Wastewater Treatment, Reuse, and High Rate Infiltration Disposal System Currituck County In accordance with the amendment request received July 22, 1997, we are forwarding herewith Permit No. WQ0004823, dated November 12, 1997, to the Pine Island-Currituck Club, L.L.C. for the construction and operation of the subject wastewater treatment, reuse (spray irrigation), and high rate infiltration facilities. This permit is being amended to combine two separate permits, WQ0004823 and WQ0011198, into one permit. In addition Phase lI of the WWTP has been increased to 375,000 GPD. This permit shall be effective from the date of issuance until August 31, 2000, shall void Permit No. WQ0004823 issued February 26, 1997, shall void Permit No. WQ0011198 issued September 5, 1995, and shall be subject to the conditions and limitations as specified therein. Please pay particular attention to the monitoring requirements in this permit. Failure to establish an adequate system for collecting and maintaining the required operational information will result in future compliance problems. If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right to request an adjudicatory hearing upon written request within thirty (30) days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings, P.O. Currituck County construction and operation of a wastewater collection system, a 600,000 GPD wastewater treatment system (to be built in two phases), a 500,000 GPD reclaimed water spray irrigation system, and a 180,000 GPD high rate infiltration system. The wastewater collection system shall consist of the construction and operation of approximately 13,144 linear feet of 8-inch gravity sewer, approximately 8,409 linear feet of 6-inch force main which will be a common force main for the following pump stations; a 455 GPM pump station with duplex pumps, high water alarms, autodialer, portable generator receptacle, and approximately 37 linear feet of 6-inch force main, an 85 GPM pump station with duplex pumps, high water alarms, autodialer, portable generator receptacle, and approximately 768 linear feet of 6-inch force main, a 490 GPM pump station with duplex pumps, high water alarms, autodialer, portable generator receptacle, and approximately 84 Iinear feet of 6- inch force main, and a 530 GPM pump station with duplex pumps, high water alarms, autodialer, portable generator receptacle, and approximately 47 linear feet of 6-inch force main to serve one hundred eighty- one (181) 4-bedroom single family lots and forty-four (44) 3-bedroom patio homes. An additional wastewater collection system shall be constructed which will consist of a 315 GPM pump station to serve the reverse osmosis water treatment plant with dual pumps, high water alarms, a 30,338 gallon storage tank, and approximately 700 feet of 4-inch force main to the irrigation pond and continued operation of an existing 200 feet of 4-inch force main to the existing high rate disposal system permitted under WQ0004823, a 620 GPM brine pump station with dual pumps, high water alarms and approximately 708 linear feet of 6-inch force main to the five-day holding pond, and a 23.5 GPM pump station to serve the domestic waste at the water treatment plant with dual pumps, high-water alarms, and approximately 571 linear feet of 2-inch force main to the existing wastewater treatment plant. The wastewater treatment facility will total 600,000 GPD of wastewater treatment capacity and will be built in two phases. Phase I is existing and consists of the following: Phase I A dual path 225,000 GPD wastewater treatment plant with a bar screen, a flow splitter box, a 56,356 gallon aerated flow equalization basin, a 112,716 gallon aeration tank, a 112,500 gallon aeration tank divided into three (3) 37,572 gallon compartments, dual 32,628 gallon clarifiers, a 33,800 gallon aerated sludge holding tank, a traveling bridge tertiary filter with a surface area of 125 ft2, a recording flow meter with totalizer, ultraviolet disinfection, dual 3,967 gallon effluent chambers with dual 350 GPM pumps, and an automatically activated emergency generator which is sized to operate Phase I of the facility or half of the entire plant. Phase II shall consist of the construction and operation of an additional 375,000 wastewater treatment facility containing: Phase II A 375,000 GPD wastewater treatment plant with a 93,925 gallon flow equalization basin with dual 300 GPM pumps, a manually cleaned bar screen, a flow splitter box, three (3) 62,500 gallon aeration basins, a 187,500 gallon aeration basin, two (2) 64,000 gallon clarifiers, a 58,582 gallon aerated sludge holding tank, a 12,800 gallon sludge holding tank, three (3) 1,260 SCFM centrifugal blowers, two (2) 790 SCFM positive displacement blowers, two (2) 370 SCFM positive displacement