In accordance with your permit renewal request received August 15, 2013, and draft permit
comments received December 19, 2013, we are forwarding herewith Permit No. WQ0004823 dated
December 23, 2013, to Pine Island-Currituck, LLC for the continued operation of the subject wastewater
treatment, high -rate infiltration and reclaimed water facilities. Please note that this renewed permit shall
become effective on March 1, 2014 (i.e. the day after the expiration date of the existing permit), which
differs from the date of this letter.
Modifications to the subject pen -nit are as follows:
Effluent monitoring frequency has been set to weekly for core effluent quality parameters
during the summer season and twice per month during all other months and is based on
actual flows instead of the permitted flow of 600,000 GPD- Other parameter frequencies
remain unchanged. Residual chlorine monitoring has been removed as UV disinfection is
employed;
Parameter WQO1, Reclaimed water flow distributed, has been added to track the amount
of reclaimed water sent to the reclaimed water storage pond. Also note that the arnount of
reclaimed water actually irrigated at the golf course must be tracked per Condition IV.6;
Groundwater monitoring is now required only at MW-1, MW-2, MW-4, and MW-5 once
(annually) and all other active monitoring wells shall be abandoned as per Condition 1. 1;
This permit shall be effective from March 1, 2014 until February 28, 2019 and shall be subject to
the conditions and limitations as specified therein- .Please pay particular attention to the monitoring
requirements now listed in Attachments A, B and C (instead of the permit text in previous permit),
Failure to establish an adequate, system for collecting and maintaining the required operational
information shall result in future compliance problems.

FOR THE
continued operation of a 600,000 gallon per day (GPD) dual train wastewater treatment, high -rate
infiltration and non -conjunctive reclaimed water facility consisting of:
One 225,000 GPD wastewater treatment plant with a bar screen, flow splitter box, 56,356 gallon aerated
flow equalization basin, I12,716 gallon aeration tank, 112,500 gallon aeration tank with three 37,572
gallon compartments, dual 32,628 gallon clarifiers, 33,800 gallon aerated sludge holding tank, and
traveling bridge tertiary filter with a surface area of 125 SF; and
One 375,000 GPD wastewater treatment plant with 93,925 gallon flow equalization basin with dual 300
GPM pumps, manually cleaned bar screen, flow splitter box, three 62,500 gallon aeration basins, 187,000
gallon aeration basin, two 64,000 gallon clarifiers, 58,582 gallon aerated sludge holding tank and
traveling bridge tertiary filter with a total surface area of 313 SF;
With each train sharing a recording flow meter with totalizer, ultraviolet disinfection, dual 3,967 gallon
effluent chambers with dual 350 GPM pumps, three 1,260 SCFM centrifugal blowers, two 790 SCFM
positive displacement blowers, one 200 SCFM positive displacement blower, continuous recording
turbidimeter, 12,800 gallon sludge holding tank, 10,000 gallon clearwell, and automatically activated
emergency generator; and
With disposal options including a 500,000 GPD reclaimed water irrigation system with 2.8 MG five-day
detention pond, 410 GPM pump station with dual pumps, high water alarins, approximately 4,511 linear
feet of 6-inch force main (to the reclaimed water irrigation pond), and approximately 700 linear feet of 4-
inch force main (back to the WWTP), a 4.7 MG reclaimed water storage pond, portable generator, and 66
acres of golf course (Holes 5, 6, 7, 8, 9, 10, 12, 13, 14, 15, 16, 17, 18, and practice range); and a 180,000
GPD high -rate infiltration system consisting of an infiltration pond with a surface area of 17,130 SF and
one spray bed with a surface area of 8,428 SF;
to serve Pine Island / Currituck Club, with no discharge of wastes to surface waters, pursuant to the
application received August 15, 2013 and in conformity with the project plans, specifications, and other
supporting data subsequently filed and approved by the Department of Environment and Natural
Resources and considered a part of this permit.

