However, additional information is required before the review may be completed. Please address the items on the attached pages no later than the close of business on September 9, 2015. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items in Sections A through B, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete. Site Map: 1. Please provide a vicinity map that meets all criteria in the "Map Guidance for Residuals Land Application Permits." B. Residuals Management Plan: 1. Process flow diagram and/or narrative — A detailed description of how residuals are generated, treated, processed, and stored at the facility. 2. Quantitative justification for residuals production rate — Ensure that the maximum amount expected to be generated by the facility for the life of the permit is listed. Because the quantity has not changed since the last permit, inclusion of justification from previous submission is adequate together with annual quantities generated over the course of the permit. 3. Sampling plan — There is no description of how samples are developed, i.e., sampling points (in time or space), sampling frequency, and Division -certified laboratories to be used. Grab and composite sampling should be described separately. Plan should also state how pathogen reduction and vector attraction reduction requirements are to be met. Finally, sampling and monitoring must be completed on the residuals as they are to be land applied.
