WQ0012748, dated October 31, 1996, to the Sea Trail Corporation for the construction and operation of the subject wastewater treatment and spray irrigation facilities. Please be advised, until a public utility has been formed, the Sea Trail Corporation can not assess operating and/or maintenance fees for the use of the wastewater systems. Within six (6) months of permit issuance, the land application of residuals permit issued to S&B Maintenance, Inc. shall be amended to include the residuals from the Sea Trail Corporation as a source. This permit shall be effective from the date of issuance until September 30, 2001, and shall be subject to the conditions and limitations as specified therein. Please pay particular attention to the monitoring requirements in this permit. Failure to establish an adequate system for collecting and maintaining the required operational information will result in future compliance problems. If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right to request an adjudicatory hearing upon written request within thirty (30) days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings, P.O. Box 29535, Raleigh, North Carolina 27626-0535 Telephone (919) 733-5083 FAX (919) 733-0719 An Equal Opportunity Affirmative Action Employer 50% recycled/ 10% post -consumer paper 1.7 NORTH CAROLINA ENVIRONMENTAL MANAGEMENT COMMISSION DEPARTMENT OF ENVIRONMENT, HEALTH AND NATURAL RESOURCES RALEIGH SPRAY IRRIGATION SYSTEM PERMIT In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and other applicable Laws, Rules, and Regulations PERMISSION IS HEREBY GRANTED TO THE Sea Trail Corporation Brunswick County construction and operation of a 500,000 GPD wastewater treatment and spray irrigation system to be built in phases consisting of. one (1) 200,000 GPD wastewater treatment facility consisting of two (2) 25,000 gallon flow equalization basins with dual 280 GPM pumps with high water alarms and one (1) 134 CFM blower, a manually cleaned bar screen, a flow splitter box, two (2) 125,010 gallon fine bubble aeration basins with three (3) 500 CFM blowers to serve the WWTP, two (2) 16,670 gallon clarifiers, two (2) 20,000 gallon aerated sludge holding tanks, two (2) 72 square foot dual bed teritary filters with one (1) 106 CFM blower, ultraviolet disinfection, a 3,675 gallon dosing tank with dual 600 GPM pumps and high water alarms, and three (3) 100,000 GPD wastewater treatment facility each consisting of one (1) 25,000 gallon flow equalization basins with dual 140 GPM pumps with high water alarms and one (1) 68 CFM blower, a manually cleaned bar screen, a flow splitter box, two (2) 62,675 gallon fine bubble aeration basins with three (2) 500 CFM blowers to serve the WWTP, two (2) 8,335 gallon clarifiers, two (2) 10,000 gallon aerated sludge holding tanks, two (2) 36 square foot dual bed teritary filters with one (1) 53 CFM blower, ultraviolet disinfection, a 2,064 gallon dosing tank with dual 120 GPM pumps and high water alarms, and a meter for the continuous on-line monitoring and recording for turbidity, a meter for the continuous on- line monitoring and recording for flow, approximately 5,450 linear feet of 8-inch force main to transport treated effluent from the WWTPs to the storage pond, a 2,500,000 gallon clay lined 5-day effluent pond, a 7,500,000 gallon storage pond, dual power feed, and approximately 767,000 square feet of irrigation area over three (3) different golf courses, to serve the Sea Trail Corporation with no discharge of wastes to the surface waters, pursuant to the application received August 2, 1996, and in conformity with the project plan, specifications, and other supporting data subsequently filed and approved by the Department of Environment, Health and Natural Resources and considered a part of this permit. This permit shall be effective from the date of issuance until September 30, 2001, and shall be subject to the following specified conditions and limitations: 1. PERFORMANCE STANDARDS 1. Upon completion of construction and prior to operation of this permitted facility, a certification must be received from a professional engineer certifying that the permitted facility has been installed in accordance with this permit, the approved plans and specifications, and other supporting materials. Mail the Certification to the Water Quality Permits and Engineering Unit, P.O. Box 29535, Raleigh, NC 27626-0535. 2. The Wilmington Regional Office, telephone number (910) 395-3900, shall be notified at least forty-eight (48) hours in advance of operation of the installed facilities so that an in - place inspection can be made. Such notification to the regional supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays. 3. The spray irrigation facilities shall be effectively maintained and operated at all times so that there is no discharge to the surface waters, nor any contamination of ground waters which will render them unsatisfactory for normal use. