so that there is no discharge to the surface waters, nor any contamination of ground waters, which will render them unsatisfactory for normal use. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions or failure of the irrigation area to adequately assimilate the wastewater, the Permittee shall take immediate corrective actions including those actions that may be required by the Division of Water Quality (Division), such as the construction of additional or replacement wastewater treatment and disposal facilities. 2. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to surface water or ground water resulting from the operation of this facility. The residuals generated from these treatment facilities must be disposed in accordance with General Statute 143-215.1 and in a manner approved by the Division. 4. Diversion or bypassing of the untreated wastewater from the treatment facilities is prohibited. Reclaimed water shall be treated in the Goldsboro Water Reclamation Facility (i.e., NPDES Permit No. NC0023949) prior to distribution, storage, and utilization. Reclaimed water shall not be utilized unless it meets the limits stipulated below. Effluent not meeting these limits shall either be discharged in accordance with the most recent issuance of the NPDES permit or reintroduced to the headworks of the Goldsboro Water Reclamation Facility for further treatment. The monthly average concentrations for all parameters except fecal coliform shall be the arithmetic mean of all samples collected during the reporting period [i.e., see Condition b The daily maximum concentrations for all parameters shall be the maximum value of all samples collected during the reporting period [i.e., see Condition III(3)]. Up to 495,500 gallons per day may be distributed from the Goldsboro Water Reclamation Facility for use on the spray irrigation site known as "the Farm". Additional reclaimed water may be distributed into the reclaimed water distribution and storage facilities; however, any volume over not able to be irrigated shall be discharged in strict accordance with the most recent issuance of NPDES Permit No. NC0023949, which includes the option to discharge any excess reclaimed water to the constructed wetlands treatment system approved for construction in Authorization to Construct No. 023949ACA, issued by the Division on June 29, 2001. Furthermore, note that this permit is a conjunctive permit and, as such, shall not change the hydraulic capacity of the Goldsboro Reclaimed Water Facility as permitted in the most recent issuance of NPDES Permit No. NC0023949. There shall be no average monthly limit for irrigation at the Goldsboro Municipal Golf Course, however the annual irrigation amount shall not exceed 30 million gallons_ Daily irrigation amounts shall be in accordance with Condition U (7) e The monthly average concentrations for fecal coliform shall be the geometric mean of all samples collected during the reporting period [i.e., see Condition I11(3)]. 6. Should reclaimed water be stored in the reclaimed water storage facilities for a prolonged period of time such that it may be reasonably expected that it would not meet the reclaimed water duality criteria as stipulated in Condition I(5), the Permittee shall either monitor the stored reclaimed water for compliance with these criteria prior to utilization on the spray irrigation site or discharge it in strict accordance with the conditions of the most recent issuance of NPDES Permit No. NC0023949. 7. The following shall be requirements for the reclaimed water distribution, storage, and utilization facilities: a. All reclaimed water valves, storage facilities, and outlets shall be tagged or labeled to warn the public or employees that the water is not intended for drinking. Where appropriate, such warning shall inform the public or employees to avoid contact with the water. b. All reclaimed water piping, valves, outlets, and other appurtenances shall be color -coded, taped, or otherwise marked to identify the source of the water as being reclaimed water. i. All reclaimed water piping and appurtenances shall be either colored purple (i.e., Pantone 522) and embossed or integrally stamped or marked "CAUTION: RECLAIMED WATER — DO NOT DRINK" or be installed with a purple (i.e., Pantone 522) identification tape or polyethylene vinyl wrap. The warning shall be stamped on opposite sides of the pipe and repeated every three feet or less. ii. Identification tape shall be at least three inches wide and have white or black lettering on purple (i.e., Pantone 522) field stating "CAUTION: RECLAIMED WATER — DO NOT DRINK." Identification tape shall be installed on top of reclaimed water pipelines, fastened at least every 10 feet to each pipe length and run continuously the entire length of the pipe. iii. Existing underground distribution systems to be retrofitted at the Goldsboro Municipal Golf Course shall be taped or otherwise identified as noted above. This identification need to extend the entire length of the distribution system but shall be incorporated within 10 feet of crossing any potable water supply line or sanitary sewer line. c, All reclaimed water valves and outlets shall be of a type, or secured in a manner, that permits operation by authorized personnel only. d. Above -ground hose bibs (i.e., spigots or other hand -operated connections) shall not be present. Hose bibs shall be located in locked below -grade vaults that shall be clearly labeled as being of non -potable quality. As an alternative to the use of locked below -grade vaults with standard hose bibs services, hose bibs, which can only be operated by a special tool or connected to a special hose connection may be placed in non -lockable underground services boxes clearly labeled as non -potable water. 