In accordance with your application for permit modification request received August 29, 2005, and
subsequent additional information received October 7, 2005, we are forwarding herewith Permit Number.
WQ0013348, dated June 16, 2006, to the Bay River Metropolitan Sewerage District for the construction
only of modifications to the Oriental Wastewater Treatment Facility site and the River Dunes reclaimed
water conjunctive reuse/disposal system site, construction and operation of a reclaimed water utilization
system extension, and the continued operation of the existing facilities.
This permit shall be effective from the date of issuance until May 31, 2011, shall void Permit No.
WQ0013348 issued June 16, 2006, and shall be subject to the conditions and limitations as specified
therein. Please pay particular attention to the monitoring requirements in this permit. Failure to establish
an adequate system for collecting and maintaining the required operational information will result in
future compliance problems.
If any parts, requirements, or limitations contained in this permit are unacceptable, you have the
right to request an adjudicatory hearing upon written request within thirty (30) days following receipt of
this permit. This request must be in the form of a written petition, conforming to Chapter I50B of the
North Carolina General Statutes, and filed with the Office of Administrative Hearings, 6714 Mail Service
Center, Raleigh, NC 27699-6714. Unless such demands are made this permit shall be final and binding.

construction one of a 500,000 gallon per day (GPD) reclaimed water treatment and reuse disposal system
with new infrastructure at two different sites consisting of the following components:
At the Oriental Wastewater Treatment Facility site:
A new 0.5 MGD Sequential Batch Reactor wastewater treatment process consisting of a 5.1 MG five-day
upset/off-line flow equalization basin (existing facultative lagoon cell ##1 is to be taken off-line, dewatered
and retrofitted for this purpose); a mechanically cleaned bar screen; a manually cleaned bypass bar
screen; an influent flow meter; two (2) 416,219 gallon (to max water level) SBR basins each with two (2)-
150 GPM WAS pumps, two (2)-30 hp aerators, one (1)-20 hp floating mixer and one (1) floating decanter
with each SBR conveying flow to the 132,189 gallon (to max water level) post-SBR equalization basin
which has three (3)- 550 GPM submersible transfer pumps and two (2)-3 hp floating mixers; two (2)-
195,015 gallon (to max water level) aerobic digesters each with two (2)-180 GPM submersible pumps,
one 7.5 hp aerator, one 7.5 hp floating mixer and one floating manual decant weir; two (2) cloth disk
tertiary filter trains each containing two - 53.8 square foot filter disks (total filter area of 215.2 ft2); a
21,667 gallon (to max water level) reclaimed water pump station with serpentine baffles (to act as a
chlorine contact basin) with three (3) 500 GPM vertical turbine reclaimed water pumps to convey flow to
the River Dunes groundwater infiltration disposal/conjunctive reclaimed water irrigation utilization
system; a 10' ID process wastewater pump station with two (2) 600 GPM submersible pumps; one duplex
hypochlorite feed system for disinfection (with injection location at the inlet to the post SBR flow
equalization basin), one duplex hypochlorite feed system for chlorine residual maintenance in the finished
reclaimed water (injected into the reclaimed waterline just prior to exiting the control building); two (2)-
400 gallon sodium hypochlorite storage tanks; one duplex sodium hydroxide feed system for enhanced
biological nitrification (injected into the plant influent line); two (2)-400 gallon sodium hydroxide
storage tanks; a reclaimed water effluent flow meter, a reclaimed water on-line total chlorine residual
monitor and an on-line turbidity monitor with automated diversion valves to automatically divert non-
complying reclaimed water into the five-day upset basin; two (2) 195,015 gallon aerobic sludge digesters
each with two (2) 180 GPM sludge transfer pumps, one (1) 7.5 hp floating aerator, one (1) 7.5 hp floating
mixer, one floating decanter, and truck unloading connection; an automatic standby generator, a new
Control Building with analytical laboratory; a digital SCADA control system with radio telemetry links to
the River Dunes site; electrical, mechanical, process, and associated appurtenances.

