CAULEY, JR., WWTP SUPERINTENDENT CITY OF KMSTON POST OFFICE DRAWER 339 KINSTON, NORTH CAROLINA 28502 Subject: Attachment A Certification Permit No. WQ0003919 City of Kinston City of Kinston Residuals Land Application Program Land Application of Residual Solids (503) Lenoir County Dear Mr. Cauley: In accordance with your permit modification application package received on January 21, 2005, we are forwarding herewith a modified Attachment A, certified August 24, 2005, to the City of Kinston. for the subject residuals land application program. This certification is being issued to modify Attachment A to increase the volume of residuals that may be land applied through this Iand application program from 1,000 dry tons per day to 1,500 dry tons per day. This volume represents the total volume of residuals generated by the Kinston Regional Water Reclamation Facility (WRF) as well as those generated at the Permittee's Peachtree WWTP, which are pumped to the Kinston Regional WRF for further treatment, processing, and storage prior to land application. The Permittee should pay particular attention to the monitoring and reporting requirements to guarantee permit and 40 CFR Part 503 compliance. Even though this Attachment A has also been modified to remove some specified sampling and operational requirements that were previously stipulated, it appears as if there is a complicated residuals treatment/storage operation at the Permittee's two wastewater treatment plants, and the Division of Water Quality was unsure from the submitted sampling/monitoring plan how the sampling/monitoring is to be completed such that it is representative of the residuals to be land applied and is in compliance with 40 CFR Part 503. As the transition is made to this new operation, the Division recommends that the residuals sampling/monitoring plan be reviewed for these criteria. Please replace the existing Attachment A of Permit No. WQ0003919 that was previously certified on October 18, 2000 (i.e., as reissued on June 4, 2004), with this modified Attachment A. Note that only the vicinity maps for any new and modified residuals source -generating facilities are attached to this modified Attachment A. It is the Permittee's responsibility to ensure that a complete permit is created by inserting this modified Attachment A and vicinity maps into Permit No. WQ0003919, issued on October 18, 2000 (i.e., as reissued on June 4, 2004), and making sure to retain any vicinity maps for any existing and unmodified residuals source -generating facilities. This modified Attachment A shall be effective from the date of certification; shall void Attachment A of Permit No. WQ0003919 that was previously certified on October 18, 2000 (i.e., as reissued on June 4, 2004); and shall be subject to the conditions and limitations as specified therein and well as those specified in the above -referenced permit. As always, remember to take the time to review this modified Attachment A thoroughly, as some of the conditions contained therein may have been changed since the last certification. Note that this modified Attachment A is hereby incorporated by reference into Permit No. WQ0003919, issued on October 18, 2000 (i.e., as reissued on June 4, 2004). If any parts, requirements, and/or limitations contained in this certification or modified Attachment A are unacceptable, you have the right to request an adjudicatory hearing upon written request within 30 days following receipt of the certification. This request shall be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes and filed with the Office of Administrative Hearings at 6714 Mail Service Center, Raleigh, North Carolina 27699-6714. Unless such demands are made, this permit shall be final and binding.
