In accordance with a telephone request received on this date from Mrs. Friedhoff, we are forwarding
herewith a corrected cover page and Pages I and 3 of the subject permit, issued August 28, 2000, for the
subject facility.
The following corrections have been made to the permit:
1. The official deed and tax maps refer to the property as Lot 2B whereas all permits issued refer to the
property as Lot 2A. There are previous plat maps that refer to the property as Lot 2A. The Division
has verified that the lot permitted is the lot in question.
Please replace the cover page and Pages 1 and 3 of the permit with the enclosed corrected pages
into Permit No. WQ0008030, issued August 28, 2000. Note that the expiration date of this permit
remains the same.
