We acknowledge receipt of plans submitted on January 9, 1985, indicating
that you have decided to specify a submersible pump for pumping grease from the
grease removal chamber in the subject facility rather than install a pneumatic
ejector. We have no objections to this change.
This is a minor modification not requiring issuance of an amendement to
Permit No. 11066.

Dear Mr. Matkins:
In accordance with your application received August 10, 1984, we are
forwarding herewith Permit No. 11066, dated December 17, 1984 to the Matkins
Meat Processors, Inc. for the construction and operation of the subject non -
discharge wastewater treatment and disposal facilities.
This permit shall be effective from the date of issuance until December
31, 1989, and shall be subject to the conditions and limitations as specified
therein.
If any parts, requirements or limitations contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing before a
hearing officer upon written demand to the Director within thirty (30) days
following receipt of this permit, identifying the specific issues to be con-
tended. Unless such demand is made, this permit shall be final and binding.
Please be aware that any future transfer of ownership and maintenance respon-
sibility of these facilities must be approved by this Division by requesting
permit reissuance, since this permit is non -transferable.
One (1) set of approved plans and specifications is being forwarded to
you. If you have any questions or need additional information concerning this

For the Discharge of Sewage, Industrial Wastes, Or Other Wastes
In accordance with the provisions of Article 21 of Chapter 143, General
Statutes of North Carolina as amended, and other applicable Laws, Rules and
Regulations
PERMISSION IS HEREBY GRANTED TO
Matkins Meat Processors, Inc.
Caswell County
FOR THE
constructiop and operation of a non -discharge type wastewater treatment and disposal
facility consisting of a bar screen, grease removal chamber with pneumatic ejector,
two (2) aerated lagoons in series with a combined capacity of 412,000 gallons,
a disinfection chamber with chlorine solution feed pump, an effluent dosing station
with simplex suction lift pump, a spray irrigation system with eighteen (18) sprinklers
discharging onto a total irrigation area of 3.92 acres, and all associated piping,
valves and appurtenances to serve the Matkins Meat Processors,Inc. plant with
no discharge of wastewater to the surface waters of the State,
pursuant to the application received August 10, 1984, and in conformity with the
project plans, specifications, and other supporting data, subsequently filed and
approved by the Department of Natural Resources and Community Development and
considered a part of this permit.
This Permit shall be effective from the date of issuance until December 31,
1989, and shall be subject to the following specified conditions and limitations:
1. This permit shall become voidable unless the facilities are constructed in
accordance with the approved plans, specifications and other supporting data.
2. This permit is effective only with respect to the nature and volume of wastes
described in the application and other supporting data.
3. The facilities shall be properly maintained and operated at all times.
4. This permit is not transferable.
5. This permit shall become voidable in the event of failure of the soil to
adequately absorb the wastes, and may be rescinded unless the facilities are
installed, maintained and operated in a manner which will protect the assigned
water quality standards of the surface waters and ground waters.

6. In event the facilities fail to perform satisfactorily including the creation
of nuisance conditions, the Permittee shall take such immediate corrective
action as may be required by this Division, including the construction of
additional or replacement wastewater treatment or disposal facilities.
7. The sludge generated from these treatment facilities must be disposed of in
accordance with General Statutes 143-215.1 and in a manner approvable by the
North Carolina Division of Environmental Management.
8. Diversion or bypassing of the untreated wastewater from the treatment and
disposal facilities is prohibited.
9. Irrigation shall, not be performed during inclement weather or when the ground
is in a condition that will cause runoff.
10. A suitable year-round vegetative cover shall be maintained on the spray irrigation
areas.
11. Lime and gypsum shall be applied to the spray irrigation areas as needed to
maintain proper soil pH and to offset high sodium loading.
12. The application rate of treated effluent on the spray fields shall not exceed
1.0 inch`es/acre/week.
13. No traffic or equipment shall be allowed on the disposal area except while ,
installation occurs or whale normal maintenance is being performed.
14. No root or leaf crops for public consumption shall be raised on these sites. s
15. The issuance of this permit shall not relieve Matkins Meat Processors, Inc.
of the responsibility for damages to surface or ground waters resulting from
the operation of this facility.
16. The facilities shall be effectively maintained and operated as a non -discharge
system to prevent the discharge of any wastewater into the surface waters
of the State.
17. No type of wastewater other than that from the Matkins Meat Processors, Inc.
plant shall be sprayed onto the irrigation area.
18. Adequate inspection, maintenance, and cleaning shall be provided by the Matkins
Meat Processors, Inc. to insure proper operation of the subject facilities.
19. The following minimum buffer distances shall be maintained between the wetted
perimeter of the spray irrigation fields and the indicated features and facilities:
a. 150 feet from property lines
b. 200 feet from any existing residence or occupied building
C. 100 feet from any potable water supply well
d. 50 feet from surface waters

20. The holding lagoons shall be constructed of compacted earth of permeability
not greater than 1 x 10-6 cm/sec and the berms shall be stabilized with grass.
21. Freeboard in the holding lagoons shall not be less than two (2) feet
at any time.
22. Any monitoring deemed necessary by the Division of Environmental Management
to insure surface and ground water protection will be established and an
acceptable sampling reporting schedule shall be followed. The following
specific monitoring requirements must be met:
a. At least three (3) monitoring wells shall be established in consultation
with the Division's regional hydrologist from the Winston-Salem Regional
Office, and shall screen the interval between 15 and 20 feet below land
surface.
b. The monitoring wells shall be sampled prior to operation of the wastewater
treatment and disposal facilities and thereafter in the months of March,
July and November for the following parameters: nitrate, ammonia, chloride,
sodium, sulfate, total dissolved solids, total organic carbon, pH, fecal
coliform, conductance, and water levels (the latter measurement to be
made immediately prior to any bailing or pumping for groundwater quality
sapnples) .
C. Soil samples shall be taken semi-annually from the spray irrigation areas
and analyzed for pH and sodium.
d. A record shall be kept of lime and gypsum applications to the spray irrigation
areas.
23. The Permittee shall employ a certified wastewater treatment plant operator
in responsible charge of the wastewater treatment facilities. Such operator
must hold a certificate of the grade at least equivalent to the classification
assigned to the wastewater treatment facilities by the Certification Commission.
24. The Permittee, at least six (6) months prior to the expiration of this Permit,
shall request its extension. Upon receipt of the request, the Commission
will review the adequacy of the facilities described therein, and if
indicated, will extend the Permit for such period of time and under such
conditions and limitations as it may deem appropriate.