At the River Dunes site:
A new reclaimed water conjunctive reuse / disposal system consisting of three (3) reclaimed water
infiltration basins with total disposal capacity of 320,044 GPD (Basin 1 has a surface area of 4.03 acres
and an effective infiltrative area of 3.82 acres with infiltrative capacity of 63,885 GPD, Basin 2 has a
surface area of 2.83 acres and an effective infiltrative area of 2.71 acres with infiltrative capacity of
44,851 GPD, and Basin 3 has a surface area of 14.97 acres and an effective infiltrative area of 12.64 acres
with infiltrative capacity of 211,308 GPD) and wet weather storage capacity in Basin #3 of 4.8 million
gallons; a reclaimed water/irrigation system pumping station at infiltration basin #3 with two (2) 1,000
GPM submersible pumps (for transferring water to infiltration basins #1 and #2 and for site irrigation), a
concrete wetwell, and a screened intake structure (in Infiltration Basin #3); each infiltration basin is
equipped with a motorized fill valve (that is remotely controlled via the main programmable logic
controller at the irrigation pump station site) and automatic water level monitoring equipment to allow
remote operation and filling of the individual basins, a conjunctive reclaimed water irrigation system
consisting of up to five (5) drip irrigation zones totaling 58.3 acres with 101,582 emitters rated at 0.62
gallons per hour (GPH) each, approximately 3,000 linear feet of 10-inch reclaimed water line,
approximately 28,400 linear feet of 8-inch reclaimed water line, approximately 8,500 linear feet of 6-inch
reclaimed water line and approximately 4,000 linear feet of 4-inch reclaimed water line.

The continued operation of the Bayboro Wastewater Treatment Facility with average day capacity of
300,000 GPD, which consists of a three (3) stage facultative lagoon with a total surface area of 9.4 acres,
two (2) 150,000 gallon clarifiers with flocculent addition (alum), two (2) 15,000 gallon aerobic digesters,
an effluent pump station, chlorine disinfection facilities, a 22.5 million gallon lined storage lagoon, and an
effluent pumping station with two (2) 1,500 gallon per minute (GPM) pump.
The continued operation of the Arapahoe Spray irrigation Facility with average day capacity of 500,000
GPD, which consists of a 100,000 gallon covered concrete flow equalization tank with an odor control
system, an irrigation pump station with three (3) vertical turbine pumps, flow metering equipment and
standby generator, approximately 23,500 linear feet of 8-inch and 12-inch force mains for effluent
distribution to ten (10) irrigation zones with a total wetted area of 137 acres, approximately 143,700 linear
feet of 3-inch and 6-inch diameter aluminum irrigation laterals to 3,260 sprinkler heads on 3-foot risers, a
telemetric alarm system for pump monitoring,
construction and operation of a reclaimed water utilization system extension consisting of approximately
an additional 2,200 linear feet of 4-inch reclaimed water main and all associated piping, valves and
appurtenances to eventually serve the River Dunes Site with no discharge of wastes to the surface waters
has been permitted pursuant to the application received August 29, 2005 and subsequent additional
information received October 7, 2005 by the Division
As well as all associated appurtenances to serve the Pamlico Regional Wastewater Facilities with no
discharge of wastes to the surface waters, pursuant to the application received November 28, 2005 and
subsequent additional information received by the Division, and in conformity with the project plan,
specifications, and other supporting data subsequently filed and approved by the Department of
Environment and Natural Resources and considered a part of this permit.
This permit shall be effective from the date of issuance until May 31, 2011, shall void Permit No.
WQ0013348 issued June 16, 2006, and shall be subject to the following specified conditions and
limitations:
Upon completion of construction and prior to operation of this permitted facility, a certification
must be received from a professional engineer certifying that the permitted facility has been
installed in accordance with this permit, the approved plans and specifications, and other
supporting materials. If this project is to be completed in phases and partially certified, you shall
retain the responsibility to track further construction approved -under the same permit, and shall
provide a final certificate of -completion once the entire project has been completed. Mail the
Certification to the Land Application Unit, 1636 Mail Service Center, Raleigh, NC 27699-1636.
2. The Washington Regional Office, telephone number (252) 946-6481, shall be notified at least
forty-eight (48) hours in advance of operation of the installed facilities so that an in -place
inspection can be made. Such notification to the regional supervisor shall be made during the
normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State
Holidays.
The surface irrigation reclaimed water utilization facilities shall be effectively maintained and
operated at all times so that there is no discharge to the surface waters, nor any contamination of
ground waters, which will render them unsatisfactory for normal use. In the event that the
facilities fail to perform satisfactorily, including the creation of nuisance conditions or failure of
the irrigation area to adequately assimilate the wastewater, the Permittee shall take immediate
corrective actions including those actions that may be required by the Division of Water Quality
(Division), such as the construction of additional or replacement wastewater treatment and
disposal facilities.
4. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to
surface water or ground water resulting from the operation of this facility.
The residuals generated from these treatment facilities must be disposed in accordance with
General Statute 143-215.1 and in a manner approved by the Division.
6. 'Diversion or bypassing of the untreated wastewater from the treatment facilities is prohibited.
7. The following buffers shall be maintained:
a. Arapahoe Spray Irrigation Facility
i. 400 feet between wetted area and any reside -nee or place of public assembly under
separate ownership,
ii. 150 feet between wetted area and property lines,
iii. 100 feet between wetted area and wells,
iv. 100 feet between wetted area and drainage ways or surface water bodies,
v. 50 feet between wetted area and public right of ways,
vi. 100 feet between treatment/storage units and any wells, and
vii. 50 feet between treatment units and property lines.
b. River Dunes Drip Irrigation Facility
i. 100 feet between wetted areas and water supply wells,
ii. 100 feet between wetted areas and waters classified as SA,
iii. 100 feet between wastewater treatment units and wells,
iv. 50 feet between reclaimed water storage/irrigation ponds and property Iines, and
V. 50 feet between wastewater treatment units and property lines.
c. River Dunes Low Rate Infiltration Facilities

