This permit shall be effective from the date of issuance until March 31, 1997 and shall be subject to the conditions and limitations as specified therein. Please pay particular attention to the monitoring requirements in this permit. Failure to establish an adequate system for collecting and maintaining the required operational information will result in future compliance problems. If any parts, requirements, or limitations contained in this permit are unacceptable to you, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of this permit. Sincerely, k -J 0 1 George T.Vverett cc: Carteret County Health Department Wilmington Regional Office, Water Quality Wilmington Regional Office, Groundwater Groundwater Section, Jack Floyd Training and Certification Unit Facilities Assessment Unit NORTH CAROLINA ENVIRONMENTAL MANAGEMENT COMMISSION DEPARTMENT OF ENVIRONMENT, HEALTH AND NATURAL RESOURCES RALEIGH SUBSURFACE DISPOSAL SYSTEM PERMIT In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as amended, and other applicable Laws, Rules, and Regulations PERMISSION IS HEREBY GRANTED TO Carteret County Board of Education Carteret County FOR THE continued operation of a 13,075 GPD subsurface wastewater disposal system consisting of two 1,500 gallon grease traps connected in series to the kitchen drain; an 11,418 gallon capacity baffled septic tank with a 2,479 gallon capacity pump chamber, dual 120 GPM dosing pumps and high-water alarms; an LPP subsurface disposal system with four (4) drain fields covering a total area of approximately 30,500 square feet; and all related piping, valves, fittings, and appurtenances to serve Broad Creek Middle School (formerly Croatan Middle School) with no discharge of wastes to the surface waters, pursuant to the application received December 16, 1991 and in conformity with the project plan, specifications, and other supporting data subsequently filed and approved by the Department of Environment, Health and Natural Resources and considered a part of this permit. This permit shall be effective from the date of issuance until March 31, 1997 and shall be subject to the following specified conditions and limitations: PERFORMANCE STANDARDS 1. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take immediate corrective action, including those as may be required by this Division, such as construction of additional or replacement wastewater treatment or disposal facilities. 2. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to surface or groundwaters resulting from the operation of this facility. 3. This permit shall become voidable if the soils fail to adequately absorb the wastes and may be rescinded unless the facilities are installed, maintained, and operated in a manner which will protect the assigned water quality standards of the surface waters and ground waters. 4. Adequate measures shall be taken to divert stormwater from the disposal area and prevent wastewater runoff from the subsurface disposal field. 5. The sludge generated from these treatment facilities must be disposed of in accordance with General Statute 143-215.1 and in a manner approved by the North Carolina Division of Environmental Management. 6. A usable green area shall be maintained for wastewater disposal. The green area shall have the capability of accommodating the average daily flow of the facility being served without exceeding the loading rates of the green area. A "green area" as defined in 15 NCAC 2H .0404(g)(7) is an area suitable for waste disposal, either in its natural state or which has been modified by planting a vegetative cover of grasses or low growing shrubbery. 7. Diversion or bypassing of the untreated wastewater from the treatment facilities is prohibited. II. -OPERATION D MAINTENANCE REQUIREMENTS The facilities shall be properly maintained and operated at all times. 2. Upon classification of the facility by the Certification Commission, the Permittee shall employ a certified wastewater treatment plant operator to be in responsible charge of the wastewater treatment facilities. The operator must hold a certificate of the type and grade at least equivalent to the classification assigned to the wastewater treatment facilities by the Certification Commission. 3. The application rates to the disposal field shall not exceed 0.45 gallons/ day/ square foot. 4. The facilities shall be effectively maintained and operated as a non -discharge system to prevent the discharge of any wastewater resulting from the operation of this facility. 5. No traffic or equipment shall be allowed on the disposal area except while installation occurs or while normal maintenance is being performed. 6. A suitable cover shall be maintained on the LPP field(s) and the field(s) shall be routinely mowed. The clippings shall be periodically removed to prevent the build-up of thatch. III. MUNITORING AND REPORTING REUIIIREMENTS Any monitoring deemed necessary by the Division of Environmental Management to insure surface and ground water protection will be established and an acceptable sampling reporting schedule shall be followed. 2. This permit is voidable unless the following inspection and maintenance procedures are followed: a. The grease traps and septic tanks shall be inspected monthly for grease and sludge build-up and pumped out as needed, but no less frequently than once per year. The scum layer shall be removed from the pump chamber when the septic tank is cleaned. b. A permanent record of inspections and maintenance shall be kept and made available upon request to the Division of Environmental Management. 