8. The Permiittee shall maintain a spill prevention, and control plan for the reclaimed water distribution, storage, and utilization facilities. 9. The following buffers shall be maintained: a. 100 feet between wetted areas and water supply wells, b. 10 feet between wetted areas and non -potable wells, c. 100 feet between wetted areas and waters classified as SA, including wetlands as delineated and designated by the appropriate state or federal agency, and d. 25 feet between wetted areas and surface waters not classified as SA. 10. Public access to the irrigation sites shall be controlled during active site use. Such controls may include the posting of signs showing the activities being conducted at each site. A sign shall be posted in plain sight in the clubhouse showing these activities. 11. The irrigation system at the Goldsboro Municipal Golf Course shall be connected to a rain or moisture sensor that shall indicate when wastewater application is not appropriate in accordance with Condition II(4) of this permit. 4 11. OPERATION AND MAINTENANCE REQUIREMENTS 1. The facilities shall be properly maintained and operated at all times. 2. Upon classification of the wastewater treatment and irrigation facilities by the Water .Pollution Control System Operators Certification Commission (WPCSOCC), the Permittee shall designate and employ a certified operator to be in responsible charge (ORC) and one or more certified operator(s) to be back-up ORC(s) of the facilities in accordance with 15A NCAC 8G .0201. The ORC shall visit the facilities in accordance with 15A NCAC 8G .0204 or as specified in this permit and shall comply with all other conditions specified in these rules. 3. A suitable vegetative cover shall be maintained on the irrigation areas. Reclaimed wastewater shall only be applied to axeas with actively growing vegetation. No irrigation shall occur during dormant periods of any permanent cover. 4. irrigation shall not be performed during inclement weather or when the ground is in a condition that will cause runoff: 5. The facilities shall be effectively maintained and operated as a non -discharge system to prevent the discharge of any wastewater resulting from the operation of this facility. 6. The sprinkler identification system listed in this table identifies the location of any individual sprinkler within the spray irrigation site. The number before the hyphen corresponds to the irrigation zone (i.e., 16 total) as shown in the approved plans. The letter after the hyphen corresponds to the location of each of the four sprinklers within the specific irrigation zone. The letter is assigned consecutively to the sprinkler is a direction from cast to west and north to south across the zone. For example, Sprinkler No. 8-a is the northernmost and easternmost sprinkler in Irrigation Zone No. 8, while Sprinkler No. 8-d is the southernmost and westernmost sprinkler in Irrigation Zone No. 8. Each sprinkler or irrigation zone shall be individually calibrated such that it does not cause the exceedance of the stipulated maximum cumulative and instantaneous application rates for the most -restrictive soil zone that it impacts (i.e., some sprinklers may impact area in more than one soil zone). The cumulative application rate shall be equivalent to the loading of reclaimed water over any 12-month period. d These sprinklers impact wetted areas in both Soil Zone No. 2 and Soil Zone No. 3. Either the individual sprinkler or the irrigation zone as a whole shall be calibrated to irrigate reclaimed water at Soil Zone No. 2 rates even though a portion of the wetted area is in Soil Zone No. 3. The wetted areas of these sprinklers shall be adjusted such that they do not impact any area that has been delineated as being within Soil Zone No. 1. 7. Within ninety (90) days of permit issuance, the Permittee shall submit to the Division of Water Quality, Aquifer Protection Section, 1636 Mail Service Center, Raleigh, NC 27699-1636 a detailed Iisting of all irrigation zones, wetted acreage in each irrigation zone, and soil type (as noted above) in each irrigation zone_ Irrigation zones shall be delineated so as to match how they will be routinely operated and how disposal records will be submitted by the Permittee. No type of wastewater other than effluent from the Goldsboro Water Reclamation Facility shall be distributed, stored, or irrigated onto the irrigation area. 10. All wastewater shall be routed to the headworks of the Goldsboro Water Reclaimation Facility or discharged in strict accordance with the conditions of the most recent issuance of NPDES Permit No. NCO023949 should the limit for fecal coliform (daily maximum concentration of 25 per 100 ml) or turbidity (instantaneous maximum of 10 NTU) be exceeded, until such time that the problems associated with the treatment capability of the wastewater treatment plant have been corrected. 11. The Permittee shall implement adequate measures to prevent reclaimed water runoff from the irrigation sites, with the exception of known storm drainage grates within the Goldsboro Municipal Golf Course that outlet to the stormwater drainage ditch. 