i. 10 feet between low rate infiltration unit and any on -property residential units that are
to be sold (e.g., condominiums, subdivisions),
ii. 500 feet between low rate infiltration unit and any impounded surface water supply or
public shallow (less than 50 feet deep) ground water supply,
iii. 100 feet between low rate infiltration unit and any private/public water supply source,
iv. 100 feet between low rate infiltration unit and any streams classified as WS or B,
V. 100 feet from mean high water between low rate infiltration unit and any "SA" or "SB"
classified surface waters,
vi. 100 feet from normal high water from low rate infiltration unit and any other stream,
canal, marsh, or coastal waters,
vii. 100 feet from normal high water from low rate infiltration unit and any Class I or Class
II impounded reservoir used as a source of drinking water,
viii. 100 feet between low rate infiltration unit and any other lake or impoundment,
ix. 15 feet between low rate infiltration unit and any building foundation/basement,
X. 15 feet between low rate infiltration unit and any top of slope of embankments or cuts
of two feet or more in vertical height,
xi. 10 feet between Iow rate infiltration unit and any water line,
xii. 100 feet between low rate infiltration unit and any swimming pool,
xiii. 20 feet between low rate infiltration unit and any other nitrification field (except repair
area),
xiv. 100 feet between low rate infiltration unit and any well with the exception of an
approved groundwater monitoring well,
xv. 25 feet between low rate infiltration unit and any drainage systems (ditches, drains,
surface water diversions, etc.) and from any groundwater lowering and surface drainage
ditches,
xvi. 50 feet between treatment units and property lines, and
xvii. 50 feet between low rate infiltration unit and public right of ways.
8. Public access to the irrigation sites shall be controlled during active site use. Such controls may
include the posting of signs showing the activities being conducted at each site. A sign shall be
posted in plain sight in the clubhouse showing these activities.
9. The disposal system shall be connected to a rain or moisture sensor that shall indicate when
wastewater application is not appropriate in accordance with Condition II(4) of this permit.
10. The following shall be requirements for the reclaimed water distribution, storage, and utilization
facilities:

the public or employees that the water is not intended for drinking. Where appropriate, such
warning shall inform the public or employees to avoid contact with the water.
b. All reclaimed water piping, valves, outlets, and other appurtenances shall be color -coded,
taped, or otherwise marked to identify the source of the water as being reclaimed water.
i. All reclaimed water piping and appurtenances shall be either colored purple (i.e., Pantone
522) and embossed or integrally stamped or marked "CAUTION: RECLAIMED
WATER — DO NOT DRINK" or be installed with a purple (i.e., Pantone 522)
identification tape or polyethylene vinyl wrap. The warning shall be stamped on opposite
sides of the pipe and repeated every three feet or less.
ii. Identification tape shall be at least three inches wide and have white or black lettering on
purple (i.e., Pantone 522) field stating "CAUTION: RECLAIMED WATER — DO NOT
DRINK." Identification tape shall be installed on top of reclaimed water pipelines,
fastened at least every 10 feet to each pipe length and run continuously the entire length
of the pipe.
c. All reclaimed water valves and outlets shall be of a type, or secured in a manner, that permits
operation by authorized personnel only.
d. Above -ground hose bibs (i.e., spigots or other hand -operated connections) shall not be
present. Hose bibs shall be located in locked below -grade vaults that shall be clearly labeled
as being of non. -potable quality. As an alternative to the use of locked below -grade vaults
with standard hose bibs services, hose bibs, which can only be operated by a special tool or
connected to a special hose connection, may be placed in non -lockable underground services
boxes clearly labeled as non -potable water.
11. Prior to operation of the modified wastewater treatment systems, the Permittee shall submit a written
contract between the Permittee and a Division -approved residuals handler. The Permittee shall also
submit documentation demonstrating that the contractor has modified their Water Quality Permit.
12. Upon completion of site grading and shaping and prior to the application of treated wastewater in
those spray areas that are being modified, a licensed soil scientist or licensed geologist shall conduct
an amended site evaluation, and an amended site evaluation report shall be submitted to the Division
of Water Quality. This report shall specifically address, but not be limited to, the effects of
grading/construction upon the original site evaluation. The evaluation should include an investigation
of key areas of grading, including soil features such as depth to seasonal high water table, hydraulic
conductivity, soil series and the appropriateness of previous irrigation design loadings for the newly
constructed spray fields.
11. OPERATION AND MAINTENANCE RE IIIREMENTS
1. The facilities shall be properly maintained and operated at all times.
2. Upon classification of the wastewater treatment and irrigation facilities by the Water Pollution
Control System Operators Certification Commission (WPCSOCC), the Permittee shall designate
and employ a certified operator to be in responsible charge (ORC) and one or more certified
operator(s) to be back-up ORC(s) of the facilities in accordance with 15A NCAC 8G .0201. The
ORC shall visit the facilities in accordance with 15A NCAC 8G .0204 or as specified in this
permit and shall comply with all other conditions specified in these rules.
3. A suitable, year-round vegetative cover shall be maintained on the irrigation areas.
4. Irrigation. (spray -and/or drip) shall -not- -be performed during inclement weather or when the
ground is in a condition that will cause runoff.
5. Adequate measures shall be taken to prevent wastewater runoff from the irrigation field.
6. The facilities shall be effectively maintained and operated as a non -discharge system to prevent
the discharge of any wastewater resulting from the operation of this facility.
7. The application rates for:
a. Arapahoe Spray Irrigation Facility — Fields 1 through 7 shall not exceed a cumulative loading
of 68.3 inches over any twelve (12) month period at an instantaneous application rate not to
exceed 0.5 inches per hour.
b. Arapahoe Spray Irrigation Facility — Fields 8 through 10 shall not exceed a cumulative
loading of 16.8 inches over any twelve (12) month period at an instantaneous application rate
not to exceed 0.2 inches per hour.
c. River Dunes Reclaimed Drip Irrigation Facility — Fields 1 through 5 shall not exceed a
cumulative loading of 10.9 inches over any twelve (12) month period at an instantaneous
application rate not to exceed 0.05 inches per hour.
d. River Dunes Reclaimed Infiltration Facility — Infiltration Basin 1 shall not exceed a loading
rate of 0.364 gallons per day per square foot.
e. River Dunes Reclaimed Infiltration Facility — Infiltration Basin 2 shall not exceed a loading
rate of 0.364 gallons per day per square foot.
f. River Dunes Reclaimed Infiltration Facility — Infiltration Basin 3 shall not exceed a loading
rate of 0.338 gallons per day per square foot.
8. Automatically activated standby power sources shall be maintained at the Oriental WWTF,
Bayboro WWTF and Arapaho Spray Site and shall be operational at all times. If a generator is
employed as an alternate power supply, it shall be tested weekly by interrupting the primary
power source.
9. No type of wastewater and/or reclaimed water other than that from the Pamlico Regional
Wastewater Facilities shall be irrigated and/or infiltrated onto the disposal area.
10. Freeboard in the Bayboro Storage Lagoon, the Oriental Lagoon, and the River Dunes Storage
Pond shall not be less than two (2) feet at any time.
11. No traffic or equipment shall be allowed on the disposal areas except while installation occurs or
while normal maintenance is performed.
12. If not already installed, waste -level gauges, to monitor waste levels in the storage ponds, shall be
installed within 60 days of issuance of this permit for existing ponds and prior to construction for
proposed ponds. The gauges shall have readily visible permanent markings indicating the
maximum liquid level at the top of the temporary liquid storage volume, minimum liquid level at
the bottom of the temporary liquid storage volume, and top of the dam elevations. Caution must
be taken not to damage the integrity of the liner when installing the gauges.
13. A protective vegetative cover shall be established and maintained on all earthen basin
embankments (outside toe of embankment to maximum pumping elevation), berms, pipe runs,
erosion control areas, and surface water diversions. Trees, shrubs, and other woody vegetation
shall not be allowed to grow on the earthen basin dikes or embankments. Earthen basin
embankment areas shall be kept mowed or otherwise controlled and accessible.