This permit shall be effective from March 1, 2014 until February 28, 2019 and shall be subject to the
following specified conditions and limitations:
L SCHEDULES
1. Within 60 days of the effective date of this permit, monitoring wells MW-16, MW-17, CC MW-1,
CC MW-2, CC MW-3, and CC MW-4 shall be permanently abandoned. Within 30 days of
abandonment, a Well Abandonment Record (Form GW-30) listing this permit number and the
appropriate monitoring well identification number shall be completed for each well abandoned and
mailed to the Division of Water Resources, Water Quality Permitting Section, 1636 Mail Service
Center, Raleigh, NC 27699-1636. A North Carolina Certified Well Contractor shall abandon the
monitoring wells according to the North Carolina Well Construction Standards (15A NCAC 02C
.0113) and local county rules.
2. No later than six months prior to the expiration of this permit, the Permittee shall request renewal of
this permit on official Division forms. Upon receipt of the request, the Division will review the
adequacy of the facilities described therein, and if warranted, will renew the permit for such period of
time and under such conditions and limitations as it may deem appropriate. PIease note Rule 15A
NCAC 02T .0105(d) requires an updated site map to be submitted with the permit renewal
application.
In accordance with 15A NCAC 02H .0404(e), if the subject wastewater treatment, infiltration, or
reclaimed water irrigation facilities are in noncompliance with the terms and conditions of this permit,
governing statutes or regulations, the subject facilities shall be connected to an operational publicly
owned wastewater collection system within 180 days of its availability. Prior to the initiation of these
connection activities, appropriate Division approval shall be received.
H. PERFORMANCE STANDARDS
1. The subject non -discharge facilities shall be effectively maintained and operated at all times so there
is no discharge to surface waters, nor any contravention of groundwater or surface water standards_
In the event the facilities fail to perform satisfactorily, including the creation of nuisance conditions
due to improper operation and maintenance, or failure of the infiltration or irrigation areas to
adequately assimilate the effluent, the Permittee shall take immediate corrective actions including
Division required actions, such as the construction of additional or replacement wastewater treatment
or infiltration/irrigation facilities.
2. This permit shall not relieve the Pertnittee of their responsibility for damages to groundwater or
surface water resulting from the operation of this facility.
3. All wells constructed for purposes of groundwater monitoring shall be constructed in accordance with
15A NCAC 02C .0108 (Standards of Construction for Wells Other than Water Supply), and any other
jurisdictional laws and regulations pertaining to well construction.
4. Effluent quality shall not exceed the limitations specified in Attachment A_
5. Application rates, whether hydraulic, nutrient or other pollutant, shall not exceed those specified in
Attachment B.

6. The compliance boundary for the high -rate infiltration and spray bed disposal systems shall be
specified in accordance with 15A NCAC 02L .0107(b). This disposal system was individually
permitted on or after December 30, 1983; therefore, the compliance boundary is established at either
250 feet from the effluent disposal area, or 50 feet within the property boundary, whichever is closest
to the effluent disposal area. An exceedance of groundwater standards at or beyond the compliance
boundary is subject to remediation action according to 15A NCAC 02L .0106(d)(2) as well as
enforcement actions in accordance with North Carolina General Statute 143-215.6A through 143-
215.6C.
In accordance with 15A NCAC 02L .0108, the review boundary for the high -rate infiltration and
spray bed disposal systems is established midway between the compliance boundary and the effluent
disposal area. Any exceedance of groundwater standards at the review boundary shall require action
in accordance with 15A NCAC 02L .0106.
For reclaimed water irrigation areas, the compliance and review boundaries are established at the
property boundary. Any exceedance of standards at the compliance or review boundary shall require
action in accordance with 15A NCAC 02L .0106.
8. No direct cross -connections shall be allowed between reclaimed water and potable water systems,
including potable water irrigation wells connected to reclaimed water irrigation system, unless such
connection has been approved by the Department pursuant to 15A NCAC 18C .0406.
9. The Permittee shall apply for a permit modification to establish a new compliance boundary prior to
any sale or transfer of property affecting a compliance boundary.
10. in accordance with 15A NCAC 02L .0107(d), no wells, excluding Division approved monitoring
wells, shall be constructed within the compliance boundary except as provided for in 15A NCAC 02L
.0107(g).
11. Except as provided for in 15A NCAC 02L .0107(g), the Permittee shall ensure any landowner who is
.not the Permittee and owns Iand within the compliance boundary shall execute and file with the
Currituck County Register of Deeds an easement running with the land containing the following
items:
a. A notice of the permit and number or other description as allowed in 15A NCAC 02L .0107(f)(1);
b. Prohibits construction and operation of water supply wells within the compliance boundary; and
c. Reserves the right of the Permittee or the State to enter the property within the compliance
boundary for purposes related to the permit.
The Director may terminate the easement when its purpose has been fulfilled or is no longer needed.

