However, additional information is required before the review may be completed. Please address the items on the following page no later than the close of business on November 19, 2014. Please be aware that you are responsible for meeting all requirements set forth in North Carolina rules and regulations. Any oversights that occurred in the review of the subject application package are still the Applicant's responsibility. In addition, any omissions made in responding to the outstanding items, or failure to provide the additional information on or before the above requested date may result in your application being returned as incomplete. Please reference the subject application number when providing the requested information. All revised and/or additional documentation shall be signed, sealed and dated (where needed), with three copies submitted to my attention at the address below. Please amend Application Item VH.9.b. to include the correct high -rate infiltration basin surface area at the freeboard water elevation. 2. Please complete the Applicant's Certification on Page 13 of the resubmitted High -Rate Infiltration System Application (FORM: HRIS 11-13). 3. As requested in the July 18, 2014 additional information letter, per Application Instruction I, please submit two 11 x 17 -inch plan sets that have been signed, sealed and dated by a North Carolina licensed Professional Engineer. Please ensure that these plan sets include the September 8, 2014 revisions. 4. Please address the following concerns regarding the proposed green area: a. The comment response and/or Sheet C-800 did not include any documentation that the Permittee has access rights to the proposed additional green area. According to the Currituck County GIS, the northwest portion of the proposed green area is owned by Currituck County. Please provide documentation of access for the proposed green area b. Per 15A NCAC 02H .0404(g)(7), the designated green area to be suitable for waste disposal. Based on a comparison of Sheet C-800 to aerial photographs, the proposed green area contains several ponds, a canal, and a marina, as well as areas what appear to be wetlands. Also, in some locations the proposed green area appears to be shown to extend off the shoreline'west into the sound. Please remove these areas from the proposed green area, and amend the plans and application accordingly. c. Per 15A NCAC 02H .0404(g)(7), the use of streets and roadways as green area are prohibited. Plan Sheet C-800 shows streets, driveways, and parking lots included in the proposed green area. Please remove these areas from the proposed green area, and amend the plans and application accordingly. d. Plan Sheet C-800 identifies some portions of the proposed green area as existing irrigation areas. Please clarify this designation, and amend the plans and application if necessary. 5. In the Hydrogeologist's letter included in the additional information package, it is recommended that the upper portion of sand from the infiltration basin be removed and replaced this fall. Also, the notes for the profile view of the basin, shown on revised Plan Sheet C-802, state that the upper 2 feet of sand will be removed and replaced. Meanwhile, the comment responses for the Operation and Maintenance Plan propose future inspection and possible maintenance of the basin, there is no mention of any proposed work in the near future. Please amend the Operation and Maintenance Plan to clarify the short term and long term maintenance plans for the high -rate infiltration basins. 6. The ;flow allocation spreadsheet included in the additional information package shows different amounts of flow available to Outer Banks Ventures. Specifically, on one line this flow is given as 176;496 GPD, and on another line it is given as 186,496 GPD. Please clarify the actual amount of the flow allocation for Outer Banks Ventures.
