Dear Ms. Mangonon:
The Aquifer Protection Section (APS) of the North Carolina Division of Water Quality (DWQ) is entrusted to
protect the groundwater quality and resources of the State of North Carolina. Our records indicate that the above -
referenced permit, which was issued on I I /10/2008, and expires on 6/30/2009, has not been renewed.
This permit was issued for the operation of the surface waste irrigation system on your property located at 2031
Harris Road, Rougemont, NC 27572. As the property owner, you are responsible for maintaining this permit.
Failure to do so may result in the revocation of the permit per 15A NCAC 2T.01 10 or assessment of civil penalties
in accordance with North Carolina General Statute §143-215.6A. Therefore, it is important that you submit a
renewal application form and an Operations and &Maintenance Agreement (O&M) within 30 days of receipt of this
letter. For your convenience, a renewal application and an O&M form are attached along with a self-addressed
envelope. These forms are also available on-line at the DWQ website at
hM2://h2o.enr.state.nc.us/lau/applications.htm] under Single Family Residence Wastewater Irrigation System.
Please respond to this notice within 30 calendar days of its receipt by selecting one of the following options:
1. APPLY FOR PERMIT RENEWAL. When you apply for and receive a renewal, operation
of your surface irrigation system will be permitted to continue. As such, all sampling
requirements, the payment of annual fees, and any operation and maintenance procedures
specified in your previous permit will be required. If you choose to renew your surface
irrigation system you must submit the following information:
a) a completed SINGLE FAMILYRESIDENCEPERMITRENEWALAPPLICATION (form WWR 09-
06) (Enclosure 1). This form must be signed by each property/permit owner.
b) a completed SINGLE FAMILY RESIDENT OPERATION AND MAINTENANCEAGREEMENT (form
SFR O&M 07/07) (Enclosure 2). This form must be signed by each property/permit
owner.

2. APPLY FOR PERMIT RESCISSION. If the surface irrigation system has not been
constructed, or if it is no longer active, you may choose to rescind your permit by submitting
the following information:
a) a completed Single Family Residence Permit Renewal Application (form WWR 09-06)
(Enclosure 1). This form must be signed by each property/permit owner. Please indicate
your wish to rescind this permit in Part II, Item 4 of this form.
Please submit the completed forms within 30 days receipt of this letter to the following address:
