In accordance with a telephone request received on October 17, 2002, we are forwarding
herewith, corrected pages 1, 4, 5, 6, 7, and the Engineer's Certification of Permit No.
WQOO 14046, issued June 14, 2002, for the subject facility. The following corrections have been
made to the permit:

Carolina as amended, and other applicable Laws, Rules, and Regulations

7,000 GPD) wastewater treatment and disposal facility consisting of an operations/lab building, —�1
with a septic tank and S.T.E.P. system, an influent flow meter, a 2,530,000 -gallon facultative
lagoon (36.7 days of treatment), a 5,370,000 -gallon storage lagoon (approximately 76.8 days of
storage), an effluent structure with a 2.4 GPH sodium hypochlorite feed pump, a 340 GPM
irrigation pump station with dual pumps, high-water alarms, approximately 28.75 acres of
existing spray irrigation fields (divided into 7 irrigation zones), and a new 396 acre irrigation
field (zone no. 8) to serve the town of Stovall, including Stovall Elementary School, with no
discharge of wastes to the surface waters, pursuant to the application received September 21,
2002, and in conformity with the project plan, specifications, and other supporting data
subsequently filed and approved by the Department of Environment and Natural Resources, and
considered a part of this permit.
This permit shall be effective from the date of issuance until May 31, 2007, shall void
Permit No. WQ0014046, issued October 3, 1997, and shall be subject to the following specified
conditions and limitations:
I. PERFORMANCE STANDARDS
I. Upon completion of construction and prior to operation of this permitted facility, a
certification (see attached Certification Form) must be received from professional
10. No type of wastewater other than that from the town of Stovall's sewer system
including Stovall Elementary School shall be sprayed onto the irrigation area.
I I . No traffic or equipment shall be allowed on the disposal area except while
installation occurs or while normal maintenance is being performed.
12. Public access to the Iand application sites shall be controlled during active site use.
Such controls must include the posting of signs showing the activities being
conducted at each site.
III. MONITORING AND REPORTING REQUIREMENTS
Influent flow shall be continuously monitored and daily flow values shall be
reported on Form NDMR.
The Permittee shall install an appropriate flow measurement device consistent with
approved engineering and scientific practices to ensure accuracy and reliability of
influent flow measurement. Flow measurement devices selected skull be capable of
measuring flows with a maximum deviation of less than 10 percent from true flow,
accurately calibrated at a minimum of once per year, and maintained to ensure that
the accuracy of the measurement is consistent with the accepted capacity of that
type of device. Records of flow measurement device calibrations shall be kept on
file by the Permittee for a period of at least three years. At a minimum, data to be
included in this documentation shall be:
a. Date of flow measurement device calibration
b. Name of person performing calibration
c. Percent from true flow
2. Any monitoring (including groundwater, surface water, soil or plant tissue analyses)
deemed necessary by the Division of Water Quality to insure surface and
groundwater protection will be established and an acceptable sampling reporting
schedule shall be followed.
3. Adequate records shall be maintained by the Permittee tracking the amount of
wastewater disposed. These records shall include, but are not necessarily limited to,
the following information:
a. date of irrigation,
b. volume of wastewater irrigated,
c. field irrigated,
d. length of time field is irrigated,

e. continuous weekly, monthly, and year-to-date hydraulic (inches/acre) loadings
for each field,
ff, weather conditions, and
g. maintenarice of cover crops.
4. The effluent from the subject facilities shall be monitored by the Permittee at the
point prior to irrigation every March, July, and November for the following
parameters:

Chloride
Sodium
Magnesium
Calcium
Sodium Adsorption Ratio by Calculation
5. Three copies of all operation and disposal records (as specified in condition 1112) on
Fonn NDAR-1 shall be submitted on or before the last day of the following month.
Three copies of all effluent monitoring data (as specified in condition III 3) on Form
NDMR-1 shall be submitted on or before the last day of the following month. All
information shall be submitted to the following address:

6. A representative annual soils analysis (Standard Soil Fertility Analysis) shall be
conducted on each spray field and the results maintained on file by the Permittee for
a minimum of five years. The Standard Soil Fertility Analysis shall include, but is
not necessarily limited to, the following parameters:
Acidity
Calcium
Copper
Magnesium
Cation Exchange Capacity
Manganese
Percent Humic Matter
Potassium
Sodium
pH Zinc
Exchangeable Sodium Percentage Phosphorus
Base Saturation (by calculation)

7. A record shall be maintained of all residuals removed from this facility. The record
shall include the name of the hauler, the permit authorizing the disposal, a letter
from a municipality agreeing to accept the residuals, the date the residuals were
hauled, and the volume of residuals removed.
S. A maintenance log shall be maintained at this facility including, but not limited to,
the following items:
a. Everyday that wastewater is land -applied via spray irrigation, the following
results should be recorded:
PH,
Residual chlorine,
Settleable matter,
Volume of wastewater irrigation per field
as recorded by the effluent flowmeter;
b. Visual observations of the plant and plant site;
c. Record of preventative maintenance; and
d. Date of calibration of flow measuring device.
9. NONCOMPLIANCE NOTIFICATION:
The Permittee shall report by telephone to the Raleigh Regional Office, telephone
number (919) 571-4700, as soon as possible, but in no case more than 24 hours or
on the next working day following the occurrence or first knowledge of the
occurrence of any of the following:
a. Any occurrence at the wastewater treatment facility which results in the
treatment of significant amounts of wastes which are abnormal in quantity or
characteristic, such as the dumping of the contents of a sludge digester, the
known passage of a slug of hazardous substance through the facility, or any
other unusual circumstances;
b. Any process unit failure, due to known or unknown reasons, that renders the
facility incapable of adequate wastewater treatment, such as mechanical or
electrical failures of pumps, aerators, compressors, etc.;
c. Any failure of a pumping station, sewer line, or treatment facility resulting in a
by-pass directly to receiving waters without treatment of all or any portion of the
influent to such station or facility; or

d. Any time that self-monitoring information indicates that the facility has gone
out of compliance with its permit limitations.
Persons reporting such occurrences by telephone shall also file a written report in
letter form within five (S) days following first knowledge of the occurrence. This
report must outline the actions taken or proposed to be taken to ensure that the
problem does not recur.
IV. GROUNDWATER REQUIREMENTS
Sampling Criteria:
Existing monitor well(s) MW -1, MW -2, MW -3, and MW -4 as shown on the plans
shall be sampled initially after construction and prior to waste disposal operations,
and, thereafter, every March, July, and November for the parameters listed below:
Water Level* pH
Chloride Fecal Coliform
Total Ammonia (as N) Nitrate (NO3-N)
Total Dissolved Solids (TDS) Total Organic Carbon
Volatile Organic Carbon (VOC) analysis - Only in November
*The measurement of water levels must be made prior to sampling for the
remaining parameters. The depth to water in each well shall be measured from the

ENGINEER'S CERTIFICATION
I, , as a duly registered Professional Engineer in
the State of North Carolina, having been authorized to observe (periodically/weekly/full tune) the
construction of the modifications to the town of Stovall's existing wastewater treatment and
disposal facility serving the town of Stovall, located in Granville County, hereby state that to the
best of my abilities, due care and diligence was used in the observation of the following
construction:
The addition of a new 3.96 acre irrigation field (Zone no. 8); to serve the town of Stovall with no
discharge of wastes to surface waters, pursuant to the application received September 21, 2001
and in conformity with the project plan, specifications, and other supporting data subsequently
filed and approved by the Department of Environment and Natural Resources.
The addition of the spray fields increases the existing capacity from 62,000 GPD to a new
capacity of 69,000 GPD on an average annual basis.
I certify that the construction of the above -referenced project was observed to be built within
substantial compliance and intent of the approved plans and specifications.
