This Office has reviewed your Pre Construction Notification application and is placing your
project on hold until we receive the following additional information:
1. Please show the exact location of the house, garage, driveway and footprint of the fill on the
lot survey. The plans submitted (exhibit A and exhibit B) do not match. In addition please
demonstrate that you have minimized the impacts to wetlands to the maximum extent
practicable. If exhibit A is what you are proposing, it appears the house could be moved
toward the street, reducing the total impact area.
2. Please provide details of how you plan to stabilize the fill area. For example, if the fill will be
stabilized by grading 3:1 sideslopes and seeding with permanent vegetation, please show this
on a plan detail.
3. Please provide this office with a Jurisdictional Determination Map signed by the US Army
Corps of Engineers (COE). The COE permit authorizes the discharge of fill to access uplands
in the back of the property. This does not appear to be what you are proposing. Please clarify.
Please respond within three weeks of the date of this letter by sending a copy of the information to

Project Information
Attach a vicinity map clearly showing the location of the property with respect to local
landmarks such as towns, rivers, and roads. Also provide a detailed site plan showing property
boundaries and development plans in relation to surrounding properties. Both the vicinity map
and site plan must include a scale and north arrow. The specific footprints of all buildings,
impervious surfaces, or other facilities must be included. If possible, the maps and plans should
include the appropriate USGS Topographic Quad Map and NRCS Soil Survey with the. property
boundaries outlined. Plan drawings, or other maps may be included at the applicant's discretion,
so long as the property is clearly defined. For administrative and distribution purposes, the
USACE requires information to be submitted on sheets no larger than 11 by 17-inch format;

VI. Proposed Impacts to Waters of the United States/Waters of the State