blowers, one (1) 200 SCFM positive displacement blower, a traveling bridge tertiary filter with a total surface area of 313 ft2, ultraviolet disinfection (Phase I to be expanded), an automatically activated emergency generator, and a continuous recording turbidimeter. The 180,000 GPD high rate infiltration system consists of an infiltration pond with a surface area of 17,130 ft2 and eight (8) spray beds with a total surface area of 67,427 ft2. The 500,000 GPD spray irrigation system shall consist of the construction and operation of a 2.8 MG five-day detention pond, a 410 GPM pump station with dual pumps, high water alarms, approximately 4,511 linear feet of 6=inch force main (to the irrigation pond), and approximately 700 linear feet of 4-inch force main (back to the WWTP), a 4.7 MG storage pond, an existing portable generator, and sixty-six (66) acres of golf course (Holes 5, 6, 7, 8, 9, 10, 12, 13, 14, 15, 16,.17, 18, and the practice range). The above described facilities shall serve the Pine Island Development and Currituck Club with no discharge of wastes to the surface waters, pursuant to the amendment request received July 22, 1997, and in conformity with the project plan, specifications, and other supporting data subsequently filed and approved by the Department of Environment, Health and Natural Resources and considered a part of this permit. This permit shall be effective from the date of issuance until August 31, 2000, shall void Permit No. WQ0004823 issued February 26, 1997, shall void Permit No. WQ0011198 issued September 5, 1995, and shall be subject to the following specified conditions and limitations: I. PERFORMANCE STANDARDS Upon completion of construction and prior to operation of this permitted facility, a certification must be received from a professional engineer certifying that the permitted facility has been installed in accordance with this permit, the approved plans and specifications, and other supporting materials. Mail the Certification to the Non -Discharge Permitting Unit, P.O. Box 29535, Raleigh, NC 27626-0535. 2. The Washington Regional Office, telephone number (919) 946-6481, shall be notified at least forty-eight (48) hours in advance of operation of the installed facilities so that an in - place inspection can be made. Such notification to the regional supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays. The spray irrigation facilities and high rate infiltration system shall be effectively maintained and operated at all times so that there is no discharge to the surface waters, nor any contamination of ground waters which will render them unsatisfactory for normal use. In the event that the facilities fail to perform satisfactorily, including the creation of nuisance conditions or failure of the irrigation area to adequately assimilate the wastewater, the Permittee shall take immediate corrective actions including those actions that may be required by the Division of Water Quality (Division), such as the construction of additional or replacement wastewater treatment and disposal facilities. 2 4. The issuance of this permit shall not relieve the Permittee of the responsibility for damages to surface or groundwaters resulting from the operation of this facility. 5. The residuals and screenings generated from these treatment facilities must be disposed in accordance with General Statute 143-215.1 and in a manner approved by the Division. 6. Diversion or bypassing of the untreated wastewater from the treatment facilities is prohibited. 7. The following buffers shall be maintained for the reclaimed water (spray irrigation) system: a) 100 feet between wetted area and water supply wells, b) 100 feet between wetted area and waters classified as SA, c) 25 feet between wetted area and surface waters not classified as SA, d) 50 feet between wetted area and non -isolated, wetter, wetlands (Type II), e) 25 feet between wetted area and isolated, drier, wetlands (Type I), f) 100 feet between wastewater treatment units and wells, and g) 50 feet between wastewater treatment units and property lines.* * An easement may be needed from DOT in order to obtain this buffer between the five- day holding pond and the exterior property line. 8. The cationic backwash water, anionic backwash water, and supernatant from the iron filter backwash at the Pine Island Water Treatment Plant will be pumped (Permit No. WQ0013006) to the high rate infiltration system and shall be combined with wastewater effluent to a level that will not exceed groundwater standards. II. OPERATION AND MAINTENANCE REQUIREMENTS. 1. The facilities shall be properly maintained and operated at all times. 