routed to the Oriental three (3) stage facultative lagoon, should the limit for fecal coliform (daily
maximum concentration of 25 per 100 ml) or turbidity (instantaneous maximum of 10 NTU) be
exceeded, until such time that the problems associated with the treatment capability of the
reclaimed water treatment plant have been corrected. The reclaimed water sent to the Oriental
facultative lagoon shall be irrigated at the Arapahoe Spray Irrigation Facility.
III. MONITORING AND REPORTING REQUIREMENTS
1. Any monitoring (including groundwater, surface water, soil or plant tissue analyses) deemed
necessary by the Division to insure surface and ground water protection will be established and
an acceptable sampling reporting schedule shall be followed.
2. Influent flow shall be continuously monitored and daily flow values shall be reported on Form
NDMR.
The Permittee shall install an appropriate flow measurement device consistent with approved
engineering and scientific practices to ensure the accuracy and reliability of influent flow
measurement. Flow measurement devices selected shall be capable of measuring flows with a
maximum deviation of less than 10 percent from true flow, accurately calibrated at a minimum of
once per year, and maintained to ensure that the accuracy of the measurements is consistent with
the accepted capability of that type of device. The Permittee shall keep records of flow
measurement device calibration on file for a period of at least three years. At a minimum, data to
be included in this documentation shall be:
a. Date of flow measurement device calibration
b, Name of person performing calibration
c. Percent from true flow
Plans and specifications for the flow measuring device shall be prepared by a licensed
Professional Engineer and submitted to the appropriate regional office 30 days prior to
installation. The flow -measuring device shall be installed within 1 year of the date of this permit.
The effluent from the Oriental Lagoon and Bayboro Wastewater Treatment Facilities shall be
monitored by the Permittee at a point prior to irrigation every month for the following
parameters:

Standards, this permit will be modified to include additional and/or more restrictive limitations.
5. The Permittee shall maintain. adequate records tracking the amount of wastewater disposed.
These records shall include, but are not necessarily limited to the following information:
a. Date and time of irrigation,
b. Volume of wastewater irrigated,
c. Zone irrigated,
d. Length of time zone is irrigated,
e. Continuous weekly, monthly, and year-to-date hydraulic (inches/acre) loadings for each zone,
f. Weather conditions, and
g. Maintenance of cover crops.
6. Freeboard in the Bayboro Storage Lagoon and River Dunes Storage Pond shall be recorded
weekly.
7. Three (3) copies of all monitoring data [as specified in Conditions M(2), M(3) and III(4)] on
Form NDMR-1 and three (3) copies of all operation and disposal records [as specified in
Conditions M(5) and III(6)] on Form NDAR-1 shall be submitted monthly on or before the last
day of the following month. All information shall be submitted to the following address:

8. A record shall be maintained of all residuals removed from this facility. This record shall include
the name of the hauler, permit authorizing the disposal or a letter from a municipality agreeing to
accept the residuals, date the residuals were hauled, and volume of residuals removed.
9. An annual representative soils analysis (Standard Soil Fertility Analysis) shall be conducted on
each irrigation field at the Arapahoe Spray Irrigation Facility and the results maintained on file by
the Permittee for a minimum of five years. The Standard Soil Fertility Analysis shall include, but
is not necessarily limited to, the following parameters:
Acidity
Magnesium
Zinc
Percent Humic Matter
Calcium
Potassium
Copper
Cation Exchange Capacity
Sodium
Phosphorus
pH
Manganese
Exchangeable Sodium
Percentage
Base Saturation (by calculation)
10. Maintenance logs shall be maintained at the Oriental Reclaimed Water Scalping Plant including
but not Iimited to the following items:
a. Daily sampling results of dissolved oxygen in the aeration basin and at the clarifier weir.
b. Visual observations of the plant and plant site.
c. Record of preventative maintenance (changing of filters, adjusting belt tensions, alarm
testing, diffuser inspections and cleanings, etc.).
d. Date of calibration of flow measurement device.
e. Date and results of power interruption testing on alternate power supply.

