In accordance with a telephone request received. on August 24, 2001 from Mr. Daniel B. Schmidt, P.E.,
acting on the City of Goldsboro's (City) behalf, we are forwarding herewith a corrected Page 2 of Permit No.
WQ0017791 that was issued on July 13, 2001 and an amended Sheet C-15 of the plans that were stamped
approved on July 13, 2001 for the subject reclaimed water distribution, storage, and utilization facilities.
The following correction/amendment has been made to the permit:
1. Mr. Schmidt related that the cost for construction of the facilities submitted by the lowest bidder
was stiII higher than that budgeted by the City. Negotiations to bring down the contract price were
successful; however, one of the items that was omitted from the project scope was the grading of the
spray irrigation site.
The proposed grading of the site was actually of great concern to the Division of Water Quality
(Division) because of its potential to change the surface soil characteristics significantly and,
therefore, the ability of the soils to assimilate reclaimed water at the maximum hydraulic loading
rates recommended by the North Carolina -licensed professional soil scientist during design and
permitting of the facilities. It is for this reason that the Division included Condition I. 3., which
required an "as -built" site -specific soil evaluation to be performed and submitted at least 60 days
prior to operation of the facilities, in the permit.
With the elimination of the grading requirement from the design, Sheet C-15 of the construction
plans has been amended by the North Carolina -licensed professional engineer. This sheet, which
has been stamped as approved by the Division, should replace the existing Sheet C-15 in the plans
that were approved by the Division. Also, the Division has amended Condition I. 3. of the permit to
require that a certification be received from a North Carolina -licensed professional soil scientist to
verify that the site, in its post -construction state, can accept the maximum hydraulic loading rates
originally recommended and approved. An evaluation is now only required if the North Carolina. -
licensed professional soil scientist is unable to make this certification.

All 'instrumentation, controls, and telemetry associated with the reclaimed water distribution, storage, and
utilization facilities shall be connected to and automatically -activated to operate from the existing instrumentation,
controls, and telemetry at the Goldsboro Water Reclamation Facility.
The reclaimed water distribution, storage, and utilization facilities shall be constructed and operated such
that no discharge of wastes to surface waters occurs, pursuant to the application received on December 15, 1999 as
well as all additional information subsequently received, and in conformity with the project plans, specifications,
and other supporting data subsequently filed and approved by the Department of Environment and Natural
Resources and considered a part of this permit.
This permit shall be effective from the date of issuance until June 30, 2006 and shall be subject to the
following specified conditions and limitations:
1. PERFORMANCE STANDARDS
1. An existing 24-inch force main that has been historically used to convey raw, municipal wastewater
from the Westbrook Pump Station to the Goldsboro Water Reclamation Facility shall be properly
cleaned and disinfected prior to its use for conveying reclaimed water from the Goldsboro Water
Reclamation facility to the reclaimed water storage and utilization facilities. The certification from the
North Carolina -licensed professional engineer required in Condition I. 4. shall serve as proof of
compliance with this condition.
2. Operation of the reclaimed water distribution, storage, and utilization facilities shall be strictly
prohibited until the turbidimeter previously approved by the Division on February 20, 2001 in
Authorization to Construct No. 0023949ACC has been installed at the Goldsboro Water Reclamation
Facility and its operation has been successfully initiated. The certification from the North Carolina -
licensed professional engineer required in Condition 1. 4. shall serve as proof of compliance with this
condition.
Extreme care shall be taken during clearing and subsequent construction on the spray irrigation site to
minimize any loss in the site's pre -construction infiltrative capacity due to compaction, grading, etc.
Upon completion of construction and prior to operation of the reclaimed water distribution, storage,
and utilization facilities, a certification shall be received from a North Carolina -licensed professional
soil scientist that verifies that the site, "as built," can still accept the maximum hydraulic loading rates
stipulated in Condition U. 8. If such a certification cannot be prepared, the Permitnee shall submit a
report prepared as well as signed, sealed, and dated by the North Carolina -licensed professional soil
scientist that provides a final recommendation of hydraulic loading rates (i.e., both instantaneous and
cumulative) for the site that is reflective of the "as -built" conditions. This permit may be modified at
that time to address any differences between the rates currently approved in Condition II. 8_ and the
new "as -built" rates. Mail the Certification or report to the NCDENR-DWQ, Non -Discharge
Permitting Unit, 1617 Mail Service Center, Raleigh, NC 27699-1617.
4. Upon completion of construction and prior to operation of the reclaimed water distribution, storage,
and utilization facilities, a certification shall be received from a North Carolina -licensed professional
engineer certifying that the facilities have been installed in accordance with this permit, the approved
plans and specifications, and other supporting materials. If the facilities are to be com-pleted in phases
and partially certified, the Permittee shall retain the responsibility to track further construction
approved under the same permit, and shall provide a final certificate of completion once all facilities
