Dear Mr. Owens:
In accordance with your permit modification application package received on November S, 2010,
we are forwarding herewith a modified Attachment B, certified December 20, 2010, to the Town of
Carolina Beach for the subject residuals land application program.
This certification is being issued to modify Attachment B to:
Permit an additional seven sites (Sites NC-CO-30-01 through NC-CO-30-07) totaling 77.8
acres of applicable land in Columbus County.
Reduce the residual application setback distances for these new sites to 100 feet from
habitable residences.
Please replace the existing Attachment B of Permit No. WQ0007728 that was previously
certified on August 3, 2009, with this modified Attachment B. It is the Permittee's responsibility to
ensure that a complete permit is created by inserting this modified Attachment B and the accompanying
site maps into Permit No. WQ0007728, issued on August 3, 2009, and making sure to retain any vicinity
and buffer maps for any existing and unmodified land application sites.
This modified Attachment B shall be effective from the date of certification; shall void
Attachment B of Permit No. WQ0007728 that was previously certified on August 3, 2009; and shall be
subject to the conditions and limitations as specified therein and well as those specified in the above -
referenced permit. As always, remember to take the time to review this modified Attachment B
thoroughly, as some of the conditions contained therein may have been changed since the last
certification. Note that this modified Attachment B is hereby incorporated by reference into Permit No.
WQ0007728. issued on August 3, 2009.

If any parts, requirements, andlor limitations contained in this certification or modified
Attachment B are unacceptable, you have the right to request an adiudicatory hearing upon writtenAwk
request within 30 days following receipt of the certification. This request shall be in the from of a written
petition. conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of
Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such
demands are made. this permit shall be final and binding.
On August 5. 2009, Session Law 2009-406. entitled "An Act to Extend Certain Government
Approvals Affecting the Development of Real Property Within the State," was enacted by the General
Assembly and signed into law. The Act, known as the Permit Extension Act of 2009, extends the
expiration date of certain government approvals and permits. On August 2, 2010, this Act was extended
an additional year. The subject permit fails within the scope of this Act and is therefore being
extended to May 31, 2015.

NOTE: The depth to the water table must be confirmed on Fields NC-CO-17-4A and NC-CO-17-4B prior to the application of residuals. Soil
testing in Fields NC-CO-17-3, NC-CO-17-4A and NC-CO-17-4B has indicated that these fields have low pH characteristics. They shall be
treated annually by the application of lime in liquid form, or disced into the soil if the lime is applied to the soil in powder form.
The setback distances from habitable residences for Fields NC-CO-30-1 through NC-CO-30-7 have been reduced to 100 feet by the written
consent of the neighboring property owners.
