In accordance with a written request received on April 13, 2017 from Danny Lassiter, we are
forwarding herewith a corrected Attachment A of Permit No. WQ0009772 issued May 12, 2016.
The corrections to the subject permit are as follows:
1. Volatile Compounds (NDVOC) and Turbidity (00076) have been removed from the sampling
requirements in PPI 002 in Attachment A.
Please replace Attachment A of Permit No. WQ0009772 issued May 12, 2016 with the
enclosed corrected Attachment A.

1. 3 x Year sampling shall be conducted every March, July and November.
2. Prior to construction and operation of the newly permitted Phase III facilities, the flow shall be limited to 580,320 GPD.
3. Upon construction, certification and operation of the newly permitted Phase III facilities, the flow shall be limited to 720,000 GPD.
