Please find enclosed the corrected Page 6 of Permit No.WQ0011413 for the subject facility. The following
correction has been made to the permit:
1. The monitoring frequency under Section U1.5 has been reduced to once permit cycle instead of annual
monitoring as specified originally. This is being done following your reminder that such a request was
submitted to this office some time before the recent renewal. Therefore, such request was inadvertently
not considered during this renewal process but does qualify to have the reduced monitoring frequency
as requested.
Please replace the original Page 6 with the corrected Page 6 (enclosed) into Permit No. WQ0006101 issued
September 15.2000.

A Toxicity Characteristics Leaching Procedure (TCLP) analysis shall be conducted by the Permittee

6. All residuals included in t�is permit must be monitored annually from the date of permit issuance, for
compliance with condition I 11 of this permit. Data to verify stabilization and vector attraction
reduction of the residuals must be maintained by the Permittee. The required data is specific to the
stabilization process utilized, but should be sufficient to clearly demonstrate compliance the Class A
pathogen requirements in 40 CFR Part 503.32(a) or with the Class B pathogen requirements and site
restrictions in 40 CFR Part 503.32(b), and one of vector attraction reduction requirements in 40 CFR
Part 503.33. In addition, the EPA certification statements concerning compliance with pathogen
requirements, vector attraction reduction requirements and management practices must be completed
annually by the proper authority or authorities if more than one is involved, either the person who
prepares the residuals, the person who derives the material, or the person who applies the residuals.
7. Three copies of all required monitoring and reporting requirements as specified in conditions lII 1, III
2, 1113, III 4,1115, and III 6 shall be submitted annually on or before March 1 of the following year to
the following address:
