Hunt, Jr., Governor Jonathan B. Howes, Secretary A. Preston Howard, Jr., P.E., Director June 26, 1996 Y1.V;VA I'T A&4 C)EHNR Lee K. Allen, Craven County Water and Sewer Board of Directors Neuse River Water & Sewer District 412 Craven Street New Bern, North Carolina 28560 Subject: Permit No. WQ0003765 Neuse River Water & Sewer District Wastewater Collection, Treatment and Spray Irrigation System Craven County Dear Mr. Allen: In accordance with the renewal request received February 14, 1996, we are forwarding herewith Permit No. WQ0003765, dated June 26, 1996, to the Neuse River Water & Sewer District for the continued operation of the subject wastewater collection, treatment, and spray irrigation facilities. In accordance with the water balance model completed by McKim & Creed Engineering for the revised Facilities 201 Plan for the Neuse River Water & Sewer District, the permitted capacity for the subject wastewater treatment facility is being revised downward from 350,000 GPD to 270,000 GPD. The land available for spray irrigation can presently only treat and dispose of 270,000 GPD, therefore, the reduction in disposal capability for the system. However, the Division acknowledges that the treatment lagoons are sized to treat the full flow of 350,000 GPD. Once Neuse River Water and Sewer Authority acquires additional land for spray irrigation, the permit can be amended to add this new acreage for spray irrigation and increase the design flow rate at that time. This permit shall be effective from the date of issuance until May 31, 2001, shall void Permit No. WQ0003765 issued September 8, 1995, and shall be subject to the conditions and limitations as specified therein. Please pay particular attention to the monitoring requirements in this permit. Failure to establish an adequate system for collecting and maintaining the required operational information will result in future compliance problems. If any parts, requirements, or limitations contained in this permit are unacceptable, you have the right to request an adjudicatory hearing upon written request within thirty (30) days following receipt of this permit. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative -Hearings, P.O. An exceedance of Groundwater Quality Standards at or beyond the Compliance Boundary is subject to immediate remediation action in addition to the penalty provisions applicable under General Statute 143-215.6A(a)(1). In accordance with I5A NCAC 2L, a REVIEW BOUNDARY is established around the disposal systems midway between the Compliance Boundary and the perimeter of the waste disposal area. Any exceedance of standards at the Review Boundary shall require remediation action on the part of the permittee. V . INSPECTIONS 1. Adequate inspection, maintenance, and cleaning shall be provided by the Permittee to insure proper operation of the subject facilities. 2. The Permittee or his designee shall inspect the wastewater treatment and disposal facilities to prevent malfunctions and deterioration, operator errors and discharges which may cause or lead to the release of wastes to the environment, a threat to human health, or a nuisance. The Permittee shall keep an inspection log or summary including at least the date and time of inspection, observations made, and any maintenance, repairs, or corrective actions taken by the Permittee. This log of inspections shall be maintained by the Permittee for a period of three years from the date of the inspection and shall be made available upon request to the Division of Environmental Management or other permitting authority,. 3. Any duly authorized officer, employee, or representative of the,Division of Environmental Management may, upon presentation of credentials, enter and inspect any property, premises or place on or related to the disposal site or facility at any reasonable time for the purpose of determining compliance with this permit, may inspect or copy any records that must be maintained under the terms and conditions of this permit, and may obtain samples of groundwater, surface water, or leachate. CLASSIFICATION The following systems shall be assigned a Class I classification, unlgP 5 the flow is of a significant quantity or the technology is unusually complex, to require consideration by the Commission on a case -by -case basis: (Check d Appropriate) 1. Oil/water Separator Systems consisting only of physical separation, pumps and disposal; 2. Seplic Tank/Sand Filter Systems consisting only of septic tanks, dosing apparatus, pumps, sand filters, disinfection and direct discharge; 3. Lagoon Systems consisliag only of preliminary treatment, lagoons, pumps, disinfection, necessary chemical treatment for algae or nutrient control, and direct discharge; 4. Ciosed-loop Recycle Systems; 5. Groundwater Remediation Systems consisting only of o0waler separators, pumps, air -stripping, carbon adsorption, disinfection and dispo -J; 6. Aquacufture operations with discharge to surface waters; ? Water Plant sludge handling and back -wash water treatment: 3, Seafood processing consisting of screening and disposal. 9. Single-family discharging systems, with the exception of Aerobic Treatment Units, will be classified if permitted after July 1, 1993 or if upon inspection by the Division, h is found that the system is not being adequately operated or maintained. Such systems will be notified of the classification or reclassification by the Commission, in writing.
