Dear Mr. McAtee:
We are forwarding herewith a corrected Permit No. WQ0005458, issued May 9, 2002, for the subject facility.
The following corrections have been made to the permit:
1. The permit was incorrectly issued as a 503 regulated program. The permit has been modified to
indicate the residuals land application program is 503 exempt.
2. Condition I.7. addressing pathogen and vector requirements has been removed.
3. Condition IIl. 4. addressing monitoring of pathogen and vector requirements has been removed.
4. Condition IV. 2. has been modified to include the previous monitoring well nomenclature as well
as new monitoring well nomenclature.
Please replace Permit No. WQ0005458 with the corrected, Permit No. WQ0005458 issued May 9, 2002.