- - 11. Noncompliance-Notiflfatiow.
The Permittee shall report by telephone to the Washington Regional Office, telephone number
(252) 946-6481, as soon as possible, but in no case more than 24 hours or on the next working
day following the occurrence or first knowledge of the occurrence of any of the following -
Any occurrence at the wastewater treatment facility which results in the treatment of
significant amounts of wastes which are abnormal in quantity or characteristic, such as the
dumping of the contents of a sludge digester; the known passage of a slug of hazardous
substance through the facility; or any other unusual circumstances.
b. Any process unit failure, due to known or unknown reasons, that render the facility incapable
of adequate wastewater treatment such as mechanical or electrical failures of pumps, aerators,
compressors, etc.
c. Any failure of a pumping station, sewer line, or treatment facility resulting in a by-pass
directly to receiving waters without treatment of all or any portion of the influent to such
station or facility.
d. Any time that self -monitoring information indicates that the facility has gone out of
compliance with its permit limitations.
Occurrences outside normal business hours may also be reported to the Division's
Emergency Response personnel at telephone number (800) 662-7956, (800) 858-0368, or
(919) 733-3300. Persons reporting such occurrences by telephone shall also file a written
report in letter form within five (5) days following first knowledge of the occurrence. This
report must outline the actions taken or proposed to be taken to ensure that the problem does
not recur.
IV. GROUNDWATER REQUIREMENTS
1. Sampling Criteria:
a. Existing monitor wells MW-1, MW-2, MW-3, MW4, MW-5, and MW-6 at the Arapahoe
spray irrigation sites; monitor wells MW-15, MW-17, MW-18, MW-19, MW-20, MW-21,
MW-22, and MW-23 located at the Oriental lagoon site; and monitor wells MW-7, MW-8,
MW-9, MW-10, MW-11, MW-12, MW-13, and MW-14 located at the Bayboro lagoon site
shall be sampled every March, July, and November for the parameters listed below. Prior to
sampling the parameters, the measurement of water levels must be taken. The depth to water
in each well shall be measured from the surveyed point on the top of the casing. if not already
performed, the measuring point (top of well casing) of all monitor wells shall be surveyed
relative to a common datum.

down -gradient monitoring well, additional sampling and analysis must be conducted to
identify the individual constituents comprising this TOC concentration. If the TOC
concentration as measured in the background monitor well exceeds 10 mgll, this
concentration will be taken to represent the naturally occurring TOC concentration. Any
exceedanees of this naturally occurring TOC concentration in the down -gradient wells shall
be subject to the additional sampling and analysis as described above.
c. For Volatile Organic Compounds (VOC's) sampled in November, use only one of the
following methods:
i. Standard Method 6230D (PQL at 0.5 ug(L or less),
ii. Standard Method 6210 (PQL at 0.5 ug/L or less),
iii. EPA Method 8021 (Low Concentration, PQL at 0.5 ug/L or less),
iv. EPA Method 9260 (Low Concentration, PQL at 0.5 ug(L or less), or
v. Another method with prior approval by the Division.
Any of the referenced methods used for VOCs shall, at a minimum, include all of the
constituents listed in Table VIII of Standard Methods 6230D. The method used shall provide
a PQL at 0.5 ug/L or less, which shall be supported by laboratory proficiency studies as
required by the Division's Laboratory Certification Unit. Any constituents detected above the
MDL, but below the PQL at 0.5 ug/L, shall be qualified (i.e., estimated) and reported.
2. Reporting / Documentation:
a. The results of the sampling and analysis must be received on the most -recent version of
"Groundwater Quality Monitoring: Compliance Report Form" (i.e., GW-59 Form), with
copies of the laboratory analyses attached, by the Division's Information Processing Unit on
or before the last working day of the month following the sampling month.
b. All reports, maps, and other documents required in the "Groundwater Requirements" section
of this permit shall be mailed to the following address:

