The following corrections have been made to the permit: 1. Total Ammonia (NH4 — as N) has been removed from the surface water monitoring parameters. Please replace page 4 with the corrected page 4 into Permit No. WQ0016376 issued June 27, 2000. The effluent from the subject facilities shall be monitored by the Permittee at the point prior to irrigation monthly for the following parameters: BODY TSS Fecal Coliform pH NH3 as N TKN NO3 as N Total Phosphorous PAN by calculation 5. Surface monitoring of the adjacent Unnamed Tributary to Turkey Creek at the point where the tributary first enters the property and at the downstream point where the tributary leaves the property shall be conducted monthly for the following parameters: NO3 - as N TKN NH3 as N Fecal Coliforms Total Phosphorous Monitoring and analysis shall be conducted in accordance with 15NCAC 2B .0505(e)(4) and analysis must be conducted by a DWQ Certified Laboratory as per 15NCAC 2H .0800. 6. Three (3) copies of all operation and disposal records (as specified in condition III.2 and II1.3) on Form NDAR-1 shall be submitted on or before the last day of the following month. Three (3) copies of all effluent monitoring data (as specified in condition 1114) on Form NDMR-I shall be submitted on or before the last day of May, September, and January. Three (3) copies of all surface water- monitoring (as specified in Condition 111.5) shall be submitted on or before the last day of the following month. A representative annual soils analysis (Standard Soil Fertility Analysis) shall be conducted on each spray field and the results maintained on file by the Permittee for a minimum of five years. The Standard Soil Fertility Analysis shall include, but is not necessarily limited to, the following parameters: Acidity Manganese Potassium Calcium Percent Humic Matter Sodium Copper pH Zinc Magnesium Exchangeable Sodium Percentage Phosphorus Cation Exchange Capacity Base Saturation (by calculation) 8. An annual report shall be submitted to the Division outlining the operation and maintenance techniques used to prevent nuisance conditions. This report shall include, but not be limited to: Operation and Maintenance Schedule(s) Equipment repair and replacement Odor Control measures Complaints lodged by citizens 4