2. Upon classification of the facility by the Water Pollution Control System Operators Certification Commission (WPCSOCC), the Permittee shall employ a certified wastewater treatment plant operator to be in responsible charge (ORC) of the wastewater treatment facilities. The operator must hold a certificate of the type and grade at least equivalent to or greater than the classification assigned to the wastewater treatment facilities by the WPCSOCC. The Permittee must also employ a certified back-up operator of the appropriate type and grade to comply with the conditions of 15A NCAC 8A .0202. The ORC of the facility must visit each Class I facility at least weekly and each Class II, III, and IV facility at least daily, excluding weekends and holidays, and must properly manage and document daily operation and maintenance of the facility and must comply with all other conditions of 15A NCAC 8A .0202. Once the facility is classified, the Permittee must submit a letter to the WPCSOCC which designates the operator in responsible charge within thirty days after the wastewater treatment facilities are 50% complete. 3. The facilities shall be effectively maintained and operated as a non -discharge system to prevent the discharge of any wastewater resulting from the operation of this facility. 4. No type of wastewater other than that from the Pine Island Development and Currituck Club shall be sprayed onto the irrigation area or disposed into the high rate infiltration system. In addition, 50,000 GPD of RO plant backwash water will be placed onto the golf course spray site and 26,800 GPD of cationic exchange backwash water will be disposed in the high rate infiltration system. 5. An automatically activated standby power source shall be on site and operational at all times. If a generator is employed as an alternate power supply, it shall be tested weekly by interrupting the primary power source. 6. The flow measurement device and turbidimeter shall be calibrated annually by a representative of a firm which is routinely engaged in the calibration of flow measurement devices and turbidimeters. Records of this calibration, including all information pertinent to the calibration, shall be available during any inspection by Division staff. 7. Diffusers shall be cleaned as needed to ensure that adequate aeration is provided. Records of this maintenance shall be maintained by the permittee. 8. The Permittee shall provide for the pump station and force main the following items: a. Pump on/off elevations located so that 2-8 pumping cycles may be achieved per hour in the pump station, b. An air relief valve located at all high points along the force main, c. A screened vent for the wet well, d. Fillets located in the wet well at the intersection of the flooring and sidewalls, e. Three feet of cover (minimum) over the force main or the use of ferrous material where three feet cannot be maintained, f. Sufficient devices which will protect the pump station from vandals, and g . Flood protection if the pump station is located below the 100-year flood elevation. CONDITIONS SPECIFIC TO THE HIGH RATE INFILTRATION SYSTEM: 9. The application rates for the high rate infiltration system shall not exceed 7.0 GPD/ft2. 10. No traffic or equipment shall be allowed on the disposal area except while installation occurs or while normal maintenance is being performed. 11. The spray beds shall be raked twice weekly during the months April through October and once a week during the months of November through March. 12. Vegetative growth must be kept out of the spray beds at all times. All vegetation must be removed manually so that minimal disturbance will occur to the disposal area. 13. The application of chemicals to the high rate infiltration spray beds is expressly prohibited. CONDITIONS SPECIFIC TO THE RECLAIMED WATERSSPRAY IRRIGATION) SYSTEM: 14. A suitable vegetative cover shall be maintained on the spray irrigation areas. 15. irrigation shall not be performed during inclement weather or when the ground is in a condition that will cause runoff. 16. Adequate measures shall be taken to prevent wastewater runoff from the spray field. 17. The application rate shall not exceed a cumulative loading of 101.4 inches over any twelve (12) month period at an instantaneous application rate not to exceed 0.4 inches per hour. 18. Public access to the irrigation sites shall be controlled during active site use. Such controls may include the posting of signs showing the activities being conducted at each site. A sign shall be posted in plain sight in the club house showing these activities. 