2 3. The low pressure lines shall be maintained as necessary (including purging of the lines and pressure testing) in order to ensure that they remain free of obstructing materials and that the pressure head on the laterals remains within the design specifications. The highest and lowest laterals on each subfield shall be checked (and adjusted as necessary) annually to ensure that the pressure heads are within the approved specifications. If adjustments are necessary to the highest and lowest laterals, the remaining laterals in that subfield shall also be checked and adjusted to the approved specifications. Adequate records showing all maintenance shall be maintained by the Permittee. These records should include but are not limited to the following: Items Inspection Frequency a) Individual dosing pump operation. Monthly b) Individual field rotation Monthly c) High water alarms Monthly d) Residual pressure on highest and lowest lateral on each subfield Annually 4. Three copies of all operation and maintenance records (as specified in condition III 2) and all effluent monitoring data (as specified in condition III 3) shall be submitted on or before January 31 of the following year to the following address: NC Division of Environmental Management Water Quality Section Facility Assessment Unit PO Box 29535 Raleigh, NC 27626-0535 5. A record shall be maintained of all residuals removed from this facility. This record shall include the name of the hauler, permit authorizing the disposal or a letter from a municipality agreeing to accept the residuals, date the residuals were hauled, and volume of residuals removed. 6. Noncom liance Notification: The Permittee shall report by telephone to the Wilmington Regional Office, telephone no. 919/ 395-3900 as soon as possible, but in no case more than 24 hours or on the next working day following the occurrence or first knowledge of the occurrence of any of the following: a. Any occurrence at the wastewater treatment facility which results in the treatment of significant amounts of wastes which are abnormal in quantity or characteristic, such as the dumping of the contents of a basin or tank; the known passage of a slug of hazardous substance through the facility; or any other unusual circumstances. b. Any process unit failure, due to known or unknown reasons, that render the facility incapable of adequate wastewater treatment such as mechanical or electrical failures of pumps, aerators, compressors, etc. c. Any failure of a pumping station, sewer line, or treatment facility resulting in a by- pass of all or any portion of the influent to such station or facility. d. Any time that self -monitoring information indicates that the facility has gone out of compliance with its permit limitations. Persons reporting such occurrences by telephone shall also file a written report in letter form within 15 days following first knowledge of the occurrence. This report must outline the actions taken or proposed to be taken to ensure that the problem does not recur. 3 1. Within 90 days of permit issuance, one (1) downgradient monitor well must be installed to monitor groundwater quality. The well shall be constructed such that the water level in the well is never above or below the screened (open) portion of the well at any time during the year. The location and construction details for this well must be approved by the Wilmington Regional Office, from which a well construction permit must be obtained. 2. The one (1), new monitor well, and the three (3) existing monitor wells must be sampled initially after construction and thereafter every March, July, and November for the following parameters: NO3 TDS NO2 Total Suspended Solids TOC pH Ammonia Nitrogen Chloride Water Level Fecal Coliforms Orthophosphate Volatile Organic Compounds - In November only (by Method 1 or Method 2 below) Method 1: Method 623OD ( Capillary - Column), "Standard Methods for the Examination of Water and Wastewater", 17th edition, 1989 Method 2: Method 502.2 " Methods for the Determination of Organic Compounds in Drinking Water", United States Environmental Protection Agency - 600/4- 88/039 The measurement of water level must be made prior to sampling for the remaining parameters. The measuring points (top of well casing) of all monitoring wells shall be surveyed to provide relative elevations of the measuring point for each of the monitoring wells. The depth of water in each well shall be measured from the surveyed point on the top of the casing. If TOC concentrations greater than 10 mg/1 are detected in any downgradient monitoring well, additional sampling and analysis must be conducted to identify the individual constituents comprising this TOC concentration. If the TOC concentration as measured in the back�oun_d monitor well exceeds 10 mg/l, this concentration will be taken to represent the naturally occurring TOC concentration. Any exceedances of this naturally occurring TOC concentration in the downgradient wells shall be subject to the additional sampling and analysis as described above. If any volatile organic compounds are detected by method 6230D, or the equivalent method 502.2, then EPA methods 604 and 611 must also be run to detect other organic compounds which may be present. The results of all analysis specified in the monitoring requirements, including 604 and 611 if required, must be submitted simultaneously The results of the sampling and analysis shall be sent to the N.C. Division of Environmental Management on Form GW-59 (Compliance Monitoring Report Form) every April, August, and December. 