The Division's Land Application Unit has received your signed and sealed certification for the subject
facility. The Division hereby accepts this certification based upon its execution and submittal by a North
Carolina Professional Engineer.
In accordance with the engineer's final certification received on July 31, 2008, We are forwarding
herewith the corrected Page 1 of Permit No. WQ0004797, issued December 29, 2006, for the subject
facility. Page 1 has been modified to reflect the actual irrigation areas presented in the engineer's final
certification

In accordance with the provisions of Article 21 of Chapter 143, General Statutes of North Carolina as
amended, and other applicable Laws, Rules, and Regulations
PERMISSION IS HEREBY GRANTED TO

FOR THE
continued operation of an average 83,000 -GPD spray irrigation treatment and disposal facility consisting
of a separation basin, a 100 -GPM pump station, approximately 2,200 linear feet of 6 -inch force main, a
2.5 -MG aerated lagoon, a 475 -GPM spray irrigation pump and a 50.49 -acre spray irrigation field and all
associated piping, valves, controls, spray heads and appurtenances
construction of one 40 hp 475 -GPM spray irrigation pump and approximately 600 feet of 8 -inch force
main and all associated piping, valves, controls, and appurtenances
upon successful completion of the preliminary pumping pilot test, construction of one 40 hp 475 -GPM
transfer pump, approximately 650 linear feet of 4 -inch force main, a 1.75 MG above ground wastewater
storage tank, a Rosemount magnetic flowmeter or equivalent, and all associated piping, valves, controls,
and appurtenances to increase the facility's capacity to 137,000 GPD.
to serve the Clement Pappas Mountain Home Facility, with no discharge of wastes to the surface waters,
pursuant to the application received August 28, 2006, and in conformity with the project plan,
specifications, and other supporting data subsequently filed and approved by the Department of
Environment and Natural Resources and considered a part of this permit.
This permit shall be effective from the date of issuance until November 30, 2011, shall void Permit
No. WQ0004797 issued July 3, 2002, and shall be subject to the following specified conditions and
limitations:
I. PERFORMANCE STANDARDS
1. Upon completion of construction and prior to operation of this permitted facility, a certification must
be received from a professional engineer certifying that the permitted facility has been installed in
accordance with this permit, the approved plans and specifications, and other supporting materials. If
this project is to be completed in phases and partially certifiedyou shall retain the responsibilfty to
track further construction approved under the samepen-nit,.and shall provide a final certificate of
completion once the entire prof pct has been completed. Mail the Certification to the Land
Application Unit, Division of Water Quality, 1636 Mail Service Center, Raleigh, NC 27699-1636.