4 19. All wastewater shall be routed to the five-day holding pond or the high rate infiltration system should the limit for fecal coliform (daily maximum concentration of 25 per 100 ml) or turbidity (instantaneous maximum of 10 NTU) be exceeded, until such time that the problems associated with the treatment capability of the wastewater treatment plant have been corrected. The wastewater in the five-day holding pond shall be pumped back to the treatment plant for retreatment or treated in the five-day pond prior to discharge to the storage pond. MONITORING AND REPORTING REQUIREMENTS 1. Any monitoring (including groundwater, surface water, soil or plant tissue analyses) deemed necessary by the Division to insure surface and ground water protection will be established and an acceptable sampling reporting schedule shall be followed. 2. During the remainder of the year, these parameters shall be monitored monthly. ** Triannually sampling shall be conducted during March, July, and November. If Groundwater sampling indicates or predicts problems with the compliance with Groundwater Standards, this permit will be modified to include additional and/or more restrictive limitations. 3. Adequate records shall be maintained by the Permittee tracking the amount of reclaimed water utilized at the golf course. These records shall include, but are not necessarily limited to the following information: a. date and time of irrigation, b. volume of wastewater irrigated, c. zone irrigated, d . length of time zone is irrigated, e. continuous weekly, monthly, and year-to-date hydraulic (inches/acre) Ioadings for each zone, f. weather conditions, and g . maintenance of cover crops. 4. Three (3) copies of all operation and disposal records (as specified in condition lII 3) on Form NDAR-1 shall be submitted on or before the last day of the following month. Three (3) copies of all effluent monitoring data (as specified in condition 1112) on Form NDMR-1 shall be submitted on or before the last day of April, August, and December. A record shall be maintained of all residuals removed from this facility. This record shall include the name of the hauler, permit authorizing the disposal or a letter from a municipality agreeing to accept the residuals, date the residuals were hauled, and volume of residuals removed. 0 6. A maintenance log shall be maintained at this facility including but not limited to the following items: a. Daily sampling results including residual chlorine, settleable matter, and dissolved oxygen in the aeration basin and at the clarifier weir. b. Visual observations of the plant and plant site. c. Record of preventative maintenance (changing of filters, adjusting belt tensions, alarm testing, diffuser inspections and cleanings, etc.). d. Date of calibration of flow measurement device and turbidimeter. e. Date and results of power interruption testing on alternate power supply. 7. Noncompliance Notification: The Permittee shall report by telephone to the Washington Regional Office, telephone number (919) 946-6481, as soon as possible, but in no case more than 24 hours or on the next working day following the occurrence or first knowledge of the occurrence of any of the following: a. Any occurrence at the wastewater treatment facility which results in the treatment of significant amounts of wastes which are abnormal in quantity or characteristic, such as the dumping of the contents of a sludge digester; the known passage of a slug of hazardous substance through the facility; or any other unusual circumstances. b. Any process unit failure, due to known or unknown reasons, that render the facility incapable of adequate wastewater treatment such as mechanical or electrical failures of pumps, aerators, compressors, etc. c. Any failure of a pumping station, sewer line, or treatment facility resulting in a by-pass directly to receiving waters without treatment of all or any portion of the influent to such station or facility. d. Any time that self -monitoring information indicates that the facility has gone out of compliance with its permit limitations. Persons reporting such occurrences by telephone shall also file a written report in letter form within five (5) days following first knowledge of the occurrence. This report must outline the actions taken or proposed to be taken to ensure that the problem does not recur. IV. GROUNDWATER REQUIREMENTS Within sixty (60) days of permit issuance, the four (4) existing monitor wells, MW-3, MW-13, MW-14, and MW-15 at the Pine Island Development shall be abandoned in accordance with 15A NCAC 2C .0113 (a)(2). A well abandonment record (Form GW-30) for each well shall be submitted to the Washington Regional Office within thirty (30) days of well abandonment. 