12. The Permittee shall implement adequate measures to mitigate drift of airborne reclaimed water from the "Farm" site during active use. Of special concern is the potential for drift from the site to impact vehicular traffic along Dixie Trail and Westbrook Road. Therefore, the Permittee shall take the following steps: a. Install wind direction and velocity meters at the Goldsboro Water Reclamation Facility. The information gathered from these meters shall be correlated with the observations/complaints of airborne reclaimed water drift. The resultant data shall be used to establish a standard operating procedure with respect to zone use and periods of irrigation. Should the above steps fail to mitigate any airborne reclaimed water drift problem identified by the Permittee or the Division, one or more of the following additional steps shall be implemented to further mitigate or eliminate this problem: • Adjust sprinklers in those irrigation zones closest to Dixie Trail and Westbrook Road to reduce their throw diameters. • Weave blocking slats into the fencing along the perimeters of the spray irrigation site immediately adjacent to Dixie Trail and Westbrook Road. • Restrict irrigation periods for those irrigation zones closest to Dixie Trail and Westbrook Road to night hours and other times of minimal vehicular traffic. • Restrict use of sprinklers in those irrigation zones closest to Dixie Trail and Westbrook Road completely. • Plant rapid growth vegetation (e.g. Tulip Poplar or Leland Cypress) along the perimeters of the spray irrigation site immediately adjacent to Dixie Trail and Westbrook Road. III. MONITORING AND REPORTING REQUIREMENTS Any monitoring (including groundwater, surface water, soil or plant tissue analyses) deemed necessary by the Division to insure surface and ground water protection will be established and an acceptable sampling reporting schedule shall be followed. 2. All flow measurement devices shall be consistent with approved engineering and scientific practices to ensure the accuracy and reliability of flow measurement. Selected devices shall be capable of measuring flows with a maximum deviation of less than 10 percent from true flow, accurately calibrated at a minimum of once per year, and maintained to ensure that the accuracy of measurement is consistent with the accepted capability of the type of device used. Records of flow measurement device calibration shall be kept on file by the Permittee for a period of at least three years. This documentation shall include the following minimum data: a. Date of flow measurement device calibration b. Name of person performing calibration c. Percent from true flow 7 3. Twice per Month Composite Fecal Coliform Reuse Storage Tank Twice per Month' Grab pH Reuse Pump Station Twice per Month Grab a This sampling point refers to the reuse pump station located at the remote reclaimed water storage and distribution facilities permitted for construction and operation with this permit. b This sampling point refers to the reuse pump station located at the Goldsboro Water Reclamation Facility that has been previously approved for construction by the Division on February 20, 2001 in Authorization to Construct No. 0023949ACC. The two time per month sampling frequency shall be performed only during the months of April through October, inclusive. During the remainder of the year, these parameters shall be monitored monthly. Representative sampling for the listed parameters shall be required at -the Reuse Storage Tank any time that reclaimed water is stored for a prolonged period of time [i.e., see Condition 1(6)] and compliance with the reclaimed water quality criteria [i.e., see Condition I(7)) shall be established prior to utilizing the reclaimed water on the spray irrigation site. 4. Adequate records shall be maintained by the Permittee tracking the amount of wastewater disposed. These records shall include, but are not necessarily limited to the following information: a. Date and time of irrigation, b. Volume of reclaimed wastewater irrigated, c. Locations and zone irrigated, d. Length of time zone is irrigated, e. Continuous weekly, monthly, and year-to-date hydraulic (inches/acre) loadings for each zone, f. Weather conditions, g. Maintenance of cover crops, and h. Compliance status of water reclamation facility. 5. Three (3) copies of all monitoring data [as specified in Condition III(3)] on Form NDMR-1 and three (3) copies of all operation and disposal records [as specified in Condition I1I(4)] on Form NDAR-1 shall be submitted monthly on or before the last day of the following month. Noncompliance Notification: The Permittee shall report by telephone to the Washington Regional Office, telephone number (252) 946-6481, as soon as possible, but in no case more than 24 hours or on the next working day following the occurrence or first knowledge of the occurrence of any of the following: Any occurrence at the wastewater treatment facility which results in the treatment of significant amounts of wastes which are abnormal in quantity or characteristic, such as the dumping of the contents of a sludge digester; the known passage of a slug of hazardous substance through the facility; or any other unusual circumstances. b. Any process unit failure, due to known or unknown reasons, that render the facility incapable of adequate wastewater treatment such as mechanical or electrical failures of pumps, aerators, compressors, etc. c. Any failure of a pumping station, sewer line, or treatment facility resulting in a by-pass directly to receiving waters without treatment of all or any portion of the influent to such station or facility. d. Any time that self -monitoring information indicates that the facility has gone out of compliance with its permit limitations. Occurrences outside normal business hours may also be reported to the Division's Emergency Response personnel at telephone number (800) 662-7956, (800) 858-0368, or (919) 733-3300. Persons reporting such occurrences by telephone shall also file a written report in letter form within five (5) days following first knowledge of the occurrence. This report must