In accordance with the additional information furnished to our agency on November 1, 2013 in
accordance with the Permit Conditions stated in existing Permit WQ0016966 issued on October 25, 2013,
we are forwarding herewith a modified Attachment B, certified November 13, 2013, to the Town of
Denton for the subject residuals Iand application program, along with a revised Figure 2.
This certification is being issued to modify Attachment B to:
• Approve the application of residuals onto four land application sites owned by Daryl
Wood in Davidson County totaling 28.10 acres for land application of residuals under
this program. Application was not allowed onto these sites until revised acreages and site
detail maps were furnished to our agency.
Please replace the existing Attachment B of Permit No. WQ0016966 that was previously
certified on October 25, 2013, with this modified Attachment B. Note that the buffer map for these
modified land application sites are attached to this modified Attachment B. It is the Permittee's
responsibility to ensure that a complete permit is created by inserting this modified Attachment B and
vicinity and the modified buffer map (Figure 2), which replaces Figures 2 through 6, into Permit No-
WQ0016966, issued on October 25, 2013, and making sure to retain any vicinity and buffer maps for any
existing and unmodified land application sites -
This modified Attachment B shall be effective from the date of certification; shall void
Attachment B of Permit No. WQ0016966 that was previously certified on October 25, 2013; and shall be
subject to the conditions and limitations as specified therein and well as those specified in the above -
referenced permit. As always, remember to take the time to review this modified Attachment B
thoroughly, as some of the conditions contained therein may have been changed since the last

certification. Note that this modified Attachment B is hereby incorporated by reference into Permit No.
WQ0016966, issued on October 25, 2013.
If any parts, requirements, and/or limitations contained in this certification or modified
Attachment B are unacceptable, you have the right to request an adjudicatory hearing upon written
request within 30 days following receipt of the certification. This request shall be in the form of a written
petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of
Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such
demands are made, this permit shall be final and binding.
