This is in reference to your application received January 16, 1992 for the continued operation of
the subject facility.
In accordance with the issuance of the sewer extension Permit No. WQ0006350 on April 10,
1992, the Division is hereby returning your application and supporting documents. No permit action has
been taken with respect to the application, however, as the application has been reviewed by Division
staff, your application fee can not be returned to you.
Please contact the Raleigh Regional Office for the proper procedure for closing out the spray
irrigation system formerly issued under Permit No. 14873 and the application for renewal under No.
WQ0006074. This activity must be completed prior to the expiration date of July 31, 1992.