2. Within sixty (60) days of permit issuance, the permittee shall submit two original copies of a scaled topographic map (scale no greater than 1".100') signed and sealed by a professional engineer or a state licensed land surveyor that indicates all of the following information: a. the location and identity of each monitoring well, b. the location of the waste disposal system, c, the location of all property boundaries, (continued on next page) 7 d. the latitude and longitude of the established horizontal control monument, e. the relative elevation of the top of the well casing (which shall be known as the "measuring point"), and f. the depth of water below the measuring point at the time the measuring point is established. The survey shall be conducted using approved practices outlined in North Carolina General Statutes Chapter 89C and the North Carolina Administrative Code Title 21, Chapter 56. The surveyor shall establish a horizontal control monument on the property of the waste disposal system and determine the latitude and longitude of this horizontal control monument to a horizontal positional accuracy of +/- 10 feet. All other features listed in a. through e. above shall be surveyed relative to this horizontal control monument. The positional accuracy of features listed in a. through e. above shall have a ratio of precision not to exceed an error of closure of 1 foot per 10,000 feet of perimeter of the survey. Any features located by the radial method will be located from a minimum of two points. Horizontal control monument shall be installed in such a manner and made of such materials that the monument will not be destroyed due to activities that may take place on the property. The map shall also be surveyed using the North American Datum of 1983 coordinate system and shall indicate the datum on the map_ All bearings or azimuths shall be based on either the true or NAD 83 grid meridian. If a Global Positioning System (GPS) is used to determine the latitude and longitude of the horizontal control monument, a GPS receiver that has the capability to perform differential GPS shall be used and all data collected by the GPS receiver will be differentially corrected. The maps and any supporting documentation shall be sent to the Permits and Compliance Unit, Groundwater Section, P.O. Box 29578, Raleigh, NC, 27626-0578. 3. Monitor wells MW-1, MW-2, MW-4, MW-5, MW-16, and MW-17 (at the Pine Island Development - depicted on Attachment A) and monitor wells MW-1 (MW-A), MW-2 (MW-B), MW-3 (MW-C), MW-4 (MW-D) (at the Currituck Club - depicted on Attachment B shall be sampled initially after construction and thereafter every March, July, and November for the following parameters: Water Level pH - Chloride Total Dissolved Solids (TDS) Fecal Coliforms Total Organic Carbon (TOC) Nitrate (NO3) Total Ammonia Volatile Organic Compounds - In November only using one of the following: (A) Standard Method 6230D, PQL at 0.5 ug/L or less (B) Standard Method 621OD, PQL at 0.5 ug/L or less (C) EPA Method 8021, Low Concentration, PQL at 0.5 ug/L or less (D) EPA Method 82600, Low Concentration, PQL at 0.5 ug/L or less (E) Another method with prior approval by the Groundwater Section Chief Any method used must meet the following qualifications: (1) A laboratory must be DWQ certified to run any method used. (2) The method used must, at a minimum, include all the constituents listed in Table VIII of Standard Method 6230D. (3) The method used must provided a PQL at 0.5 ug/L or less which must be supported by laboratory proficiency studies as required by the DWQ Laboratory Certification Unit. Any constituents detected above the MDL but below the PQL at 0.5 ug/L must be qualified (estimated) and reported. If any volatile organic compounds are detected by the methods listed, then the Washington Regional Office Groundwater Supervisor, telephone number (919) 946-6481, must be contacted immediately for further instructions regarding any additional follow-up analyses required. The results of all initial and follow-up analyses must be submitted simultaneously. The measurement of water levels must be made prior to sampling for the remaining parameters. The depth to water in each well shall be measured from the surveyed point on the top of the casing. The measuring points (top of well casing) of all monitoring wells shall be surveyed to provide the relative elevation of the measuring point for each monitoring well. If TOC concentrations greater than 10 mg/1 are detected in any downgradient monitoring well, additional sampling and analysis must be conducted to identify the individual constituents comprising this TOC concentration_ If the TOC concentration as measured in the background monitor well exceeds 10 mg/l, this concentration will be taken to represent the naturally occurring TOC concentration. Any exceedances of this naturally occurring TOC concentration in the downgradient wells shall be subject to the additional sampling and analysis as described above. The results of the sampling and analysis must be received on Form GW-59 (Groundwater Quality Monitoring: Compliance Report Form) by the Groundwater Section, Permits and Compliance Unit, P.O. Box 29578 Raleigh, N.C. 27626-0578 on or before the last working day of the month following the sampling month. 