12 3. The Compliance Boundary delineated on the attached site plan for the disposal system is specified by regulations in 15 NCAC 2L, Classifications and Water Quality Standards applicable to the groundwater of North Carolina. An exceedance of Groundwater Quality Standards beyond the Compliance Boundary is subject to penalty provisions applicable under General Statute 143-215.6A. The sale of property, by the Permittee, which is within or contiguous to the disposal system site may alter location of the Compliance Boundary. For facilities permitted on or after December 30, 1983, the Compliance Boundary is established at the lesser of 250 feet from the perimeter of the waste disposal field or 50 feet within the property boundary. If the title to any property which may affect the location of the Compliance Boundary is changed, the permittee shall notify the Division Director within 14 days. The Director shall then establish a modified Compliance Boundary which will be done as a modification to the Permit. The REVIEW BOUNDARY delineated on the attached site map for the disposal system is specified by regulations in 15 NCAC 2L, Groundwater Classifications and Standards. A REVIEW BOUNDARY is established around disposal systems midway between the Compliance Boundary and the perimeter of the waste disposal area. When the concentration of any substance equals or exceeds the maximum allowable concentration of that substance at the REVIEW BOUNDARY, as determined by monitoring, the permittee shall either (i) demonstrate, through predictive calculations or modeling, that natural site conditions, facility design and operational controls will prevent a violation of standards at the Compliance Boundary; or, (ii) submit a plan for the alteration of existing site conditions, facility design or operational controls that will prevent a violation of standards at the Compliance Boundary, and implement that plan upon its approval by the Director. 4. Any additional groundwater quality monitoring as deemed necessary by the Division of Environmental Management shall be provided. V . INSPECTIONS 1. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to insure proper operation of the subject facilities. 2. The Permittee or his designee shall inspect the wastewater treatment and disposal facilities to prevent malfunctions and deterioration, operator errors and discharges which may cause or lead to the release of wastes to the environment, a threat to human health, or a nuisance. The Perrriittee shall keep an inspection log or summary including at least the date and time of inspection, observations made, and any maintenance, repairs, or corrective actions taken by the Permittee. This log of inspections shall be maintained by the Permittee for a period of three years from the date of the inspection and shall be made available upon request to the Division of Environmental Management or other permitting authority. 3. Any duly authorized officer, employee, or representative of the Division of Environmental Management may, upon presentation of credentials, enter and inspect any property, premises or place on or related to the disposal site and facility at any reasonable time for the purpose of determining compliance with this permit; may inspect or copy any records that must be kept under the terms and conditions of this permit; or may obtain samples of groundwater, surface water, or leachate. 5 This permit shall become voidable unless the facilities are constructed in accordance with the conditions of this permit, the approved plans, specifications and other supporting data. 2. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 3. This permit is not transferable. In the event there is desire for the facilities to change ownership, or there is a name change of the Permittee, a formal permit request must be submitted to the Division of Environmental Management accompanied by an application fee, documentation from the parties involved, and other supporting materials as may be appropriate. The approval of this request will be considered on its merits and may or may not be approved. 4. A set of approved plans and specifications for the subject project must be retained by the applicant for the life of the project. 5. The annual administering and compliance fee must be paid by the Permittee within thirty (30) days after being billed by the Division. Failure to pay the fee accordingly may cause the Division to initiate action to revoke this permit as specified by 15 NCAC 2H .0205 (c)(4). 6. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to an enforcement action by the Division of Environmental Management in accordance with North Carolina General Statute 143-215.6A to 143-215.6C. 7. The subject wastewater treatment and disposal facilities shall be connected to an operational publicly owned wastewater collection system within 180 days of its availability to the subject facilities, if the subject wastewater treatment or disposal facilities are in noncompliance with the terms and conditions of this non -discharge permit or the governing statutes or regulations. Prior to the initiation of these connection activities, appropriate approval must be received from this Division. S. The issuance of this permit does not preclude the Permitwe from complying with any and all statutes, rules, regulations, or ordinances which may be imposed by other government agencies (local, state, and federal) which have jurisdiction. 