In accordance with your permit modification application package received on March 26,2003 as
well as the additional information received on April 9,2003; we are forwarding herewith a modified
Attachment B, certified May 12,2003, to City of Dunn for the subject residuals land application program.
This certification is being issued to modify Attachment B to approve an additional 656.8 acres to
the existing residuals land application program.
Please replace the existing Attachment B of Permit No. WQ0006101 that was previously
certified on April 7, 2003, with this modified Attachment B. Note that only the vicinity and buffer
maps for any new and modified land application sites are attached to this modified Attachment B. It is
the Perrnittee's responsibility to ensure that a complete permit is created by inserting this modified
Attachment B and vicinity and buffer maps into Permit No. WQ0006101 issued on May 12,2003, and
making sure to retain any vicinity and buffer maps for any existing and unmodified land application sites.
This modified Attachment B shall be effective from the date of certification; shall void
Attachment B of Permit No. WQ0006101 that was previously certified on April 7,200,; and shall be
subject to the conditions and limitations as specified therein and -%yell as those specified in the above -
referenced permit. As always, remember to take the time to li this modified Attachment B
thoroughly, as some of the conditions contained therein may have been changed since the last
certification. Note that this modified Attachment B is hereby incorporated by reference into Permit No.
WQ0006101 issued on April 7,2003.

for Land Application of Residual Solids
(to be used by WQ and GW reviewers)

Make sure the full legal-name(s) of all the owners, lessees, and operators are listed on this sheet.
Ensure that the actual usable acres are listed on this sheet.
The correct total acreage should be listed at the bottom of this table and elsewhere in the package.
2. Ceertication Page (The WQ reviewer places initials at the space in front.of each item):
This page should specific to the sites/fields in the application package and includes certification language
(in general indicating that the findings contained in the report for the site are based on the site conditions
and that all of the statements are true and correct to the best of their knowledge). The Licensed Soil
Scientist (LSS) does not necessarily need to conduct all field work. Work can be performed by'a person
under their direct supervisory control, (see LSS Board Rules : 21 NCAC 69 .0401(b)(2)).
Verify the LSS name, signature, seal, affiliation, and date of certification. North Carolina General .Statutes
Chapter 89F (this statute takes the place of our 2H .0200 rules that provides a different definition of a soil
scientist). Check http://www.ncblss.or�/director.htibl to verify the member status.

Verify that a location map (county road map) is enclosed for each site/field. It should show site/field
location and state or county highways clearly visible on the map for easy directions to the site/field. These

6. Field Buffer Map (GW reviewer concurrence required. Both WQ and GW reviewers place initials at spaces in
front of each item).
For each site/field, verify that the buffer map has all of applicable natural and identifying features clearly
marked on the map:
• north arrow, map scale, legends for various features, site/field ID,
• houses and other structures, surface water features, property lines, wells, rights of way, access'road,
• auger hole borings, and pit locations.
Check and verify that the following buffers are delineated on the buffer map:
• 400 feet from residences or places of public assembly under separate ownership for surface application
method;
• 200 feet from residences or places of public assembly under separate ownership for subsurface residual
injection method; however, the buffer zone requirement may be reduced to a minimum of 100 feet
upon written consent of the owner and the: appropriate regional office;
• 100 feet from any public or private water supply source, waters classified as SA -or SB, and any Class I
or Class II impounded reservoir used as a source of drinking water for both methods; .
• 100 feet from any streams classified as WS or B, any other stream, canal, marsh or coastal waters and
any other.lake or impoundment for surface application;
• 50 feet from any streams classified as WS or B, any other stream, canal, marsh or coastal waters and
any other lake or impoundment for subsurface application;
• 50 feet from property lines for both surface and subsurface application methods;
• 50 feet from public right of ways for both application methods;
• 10 feet from upslope interceptor drains and surface water diversions for both application methods; and
25 feet from downslope interceptor drains, surface water diversions, groundwater drainage systems and
surface drainage ditches for both application methods

buffer requirements to apply within 100 ft. of a property with separate ownership.

include the following information in a tabular format for each horizon:
• horizon designation,
• depth ,
• primary matrix coloration,
• coloration, size, and abundance of all other features (e.g. — mottles) within each horizon,
• texture,
• structure,
• consistence, and
• horizon boundary type.
List the sites/felds that lack any of the above information and request additional information on the
missing information.
The soil map has at least the predominant soil series name (or mapping unit) clearly visible with fields
clearly marked.

Verify that the soil and site/field features in the field are consistent with those described in the Soils Report.
Also, please note if any significant irregularities exist between the information presented in the soils report
and that published in the appropriate county soil survey.

NOTE:
a. Seasonal restrictions (SR): Application may not occur during periods governed under seasonal restrictions
regardless of the actual depth to the water table.
b. Sites with altered hydrology (artificial drainage): In order for an applicant to improve the site's suitability to
receive residuals, the applicant must make a demonstration to the Division that the natural hydrology has been
sufficiently altered to permit such application activities. Such a demonstration may include groundwater modeling, or
two years worth of water table monitoring prior to any application taking place. The applicant can initially submit a
site/field for approval with seasonal restrictions being applied. The applicant can then request a modification based on
additional data or modeling results that demonstrate that sitejfield restrictions might be lifted.
c. Consideration of soil series for seasonal conditions: Use the following criteria to determine if any seasonal
restrictions (SR) on residuals application should be applied to the site/field based on the predominance of a particular
soil series/type (in site/fields where more than one soil series/type are present):
• If less suitable soils are buffered out, then the site/field can be applied upon based on the restrictions of
the remaining less suitable, or if no marginal soils are present, the more suitable soils.
• If less suitable soils are not buffered out, then the entire site/field should be governed by those
restrictions appropriate to the less suitable soils.

AY V/Check the Test Results section for soil pH. Acceptable pH range for most crops is from pH 6 to 9. Liming
at recommended rate should be required if the pH is below 6 or gypsum application should be required if it
is above 9. Most North Carolina soils are acidic and should require liming. Check the lime
recommendations and ensure that lime recommendations are incorporated in the Site/field Description and
Evaluation section. Liming requirements may be a point to emphasize in the approval cover letter.

/ Ensure each crop listed is appropriate for the field and to receive residuals. For example, consider
susceptibility to Zn and Cu soils levels (e.g. — peanuts), subsurface food crops are grown each year which is
not allowed for Class B residuals (see permit conditions II. ), row crops on fields with excessive slopes, etc.
!t Utilize the worksheet: to evaluate the programs capacity from a nutrient standpoint. Utilize a typical crop
for the program or an estimated average PAN to calculate the needed acreage for the program based on the
most recent residuals sample.