3. Vertical Separation Requirements:
a. Reclaimed water irrigation activities taking place at the Arapahoe Spray Irrigation Facility
and the River Dunes Drip Irrigation Facility should not occur when the vertical separation
between the depth of irrigation and the seasonal high water table is less than one foot.
4. Applicable Boundaries:
a. Arapahoe Spray Irrigation Site:
i. The COMPLIANCE BOUNDARY for the disposal system is specified by
regulations in 15A NCAC 2L, Groundwater Classifications and Standards. The
Compliance Boundary for the disposal system individually permitted after
December 31, 1983 is established at either 250 feet from the waste disposal area,
or 50 feet within the property boundary, whichever is closer to the waste disposal
area. An exceedance of Groundwater Quality Standards at or beyond the
Compliance Boundary is subject to immediate remediation action according to
15A NCAC 2L .0106(d)(2).

ii. - The REVIEW BOUNDARY -is established around the disposal systems midway
between the Compliance Boundary and the perimeter of the waste disposal area.
Any exceedance of standards at the Review Boundary shall require action in
accordance with 15A NCAC 2L .0106(d)(1).
b. River Dunes Disposal Sites:
i. The COMPLIANCE BOUNDARY and REVIEW BOUNDARY for the
wastewater treatment and reclaimed water facilities shall be as specified by
regulations in 15A NCAC 2H .0219 (k)(1)(C)(i)(M)(Le., "Waste Not Discharged
to Surface Waters"). The Compliance Boundary and Review Boundary for
groundwater shall be established at the property boundary_ An exceedance of
Groundwater Quality Standards at or beyond the Compliance Boundary is subject
to immediate remediation action according to 15A NCAC 2L .0106 (d)(2).
S. Other Requirements:
a. Any additional groundwater quality monitoring, as deemed necessary by the Division, shall
be provided.
V. INSPECTIONS
Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to insure
proper operation of the subject facilities.
2. The Permittee or his designee shall inspect the wastewater treatment and disposal facilities to
prevent malfunctions and deterioration, operator errors and discharges which may cause or lead to
the release of wastes to the environment, a threat to human health, or a nuisance. The Permittee
shall keep an inspection log or summary including at least the date and time of inspection,
observations made, and any maintenance, repairs, or corrective actions taken by the Permittee.
This log of inspections shall be maintained by the Permittee for a period of three years from the
date of the inspection and shall be made available upon request to the Division or other permitting
authority.
3. Any duly authorized officer, employee, or representative of the Division may, upon presentation
of credentials, enter and inspect any property, premises or place on or related to the disposal site
or facility at any reasonable time for the purpose of determining compliance with this permit-,
may inspect or copy any records that must be maintained under the terms and conditions of this
permit, and may obtain samples of groundwater, surface water, or leachate.
VI. GENERAL CONDITIONS
1. This permit shall become voidable unless the facilities are constructed in accordance with the
conditions of this permit, the approved plans and specifications, and other supporting data.
2. This permit is effective only with respect to the nature and volume of wastes described in the
application and other supporting data.
3. This permit is not transferable. In the event there is a desire for the facilities to change
ownership, or there is a name change of the Permittee, a formal permit request must be submitted
to the Division accompanied by an application fee, documentation from the parties involved, and
other supporting materials as may be appropriate. The approval of this request will be considered
on its merits and may or may not be approved.
4. Failure to abide by the conditions and limitations contained in this permit may subject the
Permittee to an enforcement action by the Division in accordance with North Carolina General
Statute 143-215.6A to 143-215.6C.

statutes, rules, regulations, or ordinances which may be imposed by other government agencies
(local, state, and federal) which have jurisdiction, including but not limited to applicable river
buffer rules in 15A NCAC 2B.0200, erosion and sedimentation control requirements in 15A
NCAC Chapter 4 and under the Division's General Permit NCG010000, and any requirements
pertaining to wetlands under 15A NCAC 2B .0200 and 2H .0500.
6. The Permittee shall retain a set of approved plans and specifications for the subject facility for the
life of the project.
7. The Permittee shall pay the annual administering and compliance fee within thirty days of being
billed by the Division. Failure to pay the fee accordingly may cause the Division to initiate action
to revoke this permit as specified by 15 NCAC 2H .0205 (c)(4).
8. The Permittee, at least six (6) months prior to the expiration of this permit, shall request its
extension. Upon receipt of the request, the Commission will review the adequacy of the facilities
described therein, and if warranted, will extend the permit for such period of time and under such
conditions and limitations as it may deem appropriate.
