In accordance with an email request received on November 1, 2005 and faxed information received on
November 4, 2005 from Mary K. Buck with Synagro, Inc., we are forwarding herewith a corrected
Attachment B issued November 10, 2005, for Permit No. WQ0008764, issued October 14, 2005.
The following corrections have been made to attachment B: -
1. Fields NC-RO-04-07 thru NC-RO-04-13 are owned by John D. Carmichael.
2. Field NC-RO-10-04 is owned by Kenneth Collins and operated by Alan Locklear.
3. Field NC-RO-12-01 is identified correctly.
4. Field NC-SC-02-08 is operated by Thurman Locklear.
5. Field NC-SC-12-01 is owned by Robert Morgan.
The receipt of a revised scaled site map for Site NC-SC-02-14, as requested by permit condition IV.4.c of
the permit issued October 14, 2005, is hereby acknowledged. The following corrections have been made
to attachment B in response to submittal of the revised map for site NC-SC-02-14:
1. The acreage for field NC-SC-02-14 has been revised to 138.00 acres from 149.50 acres. The
total for Scotland County has been revised to 863.20 acres from 874.70 acres. The total acreage
available for land application has been revised to 2,476.50 acres from 2488.00 acres.
Please replace Attachment B in Permit No. WQ0008764, issued October 14, 2005 with the
corrected Attachment B issued November 10, 2005. Please replace the field map for site NC-SC-
02-14 in the attachments to Permit No. WQ0008764 issued October 14, 2005 with the enclosed field
map for this site.
