OF NC POST OFFICE BOX 240908 CHARLOTTE, NORTH CAROLINA 28224-0908 Dear Mr. Lashua: Dee Freeman Secretary Subject: Attachment A Modification Permit No. WQ0009772 Monterey Shores WWTP High -Rate Infiltration System Currituck County In accordance with your permit minor modification request received October 3, 2012, we are forwarding herewith a modified Attachment A of Permit No. WQ0009772 dated November 9, 2012, to Carolina Water Service, Inc. of NC for the continued operation of the subject wastewater treatment and high -rate infiltration facilities. This permit was modified to reduce the effluent (PPI 001) fecal coliform monitoring from three times a week to one time a week at PPI 001. Please replace the existing Attachment A of Permit No. WQ0009772 that was previously certified on September 6, 2012, with this modified Attachment A. It is the Permittee's responsibility to ensure that a complete permit is created by inserting this modified Attachment A into Permit No. WQ0009772, issued September 6, 2012. This modified Attachment A shall be effective from the date of certification; shall void Attachment A of Permit No. WQ0009772 that was previously certified on September 6, 2012; shall be subject to the conditions and limitations as specified therein and well as those specified in the above referenced permit. As always, remember to take the time to review this modified Attachment A thoroughly. Note that this modified Attachment A is hereby incorporated by reference into Permit No. WQ0009772, issued on September 6, 2012.