12. The facilities permitted herein shall be constructed and maintained according to the following
setbacks:
a. The setbacks for high -rate infiltration sites shall be as follows (all distances in feet):
i. Any private or public water supply source: 100
ii. Surface waters: 200
iii. Groundwater lowering ditches: 200
iv. Surface water diversions: 50
v. Any well with exception of monitoring wells: 100
vi. Any property line: 50
b. The setbacks for reclaimed utilization sites shall be as follows (all distances in feet):
i. Surface waters not classified SA: 25
ii. Surface waters classified SA: 100
iii. Any well with exception to monitoring wells: 100
c. The setbacks for treatment units shall be as follows (all distances in feet):
i. Any habitable residence or place of public assembly under separate ownership: 100
ii. Any private or public water supply source: 100
iii. Surface waters: 50
iv. Any well with exception of monitoring wells: 100
v. Any property line: 50
d. The setbacks for final effluent (reclaimed) storage units shall be as follows (all distances in feet):
i. Any private or public water supply source: 100
ii. Surface waters: 50
iii. Any well with exception of monitoring wells: 100
iv. Any habitable residence or place of public assembly under separate ownership: 100
M. OPERATION AND MAINTENANCE REQUIREMENTS
1. The facilities shall be properly maintained and operated at all times. The facilities sha1l be effectively
maintained and operated as a non -discharge system to prevent the discharge of any wastewater
resulting from the operation of this facility. The Permittee shall maintain an Operation and
Maintenance Plan pursuant to 15A NCAC 02T .0707, which at a minimum shall include operational
functions, maintenance schedules, safety measures and a spill response plan_
2. Upon the Water Pollution Control System Operators Certification Commission's (WPCSOCC)
classification of the subject non -discharge facilities, in accordance with 15A NCAC 08G .0200 the
Permittee shall designate and employ a certified operator in responsible charge (ORC) and one or
more certified operator(s) as back-up ORC(s). The ORC or their back-up shall visit the facilities in
accordance with 15A NCAC 08G .0200, and shall comply with all other conditions specified in the
previously cited rules.
3. Only effluent from Pine Island / Currituck Club shall be infiltrated or irrigated on the sites listed in
Attachment B_
4. The cationic backwash water, anionic backwash water, and supernatant from the iron filter backwash
at the Pine Island Water Treatment Plant (Permit No. WQ0013006) will be pumped to the 5 day upset
system for treatment.

5. An automatically activated standby power source capable of powering all essential treatment units
shall be on site and operational at all times. If a generator is employed as an alternate power supply,
it shall be tested weekly by interrupting the primary power source.
6. No automobiles or machinery shall be allowed on the infiltration sites except during equipment
installation or while maintenance is being performed.
7. Public access to the infiltration sites and wastewater treatment facilities shall be prohibited.
8. The residuals generated from the wastewater treatment facilities shall be disposed or utilized in
accordance with 15A NCAC 02T .1100. The Permittee shall maintain a residual management plan
pursuant to 15A NCAC 02T .0708.
9. Diversion or bypassing of untreated or partially treated wastewater from the treatment facilities is
prohibited.
10. A protective vegetative cover shall be established and maintained on all earthen embankments (i.e.,
outside toe of embankment to maximum allowable temporary storage elevation on the inside of the
embankment), berms, pipe runs, erosion control areas, and surface water diversions. Trees, shrubs,
and other woody vegetation shall not be allowed to grow on the earthen dikes or embankments.
Earthen embankment areas shall be kept mowed or otherwise controlled and accessible.
11. Adequate measures shall be taken to prevent reclaimed water ponding in or runoff from the irrigation
sites.
12_ In accordance with 15A NCAC 02U .0501, the Permittee shall provide notification to the public and
employees about the use of reclaimed water, and that reclaimed water is not intended for drinking.
Notification of use of reclaimed water at the Currituck club shall be posted in an area visible to all
golfers.
13. All effluent shall be routed to the five day upset pond should the limit for fecal coliform (e.g., daily
maximum concentration of 25 colonies per 100 mL) or turbidity (e.g., instantaneous maximum of 10
NTU) be exceeded, until the problems associated with the wastewater treatment plant have been
corrected. The wastewater in the five day upset pond shall be pumped back to the treatment plant
headworks for re -treatment (or treated in place) prior to infiltration.
14. The infiltration basin and spray bed shall be periodically dredged and/or raked to remove deposited
materials and vegetation that may impede the infiltration process. Dredging records shall be
maintained at the facility for a period of no less than five years, and shall be made available to the
Division upon request. The Washington Regional Office, telephone number (252) 946-6481, shall be
notified prior to dredging.
I5. Application of chemicals to the high rate infiltration pond and spray bed is strictly prohibited.
IV. MONITORING AND REPORTING REQUIREMENTS
1. Any Division required monitoring (including groundwater, plant tissue, soil and surface water
analyses) necessary to ensure groundwater and surface water protection shall be established, and an
acceptable sampling reporting schedule shall be followed.
2. Per 15A NCAC 02H .0800, a Division certified laboratory shall conduct all laboratory analyses for
the required effluent, groundwater or surface water parameters.

