Please note the following permit conditions, expiration date and attachments have been modified since the last permit issuance: . ➢ Permit Expiration Date — The expiration date has been extended from May 31, 2012 to May 31, 2015 due to Session Law 2009-406 - An Act to Extend Certain Government Approvals Affecting the Development of Real Property within the State. For more information, please visit: htto://nortal.ncdenr.orz/web/wc/aps/lau#Extension. ➢ Condition 11.14. (i.e., old Condition II.11.) — The setbacks for the high -rate infiltration units were inadvertently changed during the December 5, 1997, July 24, 2002 and June 20, 2007 permit issuances, and the setbacks for the existing treatment units were inadvertently changed during the June 20, 2007 permit issuance. Accordingly, the setbacks for these units have been restored to their original 15A NCAC 02H. 0200 requirements (See Conditions II.14.a. and II.14.b.). Please note the new treatment facilities permitted under this modification request are subject to the setback requirements in 15A NCAC 02T .0706(d), and may be found in Condition 11.14.c.