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions or failure of the irrigation area to adequately assimilate the wastewater, the Permittee shall take immediate corrective actions including those actions that may be required by the Division of Water Quality, such as the construction of additional or replacement wastewater treatment and disposal facilities. 4. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to surface or groundwaters resulting from the operation of this facility. 5. The residuals generated from these treatment facilities must be disposed in accordance with General Statute 143-215.1 and in a manner approved by the North Carolina Division of Water Quality. 6. Diversion or bypassing of the untreated wastewater from the treatment facilities is prohibited. 7. The following buffers shall be maintained: a) 100 feet between wetted area and water supply wells, b) 100 feet between wetted area and waters classified as SA, c) 100 feet between wastewater treatment units and wells, d) 50 feet between wastewater treatment units and property lines, and e) 25 feet between wetted area and surface waters not classified as SA. 8. Public access to the land application sites shall be controlled during active site use. Such controls may include the posting of signs showing the activities being conducted at each site. A sign shall be posted in plain sight in the club house showing these activities. 11. OPERATION AND MAINTENANCE REQUIREMENTS The facilities shall be properly maintained and operated at all times. 2. A suitable vegetative cover shall be maintained on the spray irrigation areas. 3. Irrigation shall not be performed during inclement weather or- when the ground is in a condition that will cause runoff. 4. Adequate measures shall be taken to prevent wastewater runoff from the spray field. 5. The facilities shall be effectively maintained and operated as a non -discharge system to prevent the discharge of any wastewater resulting from the operation of this facility. 6. Upon classification of the facility by the Certification Commission, the Permittee shall employ a certified wastewater treatment plant operator to be in responsible charge (ORC) of the wastewater treatment facilities. The operator must hold a certificate of the type and grade at least equivalent to or greater than the classification assigned to the wastewater treatment facilities by the Certification Commission. The Permittee must also employ a certified back-up operator of the appropriate type and grade to comply with the conditions of Title 15A, Chapter 8A, .0202. The ORC of the facility must visit each Class I facility at least weekly and each Class II, III, and IV facility at least daily, excluding weekends and holidays, and must properly manage and document daily operation and maintenance of the facility and must comply with all other conditions of Title 15A, Chapter 8A, .0202. Once the facility is classified, the Permittee must submit a Ietter to the Certification Commission which designates the operator in responsible charge within thirty days after the wastewater treatment facilities are 50% complete. 7. The application rate shall not exceed the following for the individual courses listed below: Jones Course: a cumulative loading of 44.2 inches over any twelve (12) month period at an instantaneous application rate not to exceed 0.15 inches per dose Byrd Course: a cumulative loading of 26.0 inches over any twelve (12) month period at an instantaneous application rate not to exceed 0.10 inches per dose Maples Course: a cumulative loading of 65.0 inches over any twelve (12) month period at an instantaneous application rate not to exceed 0.15 inches per dose 8. No type of wastewater other than that from the Sea Trail Corporation shall be sprayed onto the irrigation area. 9. All wastewater shall be routed to the five-day holding pond should the limit for fecal coliform (daily maximum concentration of 25 per 100 ml) or turbidity (instantaneous maximum of 10 NTU) be exceeded, until such time that the problems associated with the treatment capability of the wastewater treatment plant have been corrected. The wastewater in the five-day holding pond shall be pumped back to the treatment plant for retreatment or treated in the five-day pond prior to discharge to the storage pond. MONITORING AND REPORTING REQUIREMENTS 1. Any monitoring (including groundwater, surface water, soil or plant tissue analyses) deemed necessary by the Division of Water Quality to insure surface and ground water protection will be established and an acceptable sampling reporting schedule shall be followed. 2. As an indicator of proper operation and maintenance, the facility shall produce an effluent in compliance with the following limitations: Parameter Monthly Averaged Daily(Instantaneous) Maximum Flow BODS (5-day, 20°C) NH3 as N TSS Fecal Coliform Turbidity 10 mg/l 4 mg11 5 mg/1 14 per 100 mlb 500,000 GPD 15 mg/l 6 mg/l 10 mg/l 25 per 100 nil 10 NTU a Monthly averages for all but fecal coliform shall be the arithmetic mean of all samples collected during the reporting period. b Monthly average for fecal coliform shall be the geometric mean of all samples collected during the reporting period. c Daily maximum shall be the maximum value of all samples collected during the reporting period. During the remainder of the year, these parameters shall be monitored monthly. Triannually sampling shall be conducted during March, July, and November. If Groundwater sampling indicates or predicts problems with the compliance with Groundwater Standards, this permit will be modified to include additional and/or more restrictive limitations. 