Box 520 Old Fort, NC 28762 ' ffl�.�VA qT 0 �EHNR Subject: Permit No. WQ0011260 Town of Old Fort Distribution of Class A Residuals McDowell County Dear Mayor Stafford: This is in reference to a letter from McGill Associates received November 16, 1995 for the modification of the above subject facility. The modification was a redesign which will move the lime pasteurization equipment to a covered outdoor area. The remainder of the project remains unchanged. After review of the submitted plans showing the modification to the design and the subject permit, the Division determined that a modification of the permit will not be required and by this letter approves this modification of relocation to an outdoor covered area for the lime pasteurization equipment. Please be advised that construction and/or operation of wastewater collection, treatment, and/or disposal facilities without a valid permit or modification approval from the Division is a violation of North Carolina General Statute 143-215.1 and may subject the Town of OId Fort to appropriate enforcement actions in accordance with North Carolina General Statute 143-215.6. Civil penalties of up to $10,000 per day per violation may be assessed for failure to secure a permit required by North Carolina General Statute 143-215.1.
