Dear Mr. Moore:
As per an email request from Ms. Jamie Juliano of Interface Fabrics Group South, Inc., we are
forwarding herewith a corrected permit and Attachment A of Permit No. WQ0013982 dated August 26,
2005, to Interface Fabrics Group South, Inc. for the subject residuals land application grogram.
The following correction has been made to this Permit:
1. Mr. Alex Moore has since replaced Mr. James N. Caldwell as the Director of Operations, The
permit cover letter is corrected to reflect this change.
2. The Cover Letter and Attachment A of this permit have The Interface Fabrics Group South,
Inc.'s Process Water Treatment Facility identified incorrectly as Town of Elkin WTP, and
have been modified to reflect the correct identification of this water treatment facility.
3- The mineralization rate shown on Attachment A has been corrected to 0.30 to match with the
aerobic digestion employed at the Interface Fabrics Group South, lnc.'s wastewater treatment
facility.
4. The pathogen reduction requirements listed in the previous issuance of this permit were
inadvertently removed from this permit and have been re-inserted in this permit (see
Conditions 1,10 and 111.4).
Please replace the Cover Letter, Pages 1 through 12, and Attachment A with the corrected.
Cover Letter, Pages 1 through 12, and Attachment A into Permit No. WQ0013982, issued on
August 26, 2005.
