Cobey, Jr., Secretary SUBJECT: Permit No. WQ0005425 Longbranch Elementary School Robeson County T� N ;v In accordance with the permit application filed by the Public Schools of Robeson County and received in this office on August 5, 1991, we are forwarding herewith Permit No. WQ0005425. This permit, issued to the Public Schools of Robeson County for Longbranch Elementary School, is for the continued operation of an existing septic tank -nitrification field system at that school. This permit shall be effective from the date of issuance until rescinded and shall be subject to the conditions and limitations therein. In any parts, requirements, or limitations contained in this permit are unacceptable to you, you have the right to request an adjudicatory hearing upon written request. This request must be in the f arm of a written petition, conforming to Chapter 150B of North Carolina General Statutes, and filed with the Office of Administrative Hearing, P.O. Drawer 11666, Raleigh, NC 27604. Unless such demands are made, this permit shall be final and binding. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to insure proper operation of the subject facility. 7. No traffic or equipment shall be allowed on the disposal area except while normal maintenance is being performed. 8. The facility shall be effectively maintained and operated as a nondischarge system to prevent the discharge of any wastewater resulting from the operation of this facility. 9. The issuance of this Permit shall not relieve the Permittee of the responsibility for damages to surface or groundwaters resulting from the operation of this facility. 10. Diversion or bypassing of the untreated wastewater from the treatment facility is prohibited. 11. Monitoring wells shall be constructed and sampling schedules arranged as required by the Division of Environmental Management and in accordance with 15 NCAC 2C .0100. 12. Any monitoring deemed necessary by the Division of Environmental Management to insure surface and groundwater protection will be established and an acceptable sampling reporting schedule shall be followed. 13. In event the facility fails to perform satisfactorily, including the creation of nuisance conditions, the Permittee shall take such immediate corrective action as may be required by this Division, including the construction of additional or replacement wastewater treatment or a disposal facility. 14. This Permit shall be voidable in the event of failure of the soil to adequately absorb the wastes and may be rescinded unless the facility is installed, maintained, and operated in a manner which will protect the assigned water quality standards of the surface waters and groundwaters. 15. All surface drainage on this site shall be diverted away from any portion of the nitrification field or designated repair area. Permit issued this 19th day of August, 1991.
