Dear Mr. Potter:
This is in reference to your application received June 27, 1996 for the continued operation of the
subject land application of residuals permit.
In a letter dated July- 10, 1996 (copy enclosed), you were advised to provide additional technical
information within thirty (30) days to make the application package complete or your application package
would be returned as incomplete. This thirty (30) day requirement elapsed on August 9, 1996. The
Division of Water Quality must receive this information by December 16, 1996 or your request for permit
renewal will be denied.
If the Division denies your request for a ,permit, you are advised that the operation of the subject
land application of residuals program without a valid permit is a violation of North Carolina General
Statute 143-215.1 and moy subject George W. Potter Septic Tank Service to appropriate enforcement
actions in accordance with north Carolina General Statute 143-215.6. Civil penalties of up to $10,000 per
day per violation may be assessed for failure to secure a permit required by North Carolina General Statute
143-215.1,
