Dear Mr. Andrews:
A review of our records shows permit No. WQ0016388 was issued on March 1, 1999 for the
land application of sludge generated by your wastewater treatment facility. A review of our files
conducted by Mrs. Ellen Huffman with this Office indicate that the Division of Water Quality has
not received monitoring reports as required by Section III, parts 3 and 4 of your permit.
It is requested that Alchem, Inc. respond to this Notice within ten (10) working days of receipt of
this letter. Your response should include mitigating circumstances for the violation and a discussion of
any new procedures that will be implemented to prevent recurrence. Please reference the subject permit
number when providing your response.
Please be aware that Alchem, Inc. may be assessed civil penalties in this matter pursuant to G.S.
143-215.6A(a)(2), which provides that a civil penalty of not more than twenty five thousand dollars
(S25,000.00) per violation per day may be assessed against a person who violates any permit
condition, classification, standard, limitation, or management practice established pursuant to G.S.
143--214.1, 143-214.2 or 143-215.