outline the actions taken or proposed to be taken to ensure that the problem does not recur. IV. GROUNDWATER REQUIREMENTS A piezometer well shall be maintained in Soil Zone No. 1 to monitor the level of the groundwater. The general location and name for the well shall be determined and approved by the Aquifer Protection Section of the Division's Washington Regional Office. The piezometer well shall be monitored prior to initiating any waste disposal activities and monthly thereafter to determine the water level only. The depth to the water table in the well shall be measured from the surveyed point on the top of the well casing. The measuring point (i.e., top of well casing) shall be surveyed relative to a common datum. The results of the sampling and analysis shall be received on the GW-59 Form on or before the last working day of the month following the sampling month at the following address: NCDENR-DWQ Information Processing Unit 1617 Mail Service Center Raleigh, North Carolina 27699-1617 The data of all groundwater monitoring shall be reported using the most recent GW-59 Form. Updated forms (i.e., GW-1 Form, GW-30 Form, and GW-59 Form) may be downloaded from the website at http:/ih2o.enr.state.nc.us/gps/gpu/foirns.htm or requested from the address above. Waste disposal activities shall not occur when the vertical separation between the land surface and the water table is less than one foot. Verification of the water table elevation shall be confirmed by water level readings obtained from the piezometer well or by auger borings, which shall be done within 24 hours, prior to any waste disposal event. Any open borings shall be properly filled in with native soil prior to initiating the waste disposal event in order to decrease the chance of groundwater contamination. 4. The COMPLIANCE BOUNDARY and REVIEW BOUNDARY for the waste disposal area shall be as specified by 15A NCAC 2H .0219(k)(2)(B)(i)(I11). The Compliance Boundary and the Review Boundary shall be established at the property boundary. Any exceedance of groundwater quality standards at or beyond the Compliance Boundary is subject to immediate remediation action in accordance with 15A NCAC 2L .0106(d)(2). 5. Any additional groundwater quality monitoring, as deemed necessary by the Division, shall be provided. V. INSPECTIONS I. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to insure proper operation of the subject facilities. 2. The Permittee or his designee shall inspect the wastewater treatment and disposal facilities to prevent malfunctions and deterioration, operator errors and discharges which may cause or lead to the release of wastes to the environment, a threat to human health, or a nuisance. The Permittee shall keep an inspection log or summary including at least the date and time of inspection, observations made, and any maintenance, repairs, or corrective actions taken by the Permittee. This log of inspections shall be maintained by the Permittee for a period of three years from the date of the inspection and shall be made available upon request to the Division or other permitting authority. 10 3. Any duly authorized officer, employee, or representative of the Division may, upon presentation of credentials, enter and inspect any property, premises or place on or related to the disposal site or facility at any reasonable time for the purpose of determining compliance with this permit; may inspect or copy any records that must be maintained under the terms and conditions of this permit, and may obtain samples of groundwater, surface water, or leachate. VL GENERAL CONDITIONS This permit shall become voidable unless the facilities are constructed in accordance with the conditions of this permit, the approved plans and specifications, and other supporting data. 2. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. This permit is not transferable. In the event there is a desire for the facilities to change ownership, or there is a name change of the Permittee, a formal permit request must be submitted to the Division accompanied by an application fee, documentation from the parties involved, and other supporting materials as may be appropriate. The approval of this request will be considered on its merits and may or may not be approved. 4. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to an enforcement action by the Division in accordance with North Carolina General Statute 143-215.6A to 143-215.6C. The issuance of this permit does not exempt the Permittee from complying with any and all statutes, rules, regulations, or ordinances which may be imposed by other government agencies (local, state, and federal) which have jurisdiction, including but not limited to applicable river buffer rules in 15A NCAC 2B.0200, erosion and sedimentation control requirements in 15A NCAC Chapter 4 and under the Division's General Permit NCGOlO000, and any requirements pertaining to wetlands under 15A NCAC 2B .0200 and 2H .0500. 6. The Permittee shall retain a set of approved plans and specifications for the subject facility for the life of the project. 7. The Permittee shall pay the annual administering and compliance fee within thirty days of being billed by the Division. Failure to pay the fee accordingly may cause the Division to initiate action to revoke this permit as specified by 15A NCAC 02T .0105(e). The Permittee, at least six (6) months prior to the expiration of this permit, shall request its extension. Upon receipt of the request, the Commission will review the adequacy of the facilities described therein, and if warranted, will extend the permit for such period of time and under such conditions and limitations as it may deem appropriate. 9.
