This is in response to your letter dated August 26, 1996 concerning the land application residuals
for the Town of Middlesex under Non -Discharge Permit No. WQ0004801.
Permit No. WQ0004801 was issued on March 12, 1992 and expires February 29, 1997. Permit
Condition No. VI 10 states, "The Permittee, at least six (6) months prior to the expiration of this permit,
shall request its extension. Upon receipt of the request, the Commission will review the adequacy of the
acilities described therein, and if warranted, will extend the permit for such period of time and under such
.;onditions and limitations as it may deem appropriate." Failure to apply for the renewal of the permit may
subject the Permittee to possible enforcement actions. In addition, the full $400.00 permit processing fee
will be required for late renewal. A courtesy reminder notice was sent to the Permittee eight months prior
to the expiration of the permit to remind them that the permit needed to be renewed. The Division should
have been notified prior to the required six months if there were problems. Therefore, the Division will
not allow a sixty day extension.
Please be advised, the Division will accept residuals analysis that are no older than six months.
Therefore, the last quarterly analysis run for the Town of Middlesex would be adequate for submittal with
the renewal request without waiting any further for the next quarter.