3. Flow through the treatment facility shall be continuously monitored, and daily flow values shall be
reported on Form. NDMR.
The Permittee shall install and maintain an appropriate flow measurement device to ensure the
accuracy and reliability of flow measurement consistent with accepted engineering and scientific
practices. Selected flow measurement devices shall be capable of measuring flows with a maximum
deviation of less than ten percent from true flow; accurately calibrated at a minimum of once per year;
and maintained to ensure the accuracy of measurements is consistent with the selected device's
accepted capability. The Permittee shall maintain records of flow measurement device calibration on
file for a period of at least five years. At a minimum, documentation shall include:
a. Date of flow measurement device calibration,
b. Name of person performing calibration, and
c. Percent from true flow.
4. Permittee shall monitor the effluent from the subject facilities at the frequencies and locations for the
parameters specified in Attachment A.
5. The Permittee shall maintain adequate records tracking the amount of reclaimed water distributed to
the reclaimed water storage pond and shall include the following information:
a. Date of reclaimed water distributed; and
b. Volume of reclaimed water distributed to reclaimed storage pond pond (monthly total).
Monthly tracking records shall be summed and reported on Form NDMR (see Attachment A) under
parameter WQO1 (Flow, Reclaimed Water Distributed). This value shall represent the total volume
of reclaimed water distributed to the storage pond for that month.
6. The Perrnittee shall maintain adequate records tracking the actual amount of reclaimed water utilized
from the reclaimed water storage pond. Records shall be maintained for a minimum of five years. At
a minimum, these records shall include the following information for each utilization site listed in
Attachment B:
a. Date of reclaimed water utilization at golf course;
b. Continuous weekly, monthly, and year-to-date hydraulic (inches/acre) loadings;
c. TotaI volume irrigated on a monthly basis; and
d_ Weather conditions;
7. The Permittee shall maintain adequate records tracking the amount of effluent infiltrated at high rate
infiltration pond and spray bed. At a minimum, these records shall include the following information
for each infiltration site Iisted in Attachment B:
a. Date of infiltration;
b. Volume of effluent infiltrated;
c. Loading rates to each infiltration site listed in Attachment B;
d. Weather conditions;

8. Three copies of all monitoring data (as specified in Conditions IV.3., IVA.. and IV.5.) on Form
NDMR for each PPI and three copies of all operation and disposal records (as specified in Condition
IV.5.) on Form NDAR-1 (as specified in Condition IV.6.) and Form NDAR-2 (as specified in
Conditions IV.5. and IV.7.) for every site in Attachment B shall be submitted on or before the last day
of the following month. If no activities occurred during the monitoring month, monitoring reports are
still required documenting the absence of the activity. All information shall be submitted to the
following address:

9. Pursuant to § 143-215.1C., the Permittee shall provide to its users and the Division of Water
Resources an annual report summarizing the performance of the wastewater treatment, infiltration and
reclaimed water irrigation facility and the extent to which the facility has violated this permit, or
federal or State laws, regulations, or rules related to the protection of water quality. This report shall
be prepared on either a calendar or fiscal year basis and shall be provided no later than 60 days after
the end of the calendar or fiscal year. Two copies of the annual report provided to the Perrnittee's
users shall be submitted to:

10. A record shall be maintained of all residuals removed from this facility. This record shall be
maintained at the facility for a period of no less than five years, and shall be made available to the
Division upon request. At a minimum, this record shall include:
a. Name of the residuals hauler;
b. Non -Discharge permit number authorizing the residuals disposal, or a letter from a municipality
agreeing to accept the residuals;
c. Date the residuals were hauled; and
d. Volume of residuals removed.
11. A maintenance log shall be maintained at this facility. This log shall be maintained at the facility for
a period of no less than five years, and shall be made available to the Division upon request. At a
minimum, this log shall include:
a. Date and results of power interruption testing on alternate power supply;
b. Date of calibration of flow measurement device;
c. Visual observations of the plant and plant site; and
d. Record of preventative maintenance (e.g., changing of equipment, adjustments, testing,
inspections and cleanings, etc.).
12. Two copies of the monitoring well sampling and analysis results shall be submitted on a Compliance
Monitoring Form (GW-59), along with attached copies of Iaboratory analyses, on or before the last
working day of the month following the sampling month. The Compliance Monitoring Norm (GW-
59) shall include this permit number, the appropriate well identification number, and one GW-59a
certification form shall be submitted with each set of sampling results. All information shall be
submitted to the following address:

13. Noncompliance Notification:
V.
The Permittee shall report by telephone to the Washington Regional Office, telephone number (252)
946-9215, as soon as possible, but in no case more than 24 hours, or on the next working day
following the occurrence or first knowledge of the occurrence of any of the following:
a. Any occurrence at the facility resulting in the treatment of significant amounts of wastes that is
abnormal in quantity or characteristic, including the known passage of a hazardous substance.
b. Any process unit failure (e.g., mechanical, electrical, etc.), due to known or unknown reasons,
rendering the facility incapable of adequate wastewater treatment.
c. Any facility failure resulting in a by-pass directly to receiving surface waters.
d. Any time self -monitoring indicates the facility has gone out of compliance with its permit
limitations.
e. Effluent breakout from the infiltration basin(s).
Any emergency requiring immediate reporting (e.g., discharges to surface waters, imminent failure of
a storage structure, etc.) outside normal business hours shall be reported to the Division's Emergency
Response personnel at telephone number (800) 662-7956, (800) 858-0368, or (919) 733-3300.
Persons reporting such occurrences by telephone shall also file a written report in letter form within
five days following first knowledge of the occurrence. This report shall outline the actions taken or
proposed to be taken to ensure the problem does not recur.
1. The Permittee shall provide adequate inspection and maintenance to ensure proper operation of the
wastewater treatment, infiltration, and reclaimed water facilities.
2. The Permittee or their designee shall inspect the wastewater treatment, infiltration, and reclaimed
water facilities to prevent malfunctions, facility deterioration and operator errors resulting in
discharges, which may cause the release of wastes to the environment, a threat to human health or a
public nuisance. The Permittee shall maintain an inspection log that includes, at a minimum, the date
and time of inspection, observations made, and any maintenance, repairs, or corrective actions taken.
The Permittee shall maintain this inspection log for a period of five years from the date of the
inspection, and this Iog shall be made available to the Division upon request_
3. Any duly authorized Division representative may, upon presentation of credentials, enter and inspect
any property, premises or place on or related to the wastewater treatment, infiltration, and reclaimed
water facilities permitted herein at any reasonable time for the purpose of determining compliance
with this permit; may inspect or copy any records required to be maintained under the terms and
conditions of this permit, and may collect groundwater, surface water or leachate samples.
V1. GENERAL CONDITIONS
1. Failure to comply with the conditions and limitations contained herein may subject the Permittee to
an enforcement action by the Division in accordance with North Carolina General Statutes 143-
215.6A to 143-215.6C.
2. This permit shall become voidable if the permitted facilities are not constructed in accordance with
the conditions of this permit, the Division approved plans and specifications, and other supporting
documentation.