4. Irrigation wells connected to and shared with the lines for reuse water will require a complete disconnect feature (air gap), as opposed to a check valve, to protect the irrigation wells. 5. The COMPLIANCE BOUNDARY and REVIEW BOUNDARY for the spray irrigation areas, shall be established at the property boundary in accordance with 15A NCAC 2H .0219 (k)(1)(C)(1)(III). An exceedance of Groundwater Quality Standards at or beyond the Compliance Boundary is subject to immediate remediation action in addition to the penalty provisions applicable under General Statute 143-215.6A(a)(1). 6. The COMPLIANCE BOUNDARY for the high rate disposal system is specified by regulations in 15A NCAC 2L, Groundwater Classifications and Standards. The Compliance Boundary is for the disposal system constructed after December 31, 1983 is established at either (1) 250 feet from the waste disposal area, or (2) 50 feet within the property boundary, whichever is closest to the waste disposal area. An exceedance of Groundwater Quality Standards at or beyond the Compliance Boundary is subject to immediate remediation action in addition to the penalty provisions applicable under General Statute 143-215.6A(a)(1). In accordance with 15A NCAC 2L, a REVIEW BOUNDARY is established around the disposal systems midway between the Compliance Boundary and the perimeter of the waste disposal area. Any exceedance of standards at the Review Boundary shall require remediation action on the part of the permittee. 7. Any additional groundwater quality monitoring, as deemed necessary by the Division, shall be provided. V . INSPECTIONS Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to insure proper operation of the subject facilities. 2. The Permittee or his designee shall inspect the wastewater treatment and disposal facilities to prevent malfunctions and deterioration, operator errors and discharges which may cause or lead to the release of wastes to the environment, a threat to human health, or a nuisance. The Permittee shall keep an inspection log or summary including at least the date and time of inspection, observations made, and any maintenance, repairs, or corrective actions taken by the Permittee. This log of inspections shall be maintained by the Permittee for a period of three years from the date of the inspection and shall be made available upon request to the Division or other permitting authority_ Any duly authorized officer, employee, or representative of the Division may, upon presentation of credentials, enter and inspect any property, premises or place on or related to the disposal site or facility at any reasonable time for the purpose of determining compliance with this permit; may inspect or copy any records that must be maintained under the terms and conditions of this permit, and may obtain samples of groundwater, surface water, or leachate. V I. GENERAL CONDITIONS l . This permit shall become voidable unless the facilities are constructed in accordance with the conditions of this permit, the approved plans and specifications, and other supporting data. 2. This permit is effective only with respect to the nature and volume of wastes described in the application and other supporting data. 3. This permit is not transferable. In the event there is a desire for the facilities to change ownership, or there is a name change of the Permittee, a formal permit request must be submitted to the Division accompanied by an application fee, documentation from the parties involved, and other supporting materials as may be appropriate. The approval of this request will be considered on its merits and may or may not be approved. 4. Failure to abide by the conditions and limitations contained in this permit may subject the Permittee to an enforcement action by the Division in accordance with North Carolina General Statute 143-215.6A to 143-215.6C. 5. The issuance of this permit does not preclude the Permittee from complying with any and all statutes, rules, regulations, or ordinances which may be imposed by other government agencies (local, state, and federal) which have jurisdiction. 6. A set of approved plans and specifications for the subject project must be retained by the Permittee for the life of the project. 7. The annual administering and compliance fee .must be paid by the Permittee within thirty days after being billed by the Division. Failure to pay the fee accordingly may cause the Division to initiate action to revoke this permit as specified by 15 NCAC 2H .0205 (c)(4). 8. The Permittee, at least six (6) months prior to the expiration of this permit, shall request its extension.
