Dear Mr. Blanton:
It has come to our attention that the field designations that are being used on FORM ND.A R,-1 each month
to report the amount of wastewater applied to each of the approved application fields at your permitted
wastewater surface irrigation facility are different from those specified in Condition 11.7. in the current
permit (issued on June 2.2, 2006). The permit only allows application onto the approved fields specified
iii Condition II.7.
It is necessary that the field designations on the FORM agree with those specified in Condition H.7. of the
permit. Failing to report wastewater applications in compliance with the conditions of the permit will
result in future compliance problems.