3. This permit is effective only with respect to the nature and volume of wastes described in the permit
application, Division approved plans and specifications, and other supporting documentation. No
variances to applicable rules governing the construction or operation of the permitted facilities are
granted, unless specifically requested and approved in this permit pursuant to 15A NCAC 02T
.0105(n).
4. The issuance of this permit does not exempt the Permittee from complying with any and all statutes,
rules, regulations, or ordinances, which may be imposed by other jurisdictional government agencies
(e.g., local, state, and federaI). Of particular concern to the Division are applicable river buffer rules
in 15A NCAC 02B .0200; erosion and sedimentation control requirements in 15A NCAC Chapter 4
and under the Division's General Permit NCG010000; any requirements pertaining to wetlands under
15A NCAC 02B .0200 and 02H .0500; and documentation of compliance with Article 21 Part 6 of
Chapter 143 of the General Statutes.
5. In the event the permitted facilities change ownership or the Permittee changes their name, a formal
permit modification request shall be submitted to the Division. This request shall be ;made on official
Division forms, and shall include appropriate property ownership documentation and other supporting
documentation as necessary. The Permittee of record shall remain fully responsible for maintaining
and operating the facilities permitted herein until a permit is issued to the new owner.
6. The Permittee shall retain a set of Division approved plans and specifications for the life of the
facilities permitted herein.
7. The Permittee shall maintain this permit until all permitted facilities herein are properly closed or
permitted under another permit issued by the appropriate permitting authority pursuant to 15A NCAC
02T .01050).
8. This permit is subject to revocation or unilateral modification upon 60 days notice from the Division
Director, in whole or part for the requirements listed in 15A NCAC 02T .0110.
9. Unless the Division Director grants a variance, expansion of the permitted facilities contained herein
shall not be granted if the Permittee exemplifies any of the criteria in 15A NCAC 02T .0120(b).
10. The Permittee shall pay the annual fee within 30 days after being billed by the Division. Failure to
pay the annual fee accordingly shall be cause for the Division to revoke this permit pursuant to 15A
NCAC 02T .0105(e)(3).

I. Annual monitoring shall be conducted in November.
2. The measurement of water levels shall be made prior to purging the wells. The depth to water in each well shall be measured from the surveyed point on the top of the casing. The measurement
of pH shall be made after purging and prior to sampling for the remaining parameters.
3. The measuring points (top of well casing) of all monitoring wells shall be surveyed to provide the relative elevation of the measuring point for each monitoring well. The measuring points (top
of casing) of all monitoring wells shall be surveyed relative to a common datum.
41. Volatile Organic Compounds (VOC) - In November only, analyze by one of the following methods:
a. Standard Method 6230D, PQL at 0.5 µg/L or less
b. Standard Method 6210D, PQL at 0.5 µg/L or less
c. EPA Method 8021, Low Concentration, PQL at 0.5 µg/L or less
d. EPA Method 8260, Low Concentration, PQL at 0.5 µg/L or less
e. Another method with prior approval by the Water Quality Permitting Section Chief
Any metliod used must meet the following qualifications:
EL A laboratory must be DWR certified to run any method used.
b. The method used must, at a minimum, include all the constituents listed in Table V111 of Standard Method 6230D.
c. The inethod used must provide a PQL of 0.5 µg/L or less that must be supported by laboratory proficiency studies as required by DWR. Any constituents detected above the MDL but below
the PQL of 0.5 µg/L must be qualified (estimated) and reported.
5, if any volatile organic compounds (VOC) are detected as a result of monitoring as provided in Attachment C, then the Washington Regional Office supervisor must be contacted immediately for
further instructions regarding any additional follow-up analyses required.
6. If TOC concentrations greater than 10 mg/L are detected in any downgradient monitoring well, additional sampling and analysis must be conducted to identify the individual constituents
comprising this TOC concentration. If the TOC concentration as measured in the background monitor well exceeds 10 mg/L, this concentration will be taken to represent the naturally occurring
TOC concentration. Any exceedances of this naturally occurring TOC concentration in the downgradient wells shall be subject to the additional sampling and analysis as described above.
Monitoring wells shall be reported consistent with the nomenclature and location Information provided in this attachment.