3. Adequate records shall be maintained by the Permittee tracking the amount of wastewater disposed. These records shall include, but are not necessarily limited to the following information: a. date and time of irrigation, b. volume of wastewater irrigated, c. zone irrigated, d . length of time zone is irrigated, e. continuous weekly, monthly, and year-to-date hydraulic (inches/acre) loadings for each zone, f. weather conditions, and g. maintenance of cover vegetation. N 4. Three (3) copies of all operation and disposal records (as specified in condition III 3) on Form. NDAR- l shall be submitted on or before the last day of the following month. Three (3) copies of all effluent monitoring data (as specified in condition III 2) on Form NDMR-1 shall be submitted on or before the last day of April, August, and December. A record shall be maintained of all residuals removed from this facility. This record shall include the name of the hauler, permit authorizing the disposal or a letter from a municipality agreeing to accept the residuals, date the residuals were hauled, and volume of residuals removed. 6. A maintenance log shall be maintained at this facility including but not limited to the following items: a. Daily sampling results including settleable matter and dissolved oxygen in the aeration basin and at the clarifier weir. b. Visual observations of the plant and plant site. c. Record of preventative maintenance (changing of filters, adjusting belt tensions, alarm testing, diffuser inspections and cleanings, etc.). d. Date of calibration of flow measurement device and turbidimeter. 7. Noncompliance Notification: The Permittee shall report by telephone to the Wilmington Regional Office, telephone number (910) 395-3900, as soon as possible, but in no case more than 24 hours or on the next working day following the occurrence or first knowledge of the occurrence of any of the following: a. Any occurrence at the wastewater treatment facility which results in'the treatment of significant amounts of wastes which are abnormal in quantity or characteristic, such as the dumping of the contents of a sludge digester; the known passage of a slug of hazardous substance through the facility; or any other unusual circumstances. b. Any process unit failure, due to known or unknown reasons, that render the facility incapable of adequate wastewater treatment such as mechanical or electrical failures of pumps, aerators, compressors, etc. c. Any failure of a pumping station, sewer line, or treatment facility resulting in a by-pass directly to receiving waters without treatment of all or any portion of the influent to such station or facility. d . Any time that self -monitoring information indicates that the facility has gone out of compliance with its permit limitations. Persons reporting such occurrences by telephone shall also file a written report in letter form within five (5) days following first knowledge of the occurrence. This report must outline the actions taken or proposed to be taken to ensure that the problem does not recur. IV. GROUNDWATER REQUIREMENTS l . Any additional groundwater quality monitoring, as deemed necessary by the Division, shall be provided. 2. The COMPLIANCE BOUNDARY for the disposal system is specified by regulations in 15A NCAC 2L, Groundwater Classifications and Standards. The Compliance Boundary is for the disposal system constructed after December 31, 1983 is established at either (1) 250 feet from the waste disposal area, or (2) 50 feet within the property boundary, whichever is closest to the waste disposal area. An exceedance of Groundwater Quality Standards at or beyond the Compliance Boundary is subject to immediate remediation action in addition to the penalty provisions applicable under General Statute 143-215.6A(a)(1). In accordance with 15A NCAC 2L, a REVIEW BOUNDARY is established around the disposal systems midway between the Compliance Boundary and the perimeter of the waste disposal area. Any exceedance of standards at the Review Boundary shall require remediation action on the part of the permittee. 3. In accordance with 15A NCAC 2L .0107 (d), any water supply wells existing within the Compliance Boundary shall be abandoned in accordance with the procedures outlined in 15A NCAC 2C .0113. 4. No land application of waste activities shall be undertaken when the seasonal high water table is less than three feet below land surface. 