In response to information received on May 10, 2006 from Mr. Eric Thompson, SYNAGRO, regarding
the newly certified permit dated May 3, 2006, we are re -issuing the attachment A with corrections made to
reflect the proper sampling frequencies and exemptions for the sludge land application permit.
Please replace the existing Attachment A for Permit No. WQ0011038, (previously issued on May
3, 2006) with the corrected Attachment A, attached. Only the conditions have been changed/corrected. It
is the Permittee's responsibility to ensure inserting this modified Attachment A into the subject permit and
creating a complete permit.
This modified Attachment A shall void Attachment A of Permit No. WQ0011038, that was previously
certified on December 1, 2005, and shall be subject to the conditions and limitations as specified therein and
well as those specified in the above -referenced permit.
If any parts, requirements, and/or limitations contained in this certification or modified Attachment A are
unacceptable, you have the right to request an adjudicatory hearing upon written request within 30 days
following receipt of the certification. This request shall be in the from of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes and filed with the Office of Administrative Hearings at
6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such demands are made, this permit
shall be final and binding.
