In accordance with your permit modification application package received on February 22, 2012,
we are forwarding herewith a modified Attachment A, certified March 6, 2012, to the Town of Carolina
Beach for the subject residuals land application program.
This certification is being issued to modify Attachment A by increasing the annual dry tonnage of
the residuals generating at the Carolina Beach WWTP for land application from 180 to 319.
Please replace the existing Attachment A of Permit No. WQ0007728 that was previously
certified on August 3, 2009, with this modified Attachment A. Note that it is the Permittee's
responsibility to ensure that a complete permit is created by inserting this modified Attachment A into
Permit No. WQ0007728, issued on August 3, 2009.
This modified Attachment A shall be effective from the date of certification; shall void
Attachment A of Permit No. WQ0007728 that was previously certified on August 3, 2009; and shall be
subject to the conditions and limitations as specified therein and well as those specified in the above -
referenced permit. Note that this modified Attachment A is hereby incorporated by reference into Permit
No. WQ0007728, issued on August 3, 2009.
If any parts, requirements, and/or limitations contained in this certification or modified
Attachment B are unacceptable, you have the right to request an adjudicatory hearing upon written
request within 30 days following receipt of the certification. This request shall be in the form of written
petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of
Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such
demands are made, this permit shall be final and binding.

Monitoring frequencies are based on the actual dry tons applied per year, unless specified. The Monitoring Frequency Requirements Table below shall be used to determine
appropriate sampling frequency. The Permittee is responsible for managing program to assure that adequate sampling is performed.

If no land application events occur during a required sampling period (e.g. no land application occur during an entire year when annual monitoring is

clarification.