5. The interior surfaces of the 5-day holding lagoon shall be completely lined with a synthetic liner or a liner of natural material at least one foot in thickness, compacted to 95% proctor standard dry density and installed and compacted in layers no thicker than six inches, each with a hydraulic conductivity of no greater than 1 x 10-6 cm/sec. Following installation and inspection of the lagoon liner, and prior to waste disposal operations, verification of the liner's compliance with hydraulic conductivity and thickness specifications must be provided to the Division of Water Quality, Groundwater Section, by the project engineer. V . INSPECTIONS 1. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to insure proper operation of the subject facilities. 2. The Permittee or his designee shall inspect the wastewater treatment and disposal facilities to prevent malfunctions and deterioration, operator errors and discharges which may cause or lead to the release of wastes to the environment, a threat to human health, or a nuisance. The Permittee shall keep an inspection log or summary including at least the date and time of inspection, observations made, and any maintenance, repairs, or corrective actions taken by the Permittee. This log of inspections shall be maintained by the Permittee for a period of three years from the date of the inspection and shall be made available upon request to the Division of Water Quality or other permitting authority. 3. Any duly authorized officer, employee, or representative of the Division of Water Quality may, upon presentation of credentials, enter and inspect any property, premises or place on or related to the disposal site or facility at any reasonable time for the purpose of determining compliance with this permit; may inspect or copy any records that must be maintained under the terms and conditions of this permit, and may obtain samples of groundwater, surface water, or leachate. V1. GENERAL CONDITIONS 1. This permit shall become voidable unless the facilities are constructed in accordance with the conditions of this permit, the approved plans and specifications, and other supporting data. 2. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 3. This permit is not transferable. In the event there is a desire for the facilities to change ownership, or there is a name change of the Permittee, a formal permit request must be submitted to the Division of Water Quality accompanied by an application fee, documentation from the parties involved, and other supporting materials as may be appropriate. The approval of this request will be considered on its merits and may or may not be approved. 4. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to an enforcement action by the Division of Water Quality in accordance with North Carolina General Statute 143-215.6(a) to 143-215.6(c). 5_ The issuance of this permit does not preclude the Permittee from complying with any and all statutes, rules, regulations, or ordinances which may be imposed by other government agencies (local, state, and federal) which have jurisdiction. 6. A set of approved plans and specifications for the subject project must be retained by the Permittee for the life of the project. 7. The annual administering and compliance fee must be paid by the Permittee within thirty days after being billed by the Division. Failure to pay the fee accordingly may cause the Division to initiate action to revoke this permit as specified by 15 NCAC 211.0205 (c)(4). 8. The Permittee, at least six (6) months prior to the expiration of this permit, shall request its extension. Upon receipt of the request, the Commission will review the adequacy of the facilities described therein, and if warranted, will extend the permit for such period of time and under such conditions and limitations as it may deem appropriate. A basin in which all solids are maintained in suspension and by which biological oxidation or organic matter is reduced through artificially accelerated transfer of oxygen on a flow -through basis; (3) Aeratlon A process of bringing about Intimate contact between air or high purity oxygen In a liquid by spraying, aghation or drus1cn7(3a} Extended Aeration. An activated sludge process utilizing a minimum hydraulic detention time of 18 hours. (4) Agriculturally managed site. Such systems shall carry a maximum of two points regardless of pumping facifties or any other appurtenances; ' (12) Combined Removai of Carbonaceous BOD and NhroSenous Removal by Wrification- A single stage system required to achieve part-LI effluent f-Ernits on BCD and ammonia nitrogen within the same biological reactor, (13) Dechlonnatlon. A deep two story wastewater tank consisting of an upper sadlmernation chamber and a lower sludge digestion chamber. (20) Instrumented Flow Measurement. A device which indicates and records rate of flow; (21) Ion Exchange. A chemical process in which 'tans from two diflerort molecules are exchanged; (22) Land application: (a) Sludge Disposal. A final sludge disposal method by which wet sludge may be applied to land either by spraying an the surface or by subsurface injection (i.e, chisel plow); (not applicable tar types of sludge described In (11) of this Rule]; (b) Treated Effluent. The process of spraying treated wastawaler onto. a land area or other methods of application of wastewater orao a land area as a means