In accordance with your permit modification application package received on April 16, 2008 we
are forwarding herewith a modified Attachment B, certified May 9, 2008, to the City of Kinston
for the subject residuals land application program.
This certification is being issued to modify Attachment B by the addition of 189.8 acres located
in Jones County. The additional application area consists of 13 new fields designated as site NC-
JO-1; fields 1 to 9, 10A, 1013, 11, and 12; and 6 modified fields designated NC-JO-8790, fields
4A, 4B, 4C and 8 to 10. Please note that the redefined Compliance Boundary for the new and
modified sites/fields will be a distance of 100 feet from the land application area or 50 feet
within the property boundary, whichever is closer to the land application area.
Please replace the existing Attachment B of Permit No. WQ0003919 that was previously
certified on July 3, 2006, with this modified Attachment B. Note that only the vicinity and
buffer maps for any new and modified land application sites are attached to this modified
Attachment B. It is the Permittee's responsibility to ensure that a complete permit is created by
inserting this modified Attachment B and vicinity and buffer maps into Permit No.
WQ00003919, issued July 3, 2006, and making sure to retain any vicinity and buffer maps for
any existing and unmodified land application sites.

This modified Attachment B shall be effective from the date of certification; shall void
Attachment B of Permit No. WQ0003919 that was previously certified on July 3, 2006; and shall
be subject to the conditions and limitations as specified therein as well as those specified in the
above -referenced permit. As always, remember to take the time to review this modified
Attachment B thoroughly, as some of the conditions contained therein may have been changed
since the last certification. Note that this modified Attachment B is hereby incorporated by
reference into Permit No. WQ0003919, issued on July 3, 2006.
If any parts, requirements, and/or limitations contained in this certification or modified
Attachment B are unacceptable, you have the right to request an adjudicatory hearing upon
written request within 30 days following receipt of the certification. This request shall be in the
form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes
and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, NC
27699-6714. Unless such demands are made, this permit shall be final and binding. If you

Any field identified with a GW-B classification in prior Land Application ofResidual Solids Permit prohibiting land application events from Decembcr through Marcie, inclusive, and when the vertical separation between the depth of residuals
land application and the weer table is lass than three feet 15A NCAC 2T .1109 Operation and Management Practices (bxl X1) specifies that application of biaik residuals is prohibited when the vemcal separation between the seasonal high
water table and the depth of residuals application is less than one foot By issuance of this modified Attachment B, the GW-B classification and seasonal restrictions are removed from fields previously designated OW-B. Vertical separation
requirements specified in 15A NCAC ZT .1109 must be satisfied at the time of any residuals application event - one foot a!'vertical separation must be prrsent.
