Subject: NPDES Permit Issuance
Permit No. NCO003395
Wright Corporation
Columbus County
On September 4, 1996 the Division of Water Quality issued NPDES Permit No. NCO003395 to Wright
Corporation for its hexamine manufacturing facility in Columbus County. A review of the permit file has indicated
that several errors were inadvertently made in the permit. Accordingly, we are forwarding herewith modifications to
the subject permit to correct the errors. These permit modifications are to the supplement to permit cover sheet and
effluent page for outfall 001. The modifications remove the reference to the sulfuric acid facility from the description
of outfall 001 on the supplement page, change the term "Total Suspended Residue" to "Total Suspended Solids" and
remove "(Sumner)" found with the ammonia requirement from the effluent page.
This permit has also been modified to include reuse/recycle permit number WQ0006565. Conditions of the
non -discharge permit can be found in Part III, Section G. With the 'issuance of this modified permit, non -discharge
permit WQ0006565 is hereby rescinded.
Please find enclosed an amended effluent sheet, supplement page and special conditions page which should
be inserted into your permit. The old pages should be discarded. All other terms and conditions contained in the
original permit remain unchanged and in full effect. These permit modifications are issued pursuant to the
requirements of North Carolina General Statutes 143-215.1 and the Memorandum of Agreement between North
Carolina and the U. S. Environmental Protection Agency.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to
you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following redeipt of
this letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North
Carolina General Statutes, and filed with the office of Administrative Hearings, Post Office Drawer 27447, Raleigh,
North Carolina 27611-7447. Unless such a demand is made, this permit shall be final and binding.
Please take notice that this permit is not transferable. Part II, EA. addresses the requirements to be
followed in case of change in ownership or control of this discharge.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Water Quality or permits required by the Division of Land Resources, Coastal Area Management Act, or
any other Federal or Local governmental permits may be required.

Continue to discharge treated hexamine process wastewater, cooling water, boiler
blowdown and softner backwash from a treatment system consisting of a mixing
tank, distillation unit, deionization units, bioreactor, clarifier, polymer feed tank and
bag filters from facilities located at Wright Corporation, NCSR 1818, Acme,
Columbus County (See Part III of this Permit) and
2. Discharge from said treatment works at the location specified on the attached map
into Livingston Creek which is classified Class C -Swamp waters in the Cape Fear
River Basin.

E. Chronic Toxicity Permit Limit (Quarterly)
The effluent discharge shall at no time exhibit 48 hour acute toxicity as lethality in an effluent
concentration of 34% nor measure a quarterly arithmetic average chronic value less than this same
percentage of waste. The chronic value will be determined using the geometric mean of the highest
concentration having no statistically detectable impairment of reproduction or survival and the
lowest concentration that does have a statistically detectable impairment of reproduction or
survival. The presence of 48 hour acute toxicity will be determined using Fisher's Exact Test at 48
hours from test initiation. Collection methods, exposure regimes, and further statistical methods
are defined in: The North Carolina Phase II Chronic Whole Effluent Toxicity Test Procedure
(July, 199 1) or subsequent versions.
The permit holder shall perform at a minimum, quarterly monitoring using these procedures to
establish compliance with the permit condition. The first test will be performed within thirty days
from the effective date of this permit during the months of February, May, August, and
November. Effluent sampling for this testing shall be performed at the NPDES permitted final
effluent discharge below all treatment processes.
If the test procedure performed as the first test of any single quarter measures 48 hour acute
toxicity or a chronic value less than that specified above, then multiple concentration testing shall
be performed, at a minimum, in each of the two following months.
All toxicity testing results required as part of this permit condition will be entered on the Effluent
Discharge Monitoring Form (MRA) for the months in which tests were performed, using the
parameter code THP313 for the Chronic Value and TGA313 for the 48 hour Acute Toxicity measure
(Pass/Fail). Additionally, DEM Form AT -3 (original) is to be sent to the following address:

Test data shall be complete and accurate and include all supporting chemical/physical measurements
performed in association with the toxicity tests, as well as all dose/response data. Total residual
chlorine of the effluent toxicity sample must be measured and reported if chlorine is employed for
disinfection of the waste stream.
Should any test data from this monitoring requirement or tests performed by the North Carolina
Division of Environmental Management indicate potential impacts to the receiving stream, this
permit may be re -opened and modified to include alternate monitoring requirements or limits.
NOTE: Failure to achieve test conditions as specified in the cited document, such as minimum
control organism survival and appropriate environmental controls, shall constitute an invalid test
and will require immediate retesting (within 30 days of initial monitoring event). Failure to submit
suitable test results will constitute noncompliance with monitoring requirements.
F. Biocide Condition
The permittee shall not use any biocides except those approved in conjunction with the permit
application. The permittee shall notify the Director in writing not later than ninety (90) days prior
to instituting use of any additional biocide used in cooling systems which may be toxic to aquatic
life other than those previously reported to the Division of Environmental Management. Such
notification shall include completion of Biocide Worksheet Form 101 and a map locating the
discharge point and receiving stream.

G. Reuse/Recycle Inclusion Condition
The inclusion of reuse/recycle permit number WQ0006565 into this NPDES permit shall no
relieve the Permittee of the responsibility for damages to surface or groundwaters resulting
from the operation of this facility.
Any monitoring deemed necessary by the Division of Water Quality to insure surface and
groundwater protection will be established and an acceptable sampling reporting schedule
shall be followed.
The Permittee or his designee shall inspect the wastewater recycle facilities to prevent
malfunctions and deterioration, operator errors and discharges which may cause or lead to
the release of wastes to the environment, a threat to human health, or a nuisance. The
Permittee shall keep an inspection log or summary including at least the time and date of
inspection, observations made, and any maintenance, repaires, or corrective actions taken
by the Permittee, This log of inspections shall be maintained by the Permittee for a period
of three years from the date of the inspection and she be made available upon request to
the Division of Water Quality or other permitting authority.
A set of approved plans and specifications for the subject project must be retained by the
Permittee for the life of this project.
