Dear Mr. Potter:
The Permits and Engineering Unit has completed a preliminary review of the subject application.
The following items must be addressed before we can complete our review:
1. Please sign the last page of the application under the Applicant's Certification section and
submit to the Di ision.
2. In accordance with Permit Condition 1I12 (1) of your existing permit, please submit the
cumulative totals'of die following pollutants which have been land applied:
Cadmi um Copper
Lead Nickel
Zinc
3. Please suhmA an updated (and recently signed) land owner agreement.
4. Please submit the latest residual anaylsis and TCLP anaylsis for each residual source. All that
was submitted with the application package was the TCLP anaylsis for Corning. Inc.
5, In accordance with the 40 CFR Part 503 Regulations, each of your domestic residual sources
will be required to meet one of the vector attraction reduction alternatives. Please indicate
which of the vector attraction reduction alternatives your domestic sources will meet.
6. Are: records available for each of your residual sources verifying that the time and temperature
requirements for aerobic digestion (to meet pathogen reduction)'? If so, please submit the
docunnentation. Also, what percentage of the rs duals from Corning, Inc. consist of domestic
wastewater?
7. In accordance with Permit Condition. 1113 of your exisiting permit, please submit a copy of the
annual soils anaylsis including the required five heavy metals (cadimurn, copper. lead, nickel,

submit four (4) copies of all information to my attention at the address below. Also,
please note that failure to provide this additional information on or before August 9, 1996 will subject your
application to being returned as incomplete, in accordance with 15A NCAC 2H .0208.
