Dear Mr. Mosely:
In accordance with a telephone request received on October 1, 2001 from Mr. Steve Wilson of Georgia-
Pacific Corporation's Columbia, SC Field Engineering Office, we are forwarding herewith a corrected Permit No.
WQ0008087, issued on October 8, 2001, for the subject wastewater recycle facilities.
The foIIowing corrections/modifications have been made to the permit:
1. The wastewater recycle facilities for the evaporation of kiln condensate were erroneously omitted from
the system description of the permit that was renewed on April 1, 1998. These facilities were
originally approved by the Division of Water Quality (Division) for construction and operation in the
July 9, 1996 modification of this permit. The continued operation of these facilities has been approved
in this reissuance of this permit, as was originally intended by the permit renewal application that was
received by the Division on October 30, 1997. Please accept our apology for any confusion that this
omission may have caused you.
Even though it was necessary to completely reissue this permit to address this correction, no other
substantive changes have been made to the conditions of this permit other than the following:
Condition No. 1.: This condition is slightly modified from a similar one that was included in the
permit modification issued on July 9, 1996. Normally, the Division verifies that this certification
has been received during the permit renewal process; however, since the wrong issuance of this
permit was utilized during that process, we cannot be sure that such verification was performed by
the Division's reviewer. Furthermore, a review of our files and database did not indicate that this
certification was received, and the Division would like to have a copy of it on file.
Condition No. 3.: This condition was in the permit modification issued on July 9, 1996, but was
omitted from the permit renewal issued on April 1, 1998. Because this condition is specific to the
wastewater recycle facilities that were erroneously omitted from the renewed permit, it is being
included in this permit reissuance.
Condition No. 8.: This condition has been amended to reflect the adoption of new rules by the
Water Pollution Control System Operators Certification Commission (WPCSOCC). The
referenced regulation is now 15A NCAC. SG .0202.

Please replace Permit No. WQ0008087, issued on April 1, 1998, with this corrected version of Permit
No. WQ0008087. This permit shall be effective from the date of issuance until March 31, 2001; shall void Permit
No. WQ0008087, issued on April 1, 1998; and shall be subject to the conditions and Iimitations as specified
therein. Please take time to review this permit carefully. Pay particular attention to the monitoring requirements
in, as failure to establish an adequate system, for collecting and maintaining the required information will result in
future compliance problems.
If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right to
request an adjudicatory hearing upon written request within 30 days following receipt of this permit. This request
must be in the form of a written petition, conforming to Chapter 150E of North Carolina General Statutes, and filed
with the Office of Administrative Hearings, 6714 Mail Service Center, Raleigh, NC 27699-6714. Unless such
demands are made, this permit shall be final and binding.

WASTEWATER RECYCLE SYSTEM PERMIT
In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and
other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO

FOR THE
continued operation of 35-gallon per day wastewater recycle facilities for the treatment of equipment washdown
that consists of the following minimum components: a 30-foot by 48-foot wash pad with protective canopy; a 11-
foot by 25-foot clean -out pit; a protected 11-foot by 23-foot equipment room; an Ultrasorb Model LD package
treatment system containing an aeration tower, a 1/12-horsepower air compressor, a physical separation device (i.e.,
inclined tub coalescer with mechanical skimmer), a hydrocarbon accumulator, a multimedia filter with over 200
pounds of carbon, 100 pounds of ion exchange media, and 100 pounds of vulcansorb, a one -micron polishing filter,
and a turbohydrozone (i.e., ozone generator); a tablet chlorinator; a 550-gallon storage tank; a 30-gallon pressure
tank; and all associated piping, valves, electrical items, instrumentation/controls, and other appurtenances necessary
to make complete and operational wastewater recycle facilities and
continued operation of 500-gallon per day wastewater recycle facilities for the evaporation of kiln condensate that
consists of the following minimum components: a 2,783-gallon concrete evaporation pit, heated by steam piping
connected to the existing system and with an evaporative capacity of 810 gallons per hour (i.e., equivalent to 6,743
pounds per hour) and all associated piping, valves, electrical items, instrumentation/controls, and other
appurtenances necessary to make complete and operational wastewater recycle facilities.
Both of the wastewater recycle and wastewater recycle/evaporative facilities are intended to serve the Whiteville,
North Carolina Facility of the Georgia-Pacific Corporation with no discharge of wastes to the surface waters,
pursuant to the application received on October 30, 1997, and in conformity with the project plan, specifications,
and other supporting data subsequently filed and approved by the Department of Environment and Natural
Resources and considered a part of this permit.
This permit shall be effective from the date of issuance until March 31, 2003; shall void Permit No.
WQ0008087, issued on April 1, 1998; and shall be subject to the following specified conditions and limitations:
1. Within 30 calendar days of the issuance of this permit, a certification shall be received from a North
Carolina -licensed professional engineer certifying that the wastewater recycle facilities associated with
the evaporation of kiln condensate have been installed in accordance with this permit, the approved
plans and specifications, and other supporting materials. if the facilities are to be completed in phases
and partially certifiedyou shall retain the responsibility to track further construction approved under
the same permit. and shall provide a final certificate of completion once all of the facilities have been
constructed. Mail one copy of the Certification to the NCDENR-DWQ, Non -Discharge Permitting
Unit, 1617 Mail Service Center, Raleigh, NC 27699-1617.
2. The wastewater recycle facilities shall be effectively maintained and operated as a non -discharge
system to prevent the discharge of any wastewater resulting from the operation of the facilities.
3. Freeboard in the evaporation pit associated with the 500-gallon per day wastewater recycle facilities for
the evaporation of kiln condensate shall not be less than six inches at any time. The Permittee shall
take appropriate actions to prevent this pit from overflowing.
4. In the event that the wastewater recycle facilities fail to perform satisfactorily, including the creation of
nuisance conditions, the Pemlittee shall take immediate corrective action, including those actions that
may be required by this Division, such as the construction of additional or replacement facilities.
5. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to surface
waters or groundwater resulting from the operation of the wastewater recycle facilities.
6. All residuals generated by the wastewater recycle facilities shall be disposed in accordance with
General Statute § 143-215.1 and in a manner approved by the Division.
7. Diversion or bypassing of untreated wastewater from the wastewater recycle facilities shall be
prohibited.
8. The wastewater recycle facilities shall be properly maintained and operated at all times.
9. Upon classification of the wastewater recycle facilities by the Water Pollution Control Systems
Operators Certification Commission (WPCSOCC), the Permittee shall employ a certified operator to be
in responsible charge (ORC) of the facilities. The operator shall hold a certificate of the type and grade
at least equivalent to or greater than the classification assigned to the facilities by the WPCSOCC. The
Permittee shall also employ a certified back-up operator of the appropriate type and grade to comply
with the conditions of 15A NCAC 80 .0202. The ORC of the facilities shall visit Class I facilities at
least weekly and Class II, III, and IV facilities at least daily, excluding weekends and State Holidays;
shall properly manage and document daily operation and maintenance of the facilities; and shall
comply with all other conditions of 15A NCAC 8G .0202.
10. Any monitoring deemed necessary by the Division to ensure surface water and groundwater protection
shall be established and an acceptable sampling reporting schedule shall be followed.
11. Noncompliance Notification:
The Permittee shall report by telephone to the Water Quality Section of the Division's Wilmington
Regional Office, telephone number (910) 395-3900, as soon as possible, but in no case more than 24
hours or on the next working day following the occurrence or first knowledge of the occurrence of any
of the following:
a. Any occurrence at the wastewater recycle facilities that results in the treatment of significant
amounts of wastes which are abnormal in quantity or characteristic, such as the dumping of the
contents of a basin or tank, the known passage of a slug of hazardous substance through the
facilities, or any other unusual circumstances;