9. The Permittee, at least six (6) months prior to the expiration of this permit, shall request its extension. Upon receipt of the request, the Commission will review the adequacy of the facilities described therein, and if warranted, will extend the permit for such period of time and under such conditions and limitations as it may deem appropriate. 10. The first settling tanks through which wastewater is passed in a treatment works for the purpose of removing settleable and suspended solids and BOD which is associated with the solids:. (35)Pumps. All influent, effluent, and in -plan) pumps; (36)Radialion. Disinfection and/or sterilization process ulitizing devices emitting ultraviolet or gamma rays; (37)Reverse Osmosis. A treatment process in which a heavy contaminated liquid is pressurized through a membrane forming nearly pure liquid free from suspended solids; (38)Rolaling Biological Conlaclors. A fixed biological growth process in which wastewater flows through tanks in which a series of partially submerged circular surfaces are rotated; (39)Sand Fillers:(a)lntermitlent Biological. Filtration of effluent following septic tanks, lagoons, or some other treatment process in which further biodecomposilion is expected to produce desired effluents; Hydraulic loading rates on these filters are computed in gpd/ac and have a resulting low gpmisf (loss than one); (b)Recirculating biological - the same type of sand filter as t;ned in (a) with the added capability to recycle effluent back through the sand filter; (40)Sand or Mixed -Media Fillers. A polishing process by which effluent limits are achieved through a further reduction of suspended solids:(a)low rate -• gravity, hydraulically loaded filter with loading rates in the one to three gp,r/sf range;(b)high rate -- a press hydraulically loaded filter with loading rates in the five gpmisf range; At any rate, the loading rate will exceed three gpmisf; (41)Secondary Clarifiers. A lank which follows the biological unit of a treatment plant and which has the purpose of removing sludge associated with the biological treatment units; (42)Separate Sludge Reaeration. A part o€ the contact stabilization process where the activated sludge is translerred to a lank and aerated before returning it to the basin; (43)Septic Tank. A single-slory settling tank in which settled sludge is in contact with the wastewater flowing through the lank; shall not be applicable for septic tank systems serving single family residences having a capacity of 2000 gallons or less which discharge to a nitrification field; t44)Sfudge Digestion. The process by which organic or volatile matter and sludge is gasified, liquefied, mineralized or converted into more stable organic mailer through the activity of living organisms, which includes aerated holding lanks; (45)Sludge Drying Beds. An area comprising natural or artificial layers of porous materials upon which digested sewage sludge is dried by drainage and evaporation; (46)Sludge Eluirialion. A process of sludge conditioning in which certain constituents are removed by successive washings with fresh water or plant effluent; (47)Sludge Gas utilization. The process of using sewage gas for the purpose of heating buildings, driving engines, etc.; (48)Sludge Holding lank (Aerated and hlonaeraled}. A tank utilized for small wastewater treatment plants not containing a digester in which sludge may be kept fresh, and supernatant withdrawn prior to a drying method (i.e. sludge drying beds); This may be done by adding a small amount of air simply to keep the sludge fresh, but not necessarily an amount that would be required to achieve stabilization of organic matter. A nonaerated tank would simply be used to decant sludge prior to dewatering and would simply be used to decant sludge prior to dewalering and would not allow long periods (several days of detention) without resulting odor problems; (49)Siudge Incinerators. A furnace designed to burn sludge and to remove all moisture and combustible materials and reduce the sludge to a sterile ash; (50)Studge Stabilization (Chemical or Thermal). A process to make treated sludge less odorous and putrescrible, and 10 reduce the pathogenic organism content; This may be done by pH adjustment chlorine dosing, or by heal treatment; (51)Sludge Thickener. A type of sedimentation tank in which the sludge is permitted to settle and thicken through agitation and gravity; (52)Stabilizalion Lagoon. A type of oxidation lagoon in which biological oxidation of organic matter is effected by natural transfer of oxygen to the water from air (not a polishing pond); (53)Stand-By Power Supply. On site or portable electrical generating equipment; (54) Static Screens. A stationary screen designed to remove solids including non -biodegradable particulate (floalable solids, suspended solids and BOD reduction) from municipal and industrial wastewater treatment systems; (55)Tertiary Treatment. A stage of treatment following secondary which is primarily for the purpose of effluent polishing; A settling lagoon or sand or coal filler might be employed for this purpose. (56)Thermal Pollution Control Device. A device