23, 2007, we are forwarding herewith the corrected attachment B of Permit No. WQ0006101, issued
October 10, 2007, for the subject facility.
The following corrections have been made to the permit:
1. Attachment B, site identification for lands owned by John Carl Allen is changed from SA -01 to
SA -21.
Please replace Attachment B with the corrected Attachment B into Permit No. W00006101, issued
October 10, 2007.

" These land application sites are located within the 100 -year flood elevation. Therefore, the bulk residuals shall be injected or incorporated within
a 24-hour period following the residuals land application event as stipulated in Condition I1I.9(e).
A re -defined compliance boundary has been approved for this land application site. This re -defined compliance boundary is on the south of the
field and shall be located at 50 feet from the water supply well on Harold Eldridge's property. However, the 100 -foot setback between wells and
the land application area must still be met.
