Dear Mr. Dotson:
Before the Division can rescind the above subject permit, as requested in your
November 16, 1993 letter, the following information is needed: verification of proper
waste disposal, and applicable Permit Numbers for all wastes generated on site.
The Wilmington Regional Office conducted a close-out inspection of the facilities on
December 9, 1993 and found although the Washerette has ceased operation, there are
occupied buildings on site generating waste. Records at Onslow County indicate that no
one has requested a permit for any subsurface disposal system on site.
I am requesting that Mr. Howard make contact with Dave Adkins, of our Wilmington
Regional Office at 910/946-6481 to discuss the current disposal methods being used on
site. Enforcement activities may be in order if Mr. Howard can not verify the disposal
systems in use.
After verification by the Wilmington Regional Office that the permit is no longer
needed, State Permit No. WQ0005251 will be rescinded.

SUBJECT: Howard's Washerette Rescission Request

Onslow County
This is in response to your letter requesting confirmation that the
above mentioned permit is no longer required. A site visit was
performed on December 9, 1993, with a member from the Onslow County
Health Department. The following items were noted:
1. The plant was no longer receiving waste and had been pumped
out.
2. The infiltration lagoon had been eliminated.
3. The county has not received request for subsurface disposal.
4. The investigation revealed a basin with a submersible pump that
was pumping to an undetermined site.
It is the recommendation of this Office that the permit be
rescinded ONLY AFTER receiving notification from Mr. Howard of his
-disposal methods. His notification should include permit numbers
for all waste generated, indicating properly permitted disposal.
Even though the Washerette itself has ceased operation, there are
occupied buildings that generate waste.
