In accordance with your permit minor modification request received October 3, 2012, we are
forwarding herewith a modified Attachment A of Permit No. WQ0009772 dated November 9, 2012, to
Carolina Water Service, Inc. of NC for the continued operation of the subject wastewater treatment and
high -rate infiltration facilities.
This permit was modified to reduce the effluent (PPI 001) fecal coliform monitoring from three
times a week to one time a week at PPI 001.
Please replace" the existing Attachment A of Permit No. WQ0009772 that was previously
certified on September 6, 2012, with this modified Attachment A. It is the Permittee's responsibility
to ensure that a complete permit is created by inserting this modified Attachment A into Permit No.
WQ0009772, issued September 6, 2012.
This modified Attachment A shall be effective from the date of certification; shall void
Attachment A of Permit No. WQ0009772 that was previously certified on September 6, 2012; shall be
subject to the conditions and limitations as specified therein and well as those specified in the above
referenced permit. As always, remember to take the time to review this modified Attachment A
thoroughly. Note that this modified Attachment A is hereby incorporated by reference into Permit No.
WQ0009772, issued on September 6, 2012.
The customized electronic copies of your facility's NDMR and NDAR-2 , reporting forms
available for download at: http://portal.ncdenr.or web/wq/aps/lau/reporting have been updated with
this modification.

If any parts, requirements or limitations contained in this permit are unacceptable, the Permittee
has the right to request an adjudicatory hearing upon written request within 30 days following receipt of
this permit. This request shall be in the form of a written petition, conforming to Chapter 150B of the
North Carolina General Statutes, and filed with the Office of Administrative Hearings at 6714 Mail
Service Center, Raleigh, NC 27699-6714. Unless such demands are made, this permit shall be final and
binding.