providing for the transfer of heal from a fluid flowing in tubes to another fluid outside the tubes, or vice versa; or, other means of regulating liquid temperatures; (57)Thermal Sludge Conditioner. A conditioning process by which heat is added for a protracted period of time to improve the dewalerabifity of sludge by The solubilizing and hydraufizing of the smaller and more highly hydrated sludge particles; (58)Toxic Materials. Those wastes or combinations of wastes including disease -causing agents which afler discharge and upon exposure, ingestion, inhalation or assimilation into any organism either directly Irom the environment or indirectly by ingestion through food chains, wilf cause death, disease, behavioral abnorrnafilies, cancer, genetic mutations, physiological malfunctions (including malfunctions in reproduction) or physical deformations, in such organisms or their offspring; Toxic materials include, by way illustration and not limilakon: lead, cadmium, chromium, mercury, vanadium, arsenic, zinc, orlho-nitro-chlorebenzene (ONCE polychlorinated biphenyls (PCBs) and dichlorodiphenyl trichloroelhane (DDT); and any other materials that have or may hereafter be determined to have toxic proporties; (59)Trickling filler. A biological treatment unit consisting of material such as broken stone or rock over which wastewater is distribuled; A high rate Irickling filter is one which is operated at between 10 and 30 mgd per acre. A low rate Irickling filler is one which is designed to operate at one to four mgd per acre; (60)Trickling Filter (Packed Tower). A plug flow type of operation in which wastewater flows down through successive layers of media or fillrate material, Organic material is removed continually by active biological fixed growth in each successive layer. This method may produce 'secondary' quality effluent, or may be adapted to produce a nilrilied effluent; (61)Vacuum Filters, Centrifuges, or Filler Presses. Devices which are designed to remove excess water from either digested or undigested sludge prior to disposal or further treatment. A physicallchemical method for reducing soluble organic material from wastewater effluent; the column -type beds used in this method will have a flow rate varying from Iwo to eight gallons per minute per square foot and may'be either upflow or downllow carbon beds, Carbon may or may not be regge.nerated on the wastewater treatment plant site. (2)Aerated Lagoons, A basin in which -all solids are maintained in suspension and by which biological oxidation or, organic mailer is reduced through artificially accelerated transfer of oxygen on a flow -through basis; (3)Aeration. A process of bringing about intimate contact between air or high purity oxygen in a liquid by spraying, agitalion diffusion; (4)Agriculluraily managed site. Any site on which a crop is produced, managed, and harvested (Crop includes grasses, grains, trees, etc); (5)Air Stripping. A process by which the ammonium ion is first converted to dissolved ammonia (pH adjustinenl) with the ammonia then released to the atmosphere by physical means; or other similar processes which remove petroleum products such as benzene, loluene, and xylene. (6)Carbon Regeneration. The regeneration of exhausted carbon by the use of a furnace to provide extremely high temperatures which volatilize and oxidize the absorbed impurities; (7)Carbonaceous Stage. A stage of wastewater treatment designed to achieve 'secondary' effluent limits. (8)Conlrifuge. A mechanical device in which centrifugal force is used to separate solids from liquids and/or to separate liquids of different dgnsities; (9)Chemical Addition Systems - The addition of chemical (s) to wastewater at an application point for purposes of improving solids removal, pH adjustment, alkalinity control, etc.; the capability to experiment with different chemicals and different apptication points to achieve a specific result will be considered one system; the capability to add chemical (s) to dual units will be rated as one system; capability to add a chemical at difference application points for different purposes will result in the systems being rate as separate system. (10)Chemltal Sludge Conditioning. The addition of a chemical compound such as lime, ferric chloride, or a polymer to wet sludge to coalesce the mass prior to fits application to a dewalering device; (II)Closed Cycle Systems. A separate stage of wastewater treatment designed for the specific purpose of converting ammonia nitrogen to nitrate nitrogen; (26)Phosphale Removal Biological. The removal of phosphorus from wastewater by an oxic/anoxic process designed to enhance luxury uptake of phosphorus by the microorganisms; (27)Polishing Pond. A holding pond following secondary treatment with sufficienl detention 10 allow for the settling of finely suspended solids; (28)Posi Aeration. Aeration following conventional secondary treatment units to increase effluent D.O. or for any other purpose; (29)Post Aeration. (Cascade) A polishing method by which dissolved oxygen is added to the effluent by a nonmechanicatr gravity means of flowing down a series of steps or weirs; The flow occurring across the steps or weirs moves in a fairly thin layer and the operation of the cascade requires no operator adjustment; thus, zero points are assigned even though this is an essential step to meeting the limits of the discharge permit; (30)Powdered or Granular Activated Carbon Feed.
