In accordance with a written request received on April 30, 2008, we are forwarding herewith the corrected
page 1 of Attachment A in Permit No. WQ0005849, issued August 31, 2007, for the subject facility.
The following corrections have been made to the permit:
1. The measurement frequency for Total Residual Chlorine (PCS Code: 50060) and pH (PCS Code:
00400) has for PPI 001 WWTF Effluent — Pre Construction (Sampling regimen prior to completion
of the permitted modifications) has been changed from "Daily — 5 x Week" to "Once per discharge."
Please replace Page 1 of Attachment A with the corrected Page i into Permit No. WQ0005849,
issued August 31, 2007.

corrected page 1 of Attachment A in Permit No. WQ0005849, issued August 31, 2007, for the subject
facility.
The following corrections have been made to the permit:
The effluent limits in PPI 001 WWTF Effluent — Pre Construction (Sampling regimen prior to
completion of the permitted modifications) for BOD$, Fecal Coliforrrl, Ammonia (NH3), pH and
Total Suspended Solids have been removed. However, the sampling schedule remains the same.
Please replace Page I of Attachment A with the corrected Page I. into Permit No. WQ0005849,
issued August 31, 2007.
