Dear Mr. Cousineau:
Reference is made to your request for rescission of the subject State Permit. Staff of the
Raleigh Regional Office - Groundwater Section have confirmed that this NonDischarge Permit
is no longer required. Therefore, in accordance with your request, State Permit No.
WQ0007628 is rescinded, effective immediately.
If in the future you wish to remediate groundwater as permitted, you must first apply for
and receive a new State Permit. Operating without a valid State Permit will subject the facility to
a civil penalty of up to $10,000 per day.

Dear Mr. Cousineau: f
The following letter is submitted to advise you that the
Division. of Water Quality's Raleigh Regional Office (RRO)
Groundwater Section is closing the subject incident file. The
facility is located at 5036 Walter Kidde Drive, Wake Forest, NC.
A review of the file and information submitted by Kidde-
Fenwal, Inc. indicates that contamination at the subject facility
has been remediated to the extent determined necessary to comply
with current State regulations and guidelines. Kidde-Fenwal, Inc.
demonstrated in four consecutive quarterly monitoring reports over
a period of one year, from March 29, 1995, through March 27, 1996,
that targeted contamination levels were below the groundwater
quality standard concentration levels established by Title 15A NCAC
2L .0202, thus satisfying the regulatory requirements for site
closure.
Please note that this office may not have conducted a site
visit prior to closure and is relying on the accuracy of the
information submitted by you and your agent to make this
determination.
Kidde-Fenwal, Inc. is required under Title 15A NCAC 2C .0113
to permanently abandon all monitoring wells at the site in
accordance with the procedures described in Title 15A NCAC 2C
.0113(a)(2) and is required under Title 15A NCAC .0114(b) to submit
to this office a certified record of the abandonment of each well
on Gov`-30 forms provided by the Division of Water Quality within 30
days of abandonment.
in closing this file, the Section reserves the right to reopen
its investigation should information become available which
indicates the cleanup at this site does not meet regulatory
requirements for site closure.