b. Any process unit failure, due to known or unknown reasons, that renders the wastewater recycle
facilities incapable of adequate wastewater treatment, such as mechanical or electrical failures of
pumps, aerators, compressors, etc.;
c. Any failure of a pumping station, sewer line, or treatment components resulting in a by-pass
directly to receiving waters without treatment of all or any portion of the influent to such station,
line, or component; or
d. Any time that self -monitoring information indicates that the wastewater recycle facilities are not in
compliance with its permit limitations.
Persons reporting such occurrences by telephone shall also file a written report in letter form within
five days following its first knowledge. This report shall outline the actions taken or proposed to be
taken to ensure that the problem does not recur.
12. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to ensure proper
operation of the subject facilities.
13. The Permittee or his designee shall inspect the wastewater recycle facilities to prevent malfunctions
and deterioration, operator errors, and discharges that may cause or lead to the release of wastes to the
environment, a threat to human health, or a nuisance. The Permittee shall maintain an inspection log or
summary including, at a minimum, the date and time of inspection, observations made, and any
maintenance, repairs, or corrective actions taken. This log of inspections shall be maintained by the
Permittee for a period of three years from the date of the inspection and shall be made available to the
Division or other permitting authority upon request.
14. Any duly authorized officer, employee, or representative of the Division may, upon presentation of
credentials, enter and inspect any property, premises or place on or related to the wastewater recycle
facilities at any reasonable time for the purpose of determining compliance with this permit, may
inspect or copy any records that must be maintained under the terms and conditions of this permit, and
may obtain samples of groundwater, surface water, or leachate.
15. This permit shall become voidable unless the wastewater recycle facilities are constructed in
accordance with the conditions of this permit, the approved plans and specifications and other
supporting data.
16. This permit shall be effective only with respect to the nature and volume of wastes described in the
application and other supporting data.
IT This permit shall not be transferable. In the event there is a desire for the wastewater recycle facilities
to change ownership, or there is a name change of the Petnrittee, a formal permit request shall be
submitted to the Division) accompanied by documentation from the parties involved and other
supporting materials as may be appropriate. The approval of the request shall be considered on its
merits and may or may not be approved.
18. Failure to abide by the conditions and limitations contained in this permit shall subject the Permittee to
an enforcement action by the Division in accordance with North Carolina General Statutes §143-
215.6A through § 143-215.6C.
19. The issuance of this permit shall not preclude the Permittee from complying with any and all statutes,
rules, regulations, or ordinances which may be imposed by other government agencies (i.e., local, state,
and federal) that have jurisdiction, including, but not limited to applicable river buffer rules in 15A
NCAC 2B .0200, soil erosion and sedimentation control requirements in 15A NCAC Ch. 4 and under
the Division's General Permit NCG0100000, and any requirements pertaining to wetlands under 15A
NCAC 2B .0200 and 15A NCAC 2H .0500.
20. A set of the approved plans and specifications shall be retained by the Permittee for the life of the
wastewater recycle facilities.
21. The annual administering and compliance fee shall be paid by the Permittee within 30 days after being
billed by the Division. Failure to pay the fee accordingly may cause the Division to initiate action to
revoke this permit as specified by 15A NCAC 2H .0205 (c)(4).
22. The Permittee, at least six months prior to the expiration of this permit, shall request its extension.
Upon receipt of the request, the Division shall review the adequacy of the wastewater recycle facilities
described therein and, if warranted, shall extend the permit for such period of time and under such
conditions and limitations as it may deem appropriate.