of final disposal or treatment; (23) MIcroscreerL A low speed, continuously back -washed, rotating drum liher operating under gravity conditions as a polishing method for removing suspended solids from effluertt; (24) Ni"iration Process. The biochemical conversion of unoxidized nhrogan (ammonia and organic nitrogen) to oxidized nitrogen (usually nitrate); (25) Nitrogenous Stage. A separate stage of wastewater treatment designed for lire specific purpose of converting ammonia nitrogen to nitrate nitrogen; (26) Phosphate Removal, Biological. The removal of phosphorus from wastewater by an oxiclanoxic process designed to enhanca luxury uptake of phosphorus by the microorganisms; (27) Polishing Pond. A holding pond Wlawing secondary treatmert with sufficient detention lime to allow settling of finely suspended solids; (28) Post Aeratiam Aeration following conventional secondary troatment units to increase effluent D.O. or for any other purpose; (29) Post Aeration. (Cascade) A polishing method by which dssolved oxygen is added to the eflluerd by a nonmachanical, gravity means of flowing dawn a series ci steps or weirs; The flow occurring across the steps or weirs moves in a fairly thin layer and the operation of the cascade requires no operator adjustment; thus, zero points are assigned even though this is an essential step to mooting the limits of the discharge permit; (30) Powdered to Granular Activated Carbon Feed- A biophysical carbon process that utilizes biological activity and organic absorption by using powdered or granular attNzaed carbon; Virgin or regenerated carbon Is feed controlled into the system; (31) Preasr'alLon- A tank cons rooted to provide aeration prior to primary treatment; (32) Preliminary Units. Unit operations in the treatment process, such as screening and comminution, that prepare the liquor for subsequent major operations; (33) Industrial Pretreatment. (a) Pre-treatment Unit, Industrial. The conditioning of a waste at its scur::e before discharge, to remove or to neutralize substances Injurious to sewers and treallmant processes or to eHact a partial reduction in load on the treatment process which is operated by the sago governing body as the wastewatar treatment plant being rated; b) Pre-treatment Program, Industrial - must be a Slats or EPA required program to receive points on the rating sheet; (34) Primary Clarifiers. The first settling tanks through which wastewater is passed In a treatment works for the purpose of removing settleable and suspended solids and BOD which is associated with the solids; (35) Pumps. AI influent, effluent and in -plant pumps; (36) Radiation. Disinfection or sterilization process utilizing devices emitting ultraviolet or gamma rays; (37) Reverse Osmosis. A treatment process In which a heavy contaminated liquid Is pressurizod through a membrane forming nearly pure liquid free from suspended solids; (38) Rotating Biological Contractors. A fixed biological growth process in which wastewater flows through tanks In which a series of partially submerged ciraiar surfaces are rotated; (39) Sand Filters: (a) Intermittent Biological. Filtration of effluent follcwing septic tanks, lagoons, or some other treatment process In which further biodecornposhfon is expected to produce desired effluents; Hydraulic loading rates on these filters are computed In gpd/ac and have a resulting tow gprrJsf (less than one); b) Recirculating biological - the same type of sand filter as defined in Subparagraph (39) (a) o1 this Rule with the added capability to re ycia effluent back through the sand filter, (40) Sand or Mixed -Media Fliers. A polishing process by which effluent limits are achieved through a further reduclfon of suspended solids; (a) low rate -- gravity, hydraullrally loaded fitter with loading rates in the one to three gpm/st range; (b) high rate — a pressure, hydraulicalty loaded filter with loading rates in the five gprrvsf range; At arty rate, the loading rats will exceed three gprrtrsf; (4l) Secondary Clarinets. The process by which organic or volatile matter and sludge is gasified, )liquefied, mineralized or ccrwened into more stable organic matter through the activity of tiring organisms, which Includes aerated holding tanks; (45) Sludge Drying Beds. An area winprsing natural or artificial layers of porous materials upon which digested sewage sludge Is dried by drainage and evaporation; (46) Sludge Elutriation. A process of sludge conditloning in which certain cons: tuents are removed by successive washings with fresh water or pliant effluent; (47) Sludge Gas Utlllzalion- The process of using sewage gas for the purpose of heating buildings, driving engines, sit-; (48) Sludge HOidng Tank (Aerated and Nonaerated). A tank utiitzed for small wastewater treatment plants not containing a digester In which sludge may be kept fresh, and supernatant withdrawn prior to a drying method (i.e. sludge drying beds); This may be done by adding a small amount of air simply to keep the Sludge fresh, but not necessarily an arncurtt that would be required to achieve stabilization of organic matter.
