Regional Office which confirms that the permittee has never utilized this land application
permit. Therefore, as you requested, Permit No. WQ0004123 is hereby rescinded, _
effective immediately. -

If in the future, you determine that you wish to operate a nondischarge wastewater
treatment system, you must fust apply for and receive a new Nondischarge Permit.
Operation of a wastewater system without a valid Nondischarge Permit will subject the
owner to a civil penalty of up to $10,000